[Traumatic pathology of antibrachial interosseous membrane of forearm].

The antibrachial interosseous membrane (IOM) is taught over an average length of 10.6cm between the diaphyses of the radius and ulna bone. I … omplex of the IOM. The interosseous membrane has a double function: it stabilizes transversally the forearm's two bones and stabilizes longitudinally the two bones by transferring loads from the radius to the ulna. Load transmission varies according to the prono-supination position, the varus-valgus constraints on the elbow and the inclination of the wris …

Osteotomy for malunited forearm shaft fractures in children.

Osteotomy to correct malunion after forearm fracture in children is an uncommon procedure. The uncommon nature of this procedure and the complex nature of forearm anatomy contribute to the difficulty of restoring proper alignment. The purpose of this paper is to report nine cases and describe a simplified method for deformity correction. This is a retrospective review of the authors' experience with recommendations for surgical technique. Average maximum deformity prior to osteotomy was 31 degrees. All patients united with satisfactory postoperative alignment. There was one superficial wound i…

Vascularized periosteal flaps of distal forearm and hand.

BACKGROUND: Vascularized periosteal flaps (VPFs) are flexible osteogenic flaps, suitable for the reconstruction of small skeletal defects. Pedicled VPFs, harvested near the recipient site, minimize donor-site morbidity and procedure duration. The donor sites and the potential recipient sites for pedicled VPFs of the distal forearm-hand were identified in this study. METHODS: In 16 fresh cadaver upper extremities, the dimensions and arcs of rotation of the VPF pedicle(s) were studied. RESULTS: Pedicled VPFs with mean dimensions ranging between 0.5 x 3 cm and 2.5 x 4 cm could cover, after adeq…

Complications and outcomes of open pediatric forearm fractures.

Few reports have documented the complications and outcomes of open pediatric forearm fractures. The authors completed of all patients (1987-1999) with open forearm fractures. Sixty-five patients with 65 injured extremities were identified, with an average age of 10.3 years. Fifty-two open fractures were grade I, 12 were grade II, and 1 was grade IIIA. Implants stabilized 40 extremities (62%), which improved alignment but not outcome. Forearms initially stabilized with implants did not undergo additional realignment procedures (0%), compared with 18.5% of forearms without stabilization. Eleven …

Reestablished circulation after free radial forearm flap transfer.

New circulation in the free flap reestablished at the recipient site is the key to successful microvascular reconstructive surgery. This study is the first evaluation of long-term circulatory changes in nine free radial forearm flap transfers. Postoperatively, color Doppler studies revealed that the flow volume through the arterial pedicle increased rapidly during the first 3 days, gradually increased until day 14 (exceeding flow volume through the facial artery of the unoperated contralateral side), then decreased slightly until the sixth month. The pulsatility index, representing vascular re…

[Aftercare following distal forearm fractures and carpal trauma].

Rehabilitation following operative or conservative treatment of fractures of the distal forearm and carpal trauma takes effect through adequate immobilization and ought to be maximized by a differentiated application of every means of physiotherapy, ergotherapy, and physical medicine available. Rehabilitation of the fractured distal forearm and carpal trauma are presented based on the therapeutic guidelines as applied by the Department of Hand Surgery, Plastic and Microsurgery of the Berufsgenossenschaftliches Unfallkrankenhaus Hamburg.

[A new distally pedicled adipocutaneous perforator forearm flap].

A new distally pedicled adipocutaneous perforator forearm flap useful in corrections of congenital anomalies of hand was described. Due to the congenital anomaly expression the perforators may origin from radial, median, ulnar or anterior interosseous artery which supply blood flow into the flap tissues. Raising the flap from anterior and radial surface of the forearm begins from the proximal to the distal part, above the fascia leaving superficial cutaneous nerves and cephalic vein intact. The flap may be safely incised in the ratio 5:1 and the pivot point is localized in the place where perf…

A nonischemic forearm exercise test for McArdle disease.

Ischemic forearm exercise invariably causes muscle cramps and pain in patients with glycolytic defects. We investigated an alternative diagnostic exercise test that may be better tolerated. Nine patients with McArdle disease, one with the partial glycolytic defect phosphoglycerate mutase deficiency, and nine matched, healthy subjects performed the classic ischemic forearm protocol and an identical protocol without ischemia. Blood was sampled in the median cubital vein of the exercised arm. Plasma lactate level increased similarly in healthy subjects during ischemic (Delta5.1 +/- 0.7mmol L(-1))…

Transient forearm conduction block in the median nerve.

We present two cases referred for electrophysiological confirmation of carpal tunnel syndrome (CTS). Initial nerve conduction studies were normal. Approximately 20 min into the examination, both patients developed sensory symptoms and weakness in the distal median nerve territory while the elbow was extended and forearm supinated. Further studies demonstrated complete conduction block across the forearm in the median motor and sensory nerve fibers. When measurable, conduction velocities remained normal or were modestly slow. Complete clinical and electrophysiological recovery occurred within 2…

Radial forearm free flap for soft palate reconstruction.

