A patient with ampullary disconnection during gastrectomy is presented, emphasizing the chronicity and retractile scarring which exposes the bile and pancreatic ducts to simultaneous injury. As wider appreciation of this potentially lethal lesion is gained, greater use of predissection cholangiography or probe identification of the papilla should occur. Steps to detect the lesion and primary repair by duodenal advancement or, in unrecognized instances, the Hepp operation using a Roux-en-Y limb after a suitable waiting period are recommended.

Pancreatic cancer is among the leading causes of gastrointestinal cancer-related death, with a dismal prognosis. Over 80% of pancreatic cancer patients present with advanced disease, making curative resection unfeasible. These patients are often presented with malignant biliary obstruction (MBO) and gastric outlet obstruction (GOO). In these cases, palliative management is aimed to alleviate symptoms, enhance quality of life, and facilitate subsequent chemotherapy. Currently, neoadjuvant chemotherapy is frequently used in both borderline resectable and resectable pancreatic cancer, necessitati

CASE SERIES SUMMARY: Two cats, aged 13.5 and 14 years, presented with duodenal masses ultimately diagnosed as Brunner's gland (BG) adenomas. In the first cat, the lesion was a firm, pale tan, proximal duodenal polyp adjacent to the major duodenal papilla that was composed histologically of a well demarcated, expansile mass of well-differentiated BGs separated by fibrovascular septa and few adipocytes. This patient was euthanized for otitis-related vestibular signs, and the clinical significance of the duodenal polyp is unknown. The second cat presented with a 6-month history of vomiting. This

BACKGROUND: Visceral artery aneurysms (VAA) are rare with an incidence of up to 0.2% and mortality of up to 40%. Aneurysms larger than 5 cm are referred to as giant visceral artery aneurysms (GVAA). We present a case of a celiac artery aneurysm of 14 cm which required endovascular and surgical management. A review of the literature with focus on treatment is also provided.
CASE PRESENTATION: In this case report, a 76-year-old male presented with postprandial nausea and vomitus. An ultrasound and computed tomography scan showed an aneurysm of the celiac artery of approximately 14 cm. Initial t

Symptomatic gastroduodenal Crohn's disease (CD) is rare although new endoscopic/histologic data indicate a typical focally enhanced gastritis in up to half of all patients with CD. One third of the patients with symptomatic gastroduodenal CD undergo surgery, most of them for (gastro-) duodenal obstruction. Gastroenterostomy with vagotomy is the surgical treatment of choice. Resection, strictureplasty or balloon dilatation can be performed in selected patients. Enterogastric and enteroduodenal fistulas are rare, frequently missed during routine examination and often detected only during laparot

Conclusions regarding the interaction of H. pylori infection with noxious agents such as NSAIDs, alcohol, and smoking in the development of histologic gastritis and gross gastric injury are limited by the paucity of well-performed studies specifically addressing this problem. At present, we can conclude that H. pylori prevalence is not affected by NSAIDs, alcohol, or smoking. On the other hand, reflux of duodenal contents may play a role in eradication of H. pylori after gastroenterostomy. Although NSAIDs and alcohol do cause gross gastric injury they do not appear to induce changes in the inf

Over a 5-year period 114 patients with gastric cancer were evaluated. Seventy-seven (68%) of these underwent laparotomy, of which 5A (47%) had a resection performed though only 22 (19%) of these were considered curative. Thirty-seven patients (32%) were not offered surgery because they were aged, had poor cardiorespiratory function, or were thought to have advanced disease based on a combination of clinical (fixed epigastric mass, hepatomegaly, jaundice or ascites), radiological and endoscopic features. Overall 5-year survival was 10.9%, with the patients who had curative and palliative resect

In 46 patients with gastric resection and Roux-en-Y gastrojejunostomy, gastric emptying was studied with the gamma camera. Seventeen patients were free of symptoms, 11 vomited occasionally (less than 5 times weekly), and 18 were severely incapacitated with daily vomiting, weight loss, and bezoar formation. Patients with occasional vomiting had early rapid emptying similar to that seen in the patients who were without symptoms and responded satisfactorily to nonsurgical therapy. The 18 patients with severe vomiting showed a marked delay in the emptying of the solid meal (p less than 0.01) but n

CONTEXT: Delayed gastric emptying is a major cause of morbidity following pancreaticoduodenectomy.
OBJECTIVE: The impact of a Braun enteroenterostomy on delayed gastric emptying, used in reconstruction following classic pancreaticoduodenectomy, was assessed.
PATIENTS: Forty-four consecutive patients undergoing non-pylorus preserving pancreaticoduodenectomy from 2009 to 2011 by a single surgeon were included in this study.
INTERVENTIONS: The first 20 patients had a standard antecolic gastroenterostomy and the subsequent 24 had the addition of a Braun enteroenterostomy.
RESULTS: Patient char

