We report a case of Aggregatibacter actinomycetemcomitans (A. actinomycetemcomitans) bacteraemia and secondary brain abscess in a patient where periodontal disease was implicated as the probable source.

Craniocervical necrotizing fasciitis is a rare infectious process that can be life-threatening. It most commonly occurs as a result of a severe dentoalveolar infection. This article reviews the diagnosis, microbiology, anatomy, and pathophysiology behind this infectious process; the incidence; and the recommended treatments and therapies. It is hoped that this article provides the treating health care provider with an up-to-date review of this serious infectious process.

In root infections, bacteria are present not only in planktonic cells but also in biofilms, which are more resistant to host defence mechanisms and disinfectans. Apical periodontitis, which may be radiographically undetectable, may develop or persist as a host defence mechanism to prevent the systemic spread of bacteria and their by-products to other sites of the body. The risk of spreading microorganisms and septic emboli is present especially in compromised hosts; furthermore, long-standing inflammation may have systemic effects and affect general health. Effective procedures should be devel

The number of patients with pneumococcal bacteremia, empyema, and meningitis at Boston City Hospital during selected years between 1935 and 1974 is reported. The distribution of specific types in the bacteremic patients during each of the selected years and in the various focal infections in all the years is also detailed. The numbers and rates per 1,000 admissions of bacteremic pneumococcal infections and the numbers of cases of pneumococcal meningitis and empyema varied independently over these years and differed from those previously reported for 1929 to 1936. The types most frequent in pne…

One of possible and diagnostically difficult sources of bacterial sepsis may be purulent foci of odontogenic character. We present the case of a pregnant woman, in whom untreated purulent focus within oral cavity led to severe systemic infection. The disease was characterized by persistent hectic fever with accompanying features of intravascular coagulation, anemia and erythema nodosum and no response to antibiotic treatment. It was the second episode of sepsis in this patient in a period of one year, the source of the infectious process not being recognized previously. Dental examination reve

During the last two decades, there has been an increasing interest in the impact of oral health on atherosclerosis and subsequent cardiovascular disease (CVD). The advent of the inflammation paradigm in coronary pathogenesis stimulated research in chronic infections caused by a variety of micro-organisms-such as Chlamydia pneumoniae, Helicobacter pylori, and cytomegalovirus-as well as dental pathogens, since these chronic infections are thought to be involved in the etiopathogenesis of CVD by releasing cytokines and other pro-inflammatory mediators (e.g., C-reactive protein [CRP], tumor necros

Of all infections associated to oral pathology, the most relevant ones are those that are related to dental pathology. Cellulitis is an infection of the cellular adipose tissue located in the aponeurotic spaces. It can be classified on the basis of location, severity and evolution. The aponeurotic compartments that allow odontogenic infections to spread have been categorised as: superficial compartment, floor of the mouth, masticator compartment, parapharyngeal space, parotid space and paratonsillar space. The present work describes the anatomical structures that comprise these spaces. The cli

Brain abscesses are rare but can be life-threatening infections. Recent progress in microbiological classification and identification has indicated that they are sometimes caused by oral infection and dental treatment. It has been postulated that oral microorganisms may enter the cranium by several pathways: 1) by direct extension, 2) by hematogenous spread, 3) by local lymphatics, and 4) indirectly, by extraoral odontogenic infection. In the direct extension, oral infections spread along the fascial planes. Hematogenous spreading occurs along the facial, angular, ophthalmic, or other veins wh

A case of dental infection with spread to the mediastinum causing pericarditis and cardiac tamponade is described. The clinical course of the disease, the investigations undertaken and the treatment given are outlined.

A case of necrotizing fasciitis following infection of a mandibular third molar is reported. Necrotizing fasciitis is a relatively rare but fulminating clinical entity characterized by necrosis of fascia with widespread undermining of the superficial tissue and extreme systemic toxicity. Prompt recognition and proper management can reduce the morbidity and mortality in this severe soft tissue infection.