Defects of the soft palate resulting from head and neck oncologic surgery traditionally have been rehabilitated using a prosthetic obturator. In general, the results of soft palate obturation have been satisfactory, with most patients achieving velopharyngeal competence during speech and swallowing. However, soft palate obturators have several disadvantages. Device stability and retention are impaired in patients who are edentulous. In addition, prosthetic obturators cover large areas of sensate mucosa in the hard palate, maxillary alveolus, and oropharynx, contributing significantly to the lo

Preexpanded radial forearm free flap to the scalp.

The radial forearm flap is a versatile tool for the reconstructive surgeon because of its thinness, durable skin quality, aesthetic match, and relative ease of dissection. This flap is an ideal candidate for reconstruction of head and neck defects. One of the drawbacks is its relative small size, which may limit its use in certain applications. We describe the preexpansion of the radial forearm flap for its use in covering a large scalp defect. Preexpansion of the radial forearm free flap enables the reconstructive surgeon to increase the size of the flap markedly without altering its neurovas…

Preliminary soft tissue distraction in congenital forearm deficiency.

Four patients (five limbs) were treated with preliminary soft tissue distraction using an Orthofix external distractor. The patient's ages ranged from 6 months to 14 years. Forearm deficiencies treated included partial absence of the radius (one patient), radial aplasia (two patients, three limbs), and partial absence of the ulna (one patient). Soft tissue distraction (mean, 55 days) was continued in radial deficiency until the hand could be passively centralized without residual radial deviation, allowing centralization to be accomplished through a single mid-dorsal incision. Mean measurement…

The brachioradialis forearm flap: anatomy and clinical application.

The blood supply to the brachioradialis muscle and the skin of the forearm was studied in latex-injected arms. The dominant perforator to the muscle arose from the brachial artery (27.3 percent), radial recurrent artery (33.3 percent), or radial artery (39.4 percent). In all cases, adequate perforators exist from the radial artery so that transfer as either a muscle or musculocutaneous free flap based on this vessel is possible. In 10 arms the septocutaneous perforators from the radial artery were dissected to determine the relationship between the forearm and brachioradialis flaps. Transfer o…

Accurate prediction of outcome after pediatric forearm fracture.

Factors affecting outcome after pediatric forearm fracture include fracture angulation and fracture position. A new index, axis deviation, combines these factors. Two review studies were performed to determine if axis deviation correlated with outcome. In the first study, 35 subjects were reviewed 2.7 years after fracture without radiographs. In the second study, 152 fractures were reviewed a mean of 4 years after fracture, with 124 patients consenting to undergo radiographs. In both studies, the new index, axis deviation, correlated better with restricted forearm movement than either degree o…

[Management of unstable forearm shaft fractures in children].

Complete dislocation fractures of the forearm are rare in children. Operative treatment is indicated if it is an open fracture and if neurovascular injury is present. There is no accepted standard treatment--operative or nonoperative--for fractures that cannot be accurately reduced and stabilized. In the last 10 years we have operated on 37 fractures of the forearm in children. In 22 cases the indication for operative treatment was that the fractures could either not be reduced or could not be stabilized. Our results show that most fractures were not recognized as unstable (17 out of 22 cases)…

Reverse forearm flap based on an "exteriorized" pedicle.

Soft tissue coverage after multiple digital amputations was achieved with a reverse forearm flap based on an "exteriorized" vascular pedicle. The length of exposed radial artery pedicle was calculated to permit full range of motion of the involved digits before flap division and inset. No complications of the exposed vascular pedicle were noted.

[Bundle nailing of forearm fractures. Indications and results].

Hackethal developed the nailing procedure named after him in 1959. The rationale of Hackethal nailing is an elastic jamming, which can only be achieved by obeying four rules: jamming of the nails in the cortical window, jamming then in the waist of the medullary cavity, spreading the bunch of nails in the metaphysis and filling up the conus of the medullary cavity with short nails. We confined Hackethal nailing to closed and first-degree open fractures of the 2nd-5th sixth of the shaft of the forearm. If closed reduction and nailing were impossible, we performed a plate fixation (AO), in secon…

Reciprocal inhibition between forearm muscles in spastic hemiplegia.

We studied reciprocal inhibition of H-reflexes in the forearm flexor and extensor muscles in 14 patients with spastic hemiplegia secondary to a focal cerebral lesion and 14 normal volunteers. In the spastic limb, the Hmax/Mmax ratio was increased in both flexor and particularly extensor wrist muscles. The 3 normal inhibitory phases of reciprocal inhibition between extensor and flexor forearm muscles were markedly reduced on the spastic side of patients. The early disynaptic phase showed the greatest alteration. Reduced or absent inhibition between forearm muscles associated with increased spin…
