Periprosthetic femoral fractures: more comminution and stem loosening with cemented implants-a retrospective cohort study.

BACKGROUND: Periprosthetic femur fractures (PFF) are a significant complication associated with Total Hip Arthroplasty (THA) and Hemiarthroplasty (HA). There remains a gap in the literature regarding a consensus on which method of stem fixation yields higher rates of periprosthetic fractures—cemented or cementless. The aim of this study is to analyze the rates of comminution, stem loosening and requirements for full femoral component revision in periprosthetic femoral fractures when comparing cemented implants to cementless implants. METHODS: A retrospective cohort study of all femoral PFFs be

Antegrade Kirschner Wire Transfixation of Interphalangeal and Metacarpophalangeal Joint in a Comminuted Thumb Proximal Phalangeal Fracture.

Comminuted proximal phalangeal fractures of the thumb are inherently unstable injuries requiring surgical fixation to provide stability and restore function. Current fixation methods include plate and screw fixation, which is associated with stiffness (in 64% of cases), extensor lag, and hardware complications requiring further operations. Moreover, intramedullary screws carry the risk of cartilage damage and fracture displacement during insertion. These modalities requiring extensive soft tissue dissection correlate with poor functional outcomes due to fibrosis and vascular disruption. A simp

Lateral tibial plateau fractures with posterior comminution. Can a rim plate offer sufficient support?

Tibial plateau fractures with posterior comminution are difficult to reduce and stabilise. Standard lateral approach doesn't offer adequate visualisation. Posterior approach is suitable for isolated posterior fractures. The addition of a specifically contoured rim plate can enable fixation of posterior fragments through a modified lateral approach. The purpose of this study was to evaluate the advantages and limitations of this novel technique. We present a prospective series of 16 tibial plateau fractures with posterolateral depression, operated over a period of 7 years (04.2014-09.2021). The

The biomechanical effect of fibular strut grafts on humeral surgical neck fractures with lateral wall comminution.

No studies have evaluated the effect of fibular strut augmentation on the stability of locking plate fixation for osteoporotic proximal humeral fractures with lateral wall comminution. The purpose of this study was to evaluate the stability of locking plate fixation with a fibular strut graft compared with locking plate alone in an osteoporotic two-part surgical neck fracture model with lateral cortex comminution. Ten paired fresh-frozen cadaveric humeri were randomly allocated into two groups, either the locking plate alone (LP group) or locking plate with fibular strut graft augmentation (LP

Anterior Bridge Plate Osteosynthesis in Comminuted Fracture Shaft of Humerus in Manual Workers- is it Optimum Choice?

BACKGROUND: Bridge plate osteosynthesis of fractures by minimal invasion and near acceptable reduction is becoming popular and acceptable entity. Management of humeral shaft fracture has evolved a lot with their pros and cons. Anterior bridge plate osteosynthesis (ABPO) for humeral shaft fracture is pertinent to a minimal invasive procedure, and it has evolved as a new entrant in the surgical techniques. This study was designed to carry out the results and efficacy of ABPO in the comminuted fracture shaft of the humerus in the manual workers. METHODS: Study included the closed comminuted frac

Airway management in a displaced comminuted fracture of the mandible and atlas with a vertebral artery injury: A case report.

Complex cervical spine fractures are a serious complications of maxillofacial trauma and associated with high mortality and neurological morbidity. Strict vigilance in preventing further insult to the cervical spine is a crucial step in managing patients who are at risk for neurologic compromise. We report a rare case of a right transverse process of atlas fracture with right-sided vertebral artery injury that was associated with a comminuted fracture of the body and angle of the mandible, which restricted mouth opening. Airway management was performed by an awake fiber-optic nasotracheal intu

Cortical comminution in distal radial fractures can predict the radiological outcome: a cohort multicentre study.

This paper investigates whether cortical comminution and intra-articular involvement can predict displacement in distal radius fractures by using a classification that includes volar comminution as a separate parameter. A prospective multicentre study involving non-operative treatment of distal radius fractures in 387 patients aged between 15 and 74 years (398 fractures) was conducted. The presence of cortical comminution and intra-articular involvement according to the Buttazzoni classification is described. Minimally displaced fractures were treated with immobilisation in a cast while displa

En bloc joystick reduction of a comminuted intra-articular distal radius fracture: a technical trick.

A patient with a 1-month-old intra-articular distal radius fracture (treated closed in a splint) presented with an unacceptable degree of pain and stiffness caused by shortening and dorsal angulation of the distal radius. The fracture was comminuted with 4 or 5 distinct fragments, several involving the articular surface. Surgical correction was attempted. During the procedure, it was noted that, though the distal radius was shortened and angulated, there was actually acceptable congruity of the articular surface itself, despite the intra-articular nature of the fracture. Bone quality was poor

Outcome of dorsally comminuted versus intact distal radial fracture fixed with volar locking plates.

OBJECTIVE: Distal radial fractures are common. Modern trends favour operative treatment in many instances, providing stable fixation and early functional recovery. Recent biomechanical evidence suggests that volar locking plates (VLPs) enable adequate stability for dorsally displaced fractures, both in dorsally intact (DI) and in dorsally comminuted (DC) fractures. The aim of the study was to compare the clinical outcome of these two fracture groups treated with a VLP. METHODS: Retrospective case-control analysis of 91 distal radial fractures treated surgically using VLP by a single surgeon b

Dynamic digital traction for unstable comminuted intra-articular fracture-dislocations of the proximal interphalangeal joint.

Unstable intra-articular fracture-dislocations about the proximal interphalangeal joint present a formidable management dilemma. Fourteen patients with comminuted injuries of the proximal interphalangeal joint were treated with dynamic digital traction and followed for an average of 24 months. Dorsal fracture-dislocation and pilon-type injuries made up the majority of cases. Average proximal interphalangeal joint articular surface involvement was 80%. Immediate motion was instituted after traction application. The device was applied in the office with readily available materials. At final foll

Factors Associated With Accelerated Fracture Healing in Patients With Traumatic Brain Injury and Extremity Comminuted Fractures: A Retrospective Case-Control Study.

OBJECTIVE: Although traumatic brain injury (TBI) has been clinically associated with accelerated bone healing, the factors that determine which patients experience this phenomenon remain poorly defined, and previous findings are conflicting. This study aimed to investigate the clinical factors associated with accelerated fracture healing in patients with TBI combined with comminuted fractures of the limbs, so as to provide an evidence-based foundation for elucidating the clinical phenomenon of TBI-promoted fracture healing. METHODS: A retrospective case-control study design was employed. Pati

Odontoid fracture type and angulation affect nonunion risk, but comminution and displacement do not: A propensity score matched analysis of fracture morphology.

OBJECTIVE: Odontoid fractures disproportionately affect older patients who have high surgical risk, but also high rates of fracture nonunion. To guide surgical decision-making, we quantified the effect of fracture morphology on nonunion among nonoperatively managed, traumatic, isolated odontoid fractures. METHODS: We examined all patients with isolated odontoid fractures treated nonoperatively at our institution between 2010 and 2019. Multivariable regression and propensity score matching were used to quantify the effect of fracture type, angulation, comminution, and displacement on bony heal
