[Comparative study of general and spinal anesthesia in elderly women in hip surgery].

The choice between regional versus general anaesthesia for elderly patients undergoing hip surgery is debated. It is vitally important to see if the type of anaesthetic administered affects per- and postoperative morbidity and mortality. Seventy women more than 75 yr old suitable for spinal anaesthesia were included in this study. They were randomly divided into two groups: in one (n = 35), general anaesthesia was given with sodium thiopentone, fentanyl, enflurane and vecuronium; in the other (n = 35), spinal anaesthesia was performed with 3 ml 0.5% bupivacaine in isobaric solution. During the

The extended postoperative complication score: A dual-axis reporting plan for general surgery.

BACKGROUND: Grading surgical complications and sequelae is of paramount importance for analyzing results and improving patient care and system performance and in optimizing resource allocation and use. Existing systems for surgical complication classification do not consistently account for baseline physiologic vulnerability and often conflate treatment intensity with clinical severity. This limitation reduces interpretability across diverse patient populations, particularly in emergency and elective general surgery. METHODS: A multidisciplinary expert panel conducted a modified Delphi proces…

Outcomes Observed Within Embedded Geriatric Services in Emergency General Surgery: A Systematic Review.

INTRODUCTION: An aging population and advances in surgery have led to increasing need for geriatric management of elderly patients undergoing emergency general surgical care. Integration of a proactive geriatric service for these patients may beneficially impact both traditionally measured hospital and patient-based metrics. Here, we synthesize evidence regarding measurable outcomes of embedded geriatric care within emergency general surgery. METHODS: A comprehensive search was conducted across Medline, Embase, Cochrane Library, CINAHL, and Scopus databases by two independent reviewers. The i…

Reconsidering Anesthesia in Lumbar Surgery: An Umbrella Review of Awake Versus General Anesthesia.

Background/Objectives: Lumbar degenerative disease drives numerous elective spine surgeries, and anesthetic choice significantly influences airway risk, hemodynamics, analgesia, mobilization, and recovery. Interest in awake lumbar surgery, typically using spinal anesthesia (SA) with light sedation, has grown as comparative studies suggest comparable safety to general anesthesia (GA) with potential reductions in opioid use, nausea, time to ambulation, and efficiency metrics. However, these benefits may be context-dependent under standardized perioperative care. Therefore, the aim of this umbrel

Feasibility of intraoperative continuous glucose monitoring: An observational study in general surgery patients.

BACKGROUND: Perioperative hyperglycemia is associated with adverse outcomes in surgical patients, and major societies recommend intraoperative monitoring and treatment targeting glucose <180-200 mg/dL. However, compliance with these recommendations is poor, in part due to fear of unrecognized hypoglycemia. Continuous Glucose Monitors (CGMs) measure interstitial glucose with a subcutaneous electrode and can display the results on a receiver or smartphone. Historically CGMs have not been utilized for surgical patients. We investigated the use of CGM in the perioperative setting compared to curre

A Nurse-Led Voiding Algorithm for Managing Urinary Retention After General Thoracic Surgery.

BACKGROUND: Untreated postoperative urinary retention (POUR) leads to bladder overdistension. Treatment of POUR involves urinary catheterization, which predisposes patients to catheter-associated urinary tract infections. The hospital's rate of POUR after lobectomy was 21%, exceeding the Society of Thoracic Surgeons' benchmark of 6.4%. Nurses observed that more patients were being catheterized after implementation of a newly revised urinary catheter protocol. OBJECTIVE: To reduce the incidence of POUR by implementing a thoracic surgery-specific nurse-led voiding algorithm. METHODS: Experts v

Machine learning-based modeling of acute respiratory failure following emergency general surgery operations.

BACKGROUND: Emergency general surgery (EGS) operations are associated with substantial risk of morbidity including postoperative respiratory failure (PRF). While existing risk models are not widely utilized and rely on traditional statistical methods, application of machine learning (ML) in prediction of PRF following EGS remains unexplored. OBJECTIVE: The present study aimed to develop ML-based prediction models for respiratory failure following EGS and compare their performance to traditional regression models using a nationally-representative cohort. METHODS: Non-elective hospitalizations…
