Cardiac sarcoidosis (CS) is an infiltrative cardiomyopathy characterized by non-caseating granulomatous inflammation that results in significant morbidity and mortality. It has become increasingly recognized over the past several decades; however, diagnosis remains challenging due to its heterogeneous nature and limited diagnostic yield of endomyocardial biopsy. As such, there has been increasing reliance on advanced cardiac imaging to aid in the diagnosis and management of CS. Fluorodeoxyglucose positron emission tomography (18FDG-PET) has emerged as the most robust imaging modality and remai

Cardiac sarcoidosis (CS) poses significant diagnostic and therapeutic challenges due to its heterogeneous clinical manifestations and the limitations of conventional diagnostic approaches. Advances in imaging modalities, particularly cardiac magnetic resonance imaging (CMR) and ¹⁸F-fluorodeoxyglucose positron emission tomography (FDG-PET), have revolutionized the evaluation and management of this complex condition. CMR, with its superior spatial resolution and advanced techniques such as late gadolinium enhancement, T1/T2 mapping, and extracellular volume quantification, offers unparalleled in

18F fluorodeoxyglucose ([18F]FDG) positron emission tomography/computed tomography (PET/CT) has established itself as a critical diagnostic tool in the evaluation of patients with bacteremia and fever of unknown origin (FUO), particularly following futile conventional investigations. These conditions are often challenging due to diverse underlying etiologies, including infections, inflammatory conditions and malignancies. PET/CT has the advantage of being a whole-body imaging technique with high sensitivity for detecting areas of increased metabolism often associated with infection or inflamma

Extragonadal choriocarcinoma in men is an extremely rare and highly aggressive malignancy. Inconclusive biopsies due to a high necrotic component often delay diagnosis. Here is such a case, in which suggestive imaging findings on [18F]FDG PET/CT, a raised level of serum β-human chorionic gonadotropin, and gynecomastia clinched the diagnosis.

Sarcoidosis is a systemic granulomatous disease of unknown etiology, primarily affecting the lungs and the lymphatics. Painless and impalpable diffuse sarcoid myopathy is very rare and may represent a diagnostic challenge even for expert physicians. We present the 18F-FDG PET/CT findings of a 77-year-old woman with muscular sarcoidosis manifested by non-parathyroid-related hypercalcemia. 18F-FDG PET/CT alone among conventional imaging studies provided clues for the diagnosis.

The most common primary orbital cancer in adults is orbital lymphoma. Lymphomas involving the orbit, eyelid, or conjunctiva are described as ocular adnexal lymphoma (OAL). OAL comprises 2% of non-Hodgkin lymphomas but around 8% of extranodal lymphomas. OAL can appear bilaterally in any region of orbit with systemic involvement or restricted to orbit/adnexal region. When identifying systemic involvement from orbital lymphoma and other extranodal lymphomas, PET/CT has been demonstrated to have a higher sensitivity than CT. The predominant subtype of OAL is mucosa-associated lymphoid tissue (MALT

We present a rare case of an isolated splenic myofibroma in a child evaluated with 18 F-FDG PET/CT. An 8-year-old boy underwent PET/CT for a left cervical mass, which incidentally revealed a splenic nodule (1.3×1.0 cm) with markedly increased 18 F-FDG uptake (SUV max =8.1). MRI showed an enhancing, low T2-weighted signal nodule without diffusion restriction. Histopathology after splenectomy confirmed myofibroma. Isolated myofibromas are rare benign spindle cell tumors in children and can demonstrate intense 18 F-FDG uptake, mimicking malignancy. Awareness of this potential false-positive pitfa

PURPOSE: Functional positron emission tomography (fPET) with [18F]FDG allows quantification of stimulation-induced changes in glucose metabolism independent of neurovascular coupling. However, the gold standard for quantification requires invasive arterial blood sampling, limiting its widespread use. Here, we introduce a novel fPET method without the need for an input function.
METHODS: We validated the approach using two datasets (DS). For DS1, 52 volunteers (23.2 ± 3.3 years, 24 females) performed Tetris® during a [18F]FDG fPET scan (bolus + constant infusion). For DS2, 18 participants (24.

