Introducing the Moyo device for intrapartum fetal heart rate monitoring among women with high-risk pregnancies in a low-resource setting: a quasi-experimental study.

OBJECTIVE: To assess the effect of introducing the Moyo device for continuous intrapartum fetal heart rate monitoring (FHRM) on the detection of abnormal fetal heart rate patterns and perinatal mortality. DESIGN: We conducted a quasi-experimental quality improvement project using a before-and-after approach. During the preimplementation phase, intermittent FHRM was done using the existing standard of care (Pinard or a hand-held fetal Doppler). In the postimplementation phase, we introduced the Moyo device (Laerdal Global Health, Stavanger, Norway) for continuous intrapartum FHRM. We used the

Noninvasive Ultrasound Monitoring of Embryonic and Fetal Development in Chinchilla lanigera to Predict Gestational Age: Preliminary Evaluation of This Species as a Novel Animal Model of Human Pregnancy.

Ultrasound is a noninvasive routine method that allows real-time monitoring of fetal development in utero to determine gestational age and to detect congenital anomalies and multiple pregnancies. To date, the developmental biology of Chinchilla lanigera has not yet been characterized. This species has been found to undergo placentation, long gestation, and fetal dimensions similar to those in humans. The aim of this study was to assess the use of high-frequency ultrasound (HFUS) and clinical ultrasound (US) to predict gestational age in chinchillas and evaluate the possibility of this species

[Use of a combination of non stress fetal heart rate monitoring and the oxytocin challenge test in high-risk pregnancies. The effects on perinatal mortality (author's transl)].

A protocol for antepartum supervision which included "non stress fetal monitoring" (NSFM) and the "oxytocin Challenge Test" (OCT) was followed in a series of 640 high-risk pregnancies. The perinatal mortality in this group was compared with that obtained in a group of 3,049 non-selected deliveries which occurred during the same period of time and which were not monitored in the same way. The perinatal mortality which could be attributed to placental insufficiency in the first group (the supervised group) was at 4.68 per 1000, about half that of the non-supervised group (8.72 per 1000) in spite…

Does a non-dipping heart rate profile predict adverse maternal-fetal outcomes? evidence from 24-h ambulatory monitoring in the second half of high-risk pregnancies.

Hypertensive disorders of pregnancy are a leading cause of maternal-fetal morbidity and mortality, with preeclampsia representing a major contributor to adverse outcomes. Ambulatory blood pressure monitoring (ABPM) has improved risk stratification by identifying blood pressure phenotypes, particularly nocturnal hypertension. Given the shared autonomic mechanisms underlying nocturnal blood pressure elevation, a non-dipping heart rate pattern may theoretically provide additional prognostic information. We conducted a retrospective cohort study of 1336 high-risk pregnant women evaluated between 2

Non-Invasive Fetal Monitoring: A Maternal Surface ECG Electrode Placement-Based Novel Approach for Optimization of Adaptive Filter Control Parameters Using the LMS and RLS Algorithms.

This paper is focused on the design, implementation and verification of a novel method for the optimization of the control parameters (such as step size μ and filter order N) of LMS and RLS adaptive filters used for noninvasive fetal monitoring. The optimization algorithm is driven by considering the ECG electrode positions on the maternal body surface in improving the performance of these adaptive filters. The main criterion for optimal parameter selection was the Signal-to-Noise Ratio (SNR). We conducted experiments using signals supplied by the latest version of our LabVIEW-Based Multi-Chan…

The effect of continuous electronic fetal monitoring on mode of delivery and neonatal outcome among low-risk laboring mothers at Debre Markos comprehensive specialized hospital, Northwest Ethiopia.

BACKGROUND: Electronic fetal heart rate monitoring (EFM) has been widely used in obstetric practice for over 40 years to improve perinatal outcomes. Its popularity is growing in Ethiopia and other sub-Saharan African countries to reduce high perinatal morbidity and mortality rates. However, its impact on delivery mode and perinatal outcomes in low-risk pregnancies remains controversial. This study aimed to assess the effect of continuous EFM on delivery mode and neonatal outcomes among low-risk laboring mothers at Debre Markos Comprehensive Specialized Hospital, Northwest Ethiopia. METHODS: A

New Neonatal and Prenatal Approach to Home Therapy with Amoxicillin, Rifaximin, and Anti-Inflammatory Drugs for Pregnant Women with COVID-19 Infections-Monitoring of Fetal Growth as a Prognostic Factor: A Triple Case Series (N.A.T.H.A.N.).

Background: Since the COVID-19 pandemic, managing acute infections in symptomatic individuals, regardless of vaccination status, has been widely debated and extensively studied. Even more concerning, however, is the impact of COVID-19 on pregnant women-especially its effects on fetuses and newborns. Several studies have documented complications in both expectant mothers and their infants following infection. Methods: In our previous works, we provided scientific evidence of the bacteriophage behavior of SARS-CoV-2 (Severe Acute Respiratory Syndrome Coronavirus 2). This demonstrated that a well

Mobil Monitoring Doppler Ultrasound (MoMDUS) study: protocol for a prospective, observational study investigating the use of artificial intelligence and low-cost Doppler ultrasound for the automated quantification of hypertension, pre-eclampsia and fetal growth restriction in rural Guatemala.

