BACKGROUND: Like most medical disciplines, otorhinolaryngology is confronted with an increasing number of elderly and very old patients. In addition to the challenges of diagnostics and treatment, questions related to the influence of geriatric multimorbidity and functional limitations on treatment decisions and success also arise.
MATERIALS AND METHODS: A literature search was performed on the use of geriatric assessment instruments in otorhinolaryngology.
RESULTS: While there are already a great deal of data on the interaction of hearing disorders and cognition, the importance of other com

Greater life expectancy with advancements in technology and medicine has led to an increasing interest in facial rejuvenation. Facial aging is an inevitable process that largely results from soft tissue descent and volumetric deflation. However, a comprehensive knowledge of the aging process and precise assessment of the exact pathologies yielding the patient's senescent appearance is essential to produce the best cosmetic outcome. The surgeon must evaluate each region independently and the aging face as a whole to ensure a pleasing, natural appearance.

Clozapine is the first second generation antipsychotic with different receptor profile of action. Clozapine is the most efficacious drug for the treatment of psychotic disorder and is the drug of choice in treatment resistant schizophrenia. Clozapine is used in elderly patients infrequently owing to its adverse effects profile and tolerability. There is paucity of literature with respect to clozapine use in late life. In this narrative review, we discuss clozapine use in elderly and challenges associated with its use.

Elderly people more often suffer from vitamin D insufficiency. It is caused by insufficient supply with diet and scarce sun exposure, due to life style. This is a very common situation in Poland and worldwide. Vitamin D influences functioning of many various organs. Its deficiency may cause bone mineralization disorders, osteomalacia, osteoporosis, muscle weakening, which can result in higher risk of falls. Its influence on cardiovascular diseases, type 2 diabetes and cognitive functions is widely discussed. Supplementation is crucial in elderly population. It should be administrated all year,

INTRODUCTION: Oropharyngeal dysphagia is one of the lesser known geriatric syndromes, despite its enormous impact on functional capacity, quality of life, and health of those affected.
MATERIAL AND METHODS: A descriptive and prospective study was conducted by the Geriatric Department of Barbastro Hospital (Huesca), from March 2012 to October 2014, as biannual and annual reviews in October 2015. This study included all patients on whom a volume-viscosity clinical examination (MECV-V test) was performed to suspecting dysphagia.
RESULTS: The study included 266 patients with a mean age of 82.35±

Because of advances in medicine and other sciences, the average human life span is longer now than any other time in history. The physiologic effects of aging as well as multimorbidity, polypharmacy, and other geriatric-specific syndromes create additional challenges when elderly patients experience a traumatic injury. However, there is a growing evidence base that can inform the clinical decision-making process. This narrative review of the literature addresses the state of the science regarding geriatric syndromes, guidelines and protocols, indices and models for prognostication, outcomes an

INTRODUCTION: Ankle fractures are the third most common orthopaedic injury seen in the geriatric patient. Studies have identified mortality benefits with operative management, but treatment must be considered on a case-by-case basis. In the era of value-based analysis, a thorough of understanding of outcomes and costs of treatment is required. The purpose of this study was to analyze the inpatient and readmission costs associated with operative and nonoperative management of geriatric ankle fractures.
METHODS: Patients were identified using diagnosis codes for ankle fractures from all 2008 Pa

BACKGROUND: Functional health issues and the ability to autonomously participate in social life play a more pronounced role for geriatric patients than the mere prolongation of life. Quality-adjusted life years reflect the relation of health and life span. Comorbidities and functional or cognitive impairment represent independent predictors for the overall survival of geriatric patients. This must be kept in mind when planning systemic cancer treatment.
THERAPY: All forms of cancer-specific therapy should evaluate whether the patient can truly benefit taking into consideration geriatric aspec

Foot pathologies are common in nearly 80% of all elderly patients, and studies have indicated inappropriate footwear as one of the major underlying cause. It has been postulated that ill-fitting shoe wear affects plantar pressure, thus exacerbating weak balance. Complications arising from foot pathologies, which include difficulty in maintaining balance, have increased the risk of falls that can result in fractures and other serious injuries. The link between footwear and the onset or progression of certain foot pathologies has emphasized the need to explore and promote preventative measures t

AIM: To carry out an oral biopsy survey in geriatric patients from the participating institutions.
METHODS: The biopsy records of the participating institutions were reviewed for oral lesions from patients aged 65 years and older diagnosed from 2003 to 2012. Demographic data and the site of the lesions were collected. Histopathological diagnoses were categorized into two categories: non-neoplastic lesions (reactive/inflammatory lesion, cyst, allergic/immunologic disorders, potentially malignant disorders, infection and others) and neoplastic lesions (benign and malignant tumors). Data were an

BACKGROUND AND PURPOSE: Musculoskeletal problems, including shoulder pain, are common in the general population and are often cited as reasons for physician visits. Although many risk factors for shoulder pain are postulated, the effects of shoulder pain on functional level and perceived quality of life are poorly characterized in older adults. In this study, we set out to determine the prevalence and impact of shoulder symptoms and dysfunction in an older adult veteran population.
METHODS: A chart review, cross-sectional survey, and examination were performed. A sample of 93 individuals, age

The influenza virus is a significant cause of morbidity and mortality each year in the United States, and is a major public health problem. Individuals aged 65 years and older comprise a sizeable population subgroup at high risk of infection and subsequent complications. Although influenza may cause substantial morbidity and mortality across the age spectrum, it becomes particularly problematic for those older than 65. This article presents an overview of influenza, with a focus on how the influenza virus has particular implications for the geriatric population.

OBJECTIVES/HYPOTHESIS: To evaluate the hearing performance with cochlear implants (CIs) in patients who were 70 years or older at the time of implantation (geriatric patients) and compare it with the performance in younger adults.
STUDY DESIGN: Individual, retrospective, cohort study.
METHODS: A cohort of 1,005 postlingually deafened adults was selected for this study. According to their age at the time of cochlear implantation, patients were divided into four age groups: group 1, 18 to 39 years; group 2, 40 to 59 years; group 3, 60 to 69 years; and group 4, 70 years and older). The aim of this review is to offer an updated survey about the relationship between old age, vitamin D and different clinical conditions. Two third of people over 65 years has insufficient serum levels of vitamin D (<30 ng/ml). Almost half of them present deficiency (<20 ng/ml), severe in many cases (<15 ng/ml). This proportion increases when we analyze non-white populations, women, and people with obesity, diabetes mellitus or diets poor in Vitamin D. Low serum vitamin D concentration has been linked to mortality, osteoporosis, falls propensity, fractures, frailty, and cardiovascular diseases

Surgery in geriatric patients is accompanied by increases in morbidity and mortality, increases in functional abnormalities and poor outcomes, and increases in severe malnutrition, compared with surgery of similar magnitude in nongeriatric patients. Hospitalized elderly patients are at significant risk of presenting with, or developing, protein-energy and other nutrient deficiencies. However, nutritional assessment of older geriatric patients, 65 to 100 years of age, is a challenging task because of lack of adequate age-specific reference data in this diverse and heterogeneous population.