Neurolymphomatosis (NL) is an uncommon and rare neurologic disorder characterised by extranodal lymphoma, where the tumour cells invade the cranial nerves, nerve plexus, nerve root, spinal nerve roots, trunk nerves or peripheral nerves. MRI is the modality of choice, but is often challenging in detection of early recurrence, assessing residual disease and response evaluation. 18FFDG PET/CT has superior diagnostic performance compared with body CT in the evaluation of NL. 18F-FDG PET-CT is helpful in evaluation of disease extent and potential to guide biopsy. 18F-FDG PETCT is a highly sensitive

A 72-year-old man presented with a painless penile mass for 3 months. Contrast-enhanced CT revealed heterogeneous enhancement, whereas 18 F-FDG PET/CT displayed inhomogeneous 18 F-FDG accumulation in the lesion without other abnormal activity. The histopathological examination from biopsied specimen confirmed the diagnosis of a plasmacytoma. However, the subsequent tests, including serum/urine immunofixation electrophoresis, serum/urine free light chain assay, and bone marrow smear/biopsy, did not show any abnormalities. The conclusive diagnosis was a solitary extramedullary plasmacytoma of th

OBJECTIVE: The performance of 18 F-FDG PET-based radiomics and deep learning in detecting pathological regional nodal metastasis (pN+) in resectable lung adenocarcinoma varies, and their use across different generations of PET machines has not been thoroughly investigated. We compared handcrafted radiomics and deep learning using different PET scanners to predict pN+ in resectable lung adenocarcinoma.
METHODS: We retrospectively analyzed pretreatment 18 F-FDG PET from 148 lung adenocarcinoma patients who underwent curative surgery. Patients were separated into analog (n = 131) and digital (n

INTRODUCTION: Pre-invasive squamous lesions of the central airways can progress into invasive lung cancers. Identifying these high-risk patients could enable detection of invasive lung cancers at an early stage. In this study, we investigated the value of 18 F-fluorodeoxyglucose (18 F-FDG) positron emission tomography (PET) scans in predicting progression in patients with pre-invasive squamous endobronchial lesions.
METHODS: In this retrospective study, patients with pre-invasive endobronchial lesions, who underwent an 18 F-FDG PET scan at the VU University Medical Center Amsterdam, between J

BACKGROUND: To date, studies on positron emission tomography (PET) with 18 F-fluorodeoxyglucose (FDG) in progressive supranuclear palsy (PSP) usually included PSP cohorts overrepresenting patients with Richardson's syndrome (PSP-RS).
OBJECTIVES: To evaluate FDG-PET in a patient sample representing the broad phenotypic PSP spectrum typically encountered in routine clinical practice.
METHODS: This retrospective, multicenter study included 41 PSP patients, 21 (51%) with RS and 20 (49%) with non-RS variants of PSP (vPSP), and 46 age-matched healthy controls. Two state-of-the art methods for the

Changes in cerebral glucose metabolism after subarachnoid hemorrhage have been rarely described. We present a case with subacute subarachnoid hemorrhage showing unexpected elevated FDG uptake at adjacent cerebral parenchyma on FDG PET/CT. The density of the cerebral parenchyma was normal on CT. The patient received medical management without any neurological complication.

Immunosuppressive therapy with prednisolone (PSL) is the first-line treatment for cardiac sarcoidosis (CS), and 18F-fluoro-2-deoxyglucose positron emission tomography (FDG-PET) is used to evaluate its efficacy to guide treatment. However, the appropriate timing of FDG-PET in CS remains unknown. This single-center, retrospective, observational study included 15 consecutive CS patients who underwent 3 serial FDG-PET scans (at baseline, in the early phase [1-2 months after PSL introduction], and in the late phase [≥ 5 months after PSL introduction with a maintenance dose of PSL]). We adhered to t

Spinal cord sarcoidosis (SCS) is an uncommon disease with estimated incidence of 0.4% in patients with systemic sarcoidosis. Spinal cord sarcoidosis typically manifests late in the disease course and significantly contributes to patient morbidity. Therefore, early SCS diagnosis and prompt initiation of therapy are crucial. Herein, we report the case of a 51-year-old man with symptomatic SCS managed by 18 F-FDG PET/MRI, allowing diagnosis and treatment efficacy evaluation of spine involvement. We believe that the increasing clinical availability of hybrid PET/MRI devices will offer new opportun