The serum levels of pepsinogen A (PGA) were measured in patients who underwent various forms of gastric resection to assess whether Helicobacter pylori (HP) colonization has any influence. Included in this study were 48 patients who underwent subtotal gastrectomy for a peptic ulcer (SGPU), 36 who underwent radical subtotal gastrectomy for gastric carcinoma (SGGC), 16 who underwent truncal vagotomy plus antrectomy (TV + AE), 24 with recurrent ulcer (RU) and 27 who underwent total gastrectomy (TG). The mean serum PGA levels in these five groups and in 40 healthy controls were 49.1 +/- 30.4 ng/ml

BACKGROUND: The optimal treatment for malignant gastric outlet obstruction (GOO) remains uncertain. This systematic review aimed to comprehensively investigate the efficacy and safety of four palliative treatments for malignant GOO: gastrojejunostomy, endoscopic ultrasound-guided gastroenterostomy (EUS-GE), stomach-partitioning gastrojejunostomy (PGJ), and endoscopic stenting.
METHODS: We searched PubMed, Embase, Cochrane Library, Scopus, and Web of Science databases, ClinicalTrials.gov, and the World Health Organization International Clinical Trials Registry Platform for randomized controlle

BACKGROUND: Self-expandable metal stent (SEMS) migration after endoscopic ultrasound-guided hepaticogastrostomy (EUS-HGS) is a severe complication. The migration risk could be related to the surface friction of SEMS, assumed to be affected by the wire structure and mechanical properties, including radial force (RF); however, their relevance remains unclear. This experimental study aimed to assess the mechanical properties of SEMS involved in the onset of stent migration by measuring the sliding-resistance force (SF) as the SEMS moves through the stomach wall.
METHODS: The SF of seven types of

Endoscopic ultrasonography-guided fine needle aspiration biopsy (EUS-FNA) is a safe and accurate procedure for the diagnosis of pancreatic lesions. However, in general, surgically altered anatomy excluding Billroth I gastrectomy is considered a relative contraindication to EUS-FNA. Herein, we described a successful case of EUS-FNA in a patient with prior Billroth II gastrectomy. A 78-year-old man, who had previously undergone Billroth II gastrectomy for duodenal ulcer, undertook EUS-FNA using a curved linear echoendoscope. An echoendoscope was advanced into the duodenum and EUS-FNA could be ca

We report a case of intrathoracic herniation of the colonic interposition pulled up through the retrosternal space after subtotal esophagectomy for esophageal cancer. The patient, a 68-year-old man, presented with progressive dysphagia about 1 year after this operation. We performed left thoracotomy and laparotomy, which revealed the reconstructed colon herniating into the left thoracic cavity through a large defect in the left mediastinal pleura. The redundant colon was resected, and the colonic graft was shortened and straightened. We concluded that the defect in the mediastinal pleura and c

BACKGROUND AND OBJECTIVES: Morbidity rates following pancreaticoduodenectomy (PD) remain high with delayed gastric emptying (DGE) and slow resumption of oral diet contributing to increased postoperative length of stay. A Braun enteroenterostomy has been shown to decrease bile reflux following gastric resection. We hypothesize that addition of Braun enteroenterostomy during PD would reduce the sequelae of DGE.
METHODS: From our PD database, patients were identified that underwent classic PD with partial gastrectomy from 2001 to 2006. All patients with reconstruction utilizing a single loop of

The gastrojejunocolic fistula represents a clinical entity that occurs very rarely following gastro-jejunal anastomoses and manifests itself clinically and paraclinically by a severe malabsorption syndrome. The results of the physiopathological approach may be summed up as follows: reduced level of seric proteins, fluid and electrolytic depletion, deficiencies in the absorption of the vitamins soluble in fats and water, which may all vary from mildness to severeness, depending on the flow rate of the fistula. Most often, the diagnosis is set by performing barium enema, which is positive for al

Jejunogastric intussusception (GI) is an extremely rare complication of gastrojejunostomy (GJS) that may appear any time after surgical intervention. Less than 200 cases have been reported so far, on very small series. Young female, 32, who 12 years ago was operated for a gastroduodenal disease that she doesn't know many details about. She presented severe pain in the superior abdominal segment posteriorly irradiated, incoercible biliary nausea followed by hematemesis. The endoscopic, imaging and biological explorations suggested a huge gastric tumor that occupied the whole stomach and was ble