INTRODUCTION: Undetected high-risk conditions in pregnancy are a leading cause of perinatal mortality in low-income and middle-income countries. A key contributor to adverse perinatal outcomes in these settings is limited access to high-quality screening and timely referral to care. Recently, a low-cost one-dimensional Doppler ultrasound (1-D DUS) device was developed that front-line workers in rural Guatemala used to collect quality maternal and fetal data. Further, we demonstrated with retrospective preliminary data that 1-D DUS signal could be processed using artificial intelligence and dee

A before and after study of the impact on obstetric and perinatal outcomes following the introduction of an educational package of fetal heart rate monitoring education coupled with umbilical artery lactate sampling in a low resource setting labor ward in South Africa.

INTRODUCTION: Rates of cesarean section (CS) are increasing and abnormal fetal heart rate tracing and concern about consequent acidosis remain one of the most common indications for primary CS. Umbilical artery (UA) lactate sampling provides clinicians with point of care feedback on CTG interpretation and intrapartum care and may result in altered future practice. MATERIALS AND METHODS: From 3rd March - 12th November 2014 we undertook a before and after study in Pretoria, South Africa, to determine the impact of introducing a clinical package of fetal heart rate monitoring education and promp

[Obligatory monitoring methods during delivery].

Fetal monitoring by cardiotocogram (CTG), fetal blood gas analysis (FBA) and pulse oximetry are mostly accepted by obstetricians. Other new methods are in discussion. Cost, benefit and need of the different methods are in continuous controversy. Successful screening by monitoring is focused on early detection and prevention of fetal asphyxia. Through the predictive value of fetal acidosis by CTG is only 30%, the cardiotocogram is the accepted standard method of fetal monitoring. In cases of difficult assessment of intrapartum CTG (4-6%) additional FBA is recommended. Perinatal statistics demon…

Maternal or fetal heart rate? Avoiding intrapartum misidentification.

Electronic fetal monitoring technology is capable of monitoring and recording maternal heart rate (MHR) patterns that mimic fetal heart rate (FHR) patterns. The ability to distinguish one from the other requires knowledge of FHR and MHR characteristics and monitoring technology. Application of this knowledge may prevent fetal injury and death. This article reviews heart rate monitoring technology and the similarities and differences between MHR and FHR baselines, accelerations, and decelerations. Three case reports are described in which the MHR was mistaken for the FHR prior to the diagnosis …

A CNN-RNN unified framework for intrapartum cardiotocograph classification.

BACKGROUND AND OBJECTIVE: Prenatal fetal monitoring, which can monitor the growth and health of the fetus, is very vital for pregnant women before delivery. During pregnancy, it is crucial to judge whether the fetus is abnormal, which helps obstetricians carry out early intervention to avoid fetal hypoxia and even death. At present, clinical fetal monitoring widely used fetal heart rate monitoring equipment. Fetal heart rate and uterine contraction signals obtained by fetal heart monitoring equipment are important information to evaluate fetal health status. METHODS: This paper is based on 1D…

Sensors for Fetal Hypoxia and Metabolic Acidosis: A Review.

This article reviews existing clinical practices and sensor research undertaken to monitor fetal well-being during labour. Current clinical practices that include fetal heart rate monitoring and fetal scalp blood sampling are shown to be either inadequate or time-consuming. Monitoring of lactate in blood is identified as a potential alternative for intrapartum fetal monitoring due to its ability to distinguish between different types of acidosis. A literature review from a medical and technical perspective is presented to identify the current advancements in the field of lactate sensors for th…

Blunt trauma in the obstetric patient: monitoring practices in the ED.

This study was undertaken to determine the usually used approach to fetal monitoring in the emergency department (ED) of the less severely injured obstetric patient who has sustained blunt trauma. A written survey was sent to clinical directors of teaching programs in emergency medicine (EM) with inquiries on the usual way of monitoring, what studies were performed, and the usual disposition of the less-injured obstetric patient. From the 112 teaching programs surveyed in early 1996, there were 87 responses (78%). Seventy-eight percent of programs generally have fetal monitoring performed for …

Fetal paroxysaml supraventricular tachycardia recorded by intrauterine scalp electrocardiography.

Fetal and neonatal paroxysmal supraventricular tachycardia (PST) requires attention because it may be associated with cardiac pathology and neonatal demise. Continuous monitoring of fetal heart rate and the ability to record clearly interpretable electrocardiograms from fetal scalp electrodes makes discovery and diagnoses of these fetal arrhythmias relatively simple. This report describes the discovery of this arrhythmia during routine DOPPLER ultrasound fetal monitoring and its documentation as PST by fetal scalp electrocardiography. (Fig. 1) The fetal heart rate during PST was 300 BPM, highe…
