Superficial femoral artery perforator flap: anatomical study of a new flap and clinical cases.

BACKGROUND: The medial thigh has been infrequently studied as a donor site for pedicled or free flaps. In their previous studies, the authors observed a direct cutaneous branch from the superficial femoral artery. This study aimed to investigate the anatomy and potential possibility for flap elevation (the midmedial thigh flap) on this direct branch of the superficial femoral vessels. METHODS: Circumferential adipocutaneous thigh flaps were harvested from 14 fresh adult cadaver legs. The direct cutaneous branch from the superficial femoral vessels was located between the sartorius and gracili

The anterior Obturator Artery Perforator (aOAP) flap: surgical anatomy and application of a method for vulvar reconstruction.

OBJECTIVE: Vulvar reconstruction following oncologic resection is challenging. Some flaps used for reconstruction can show adverse characteristics such as excessive tissue bulk or increased distance to the defect. Region of the sulcus genitofemoralis is of thin and pliable tissue proximate to the vulva. Vasculature and suitability of that region used for vulvar reconstruction were focused in this work. METHODS: Vascular architecture of the region comprising the sulcus genitofemoralis was examined bilaterally on 10 female corpses (n = 20 specimens). In addition, tissue characteristics and suit

Free functional muscle transfer in children: A long-term case series from a single microsurgical unit.

BACKGROUND: Restoration of active motion in children with upper-limb paralysis or muscle loss remains challenging. Free functional muscle transfer (FFMT) is a reconstructive option when local muscle donors are unavailable, but uncertainties persist regarding optimal timing, donor nerve selection, and long-term effects of growth and secondary deformities. This study reports a single-institution experience addressing these issues. METHODS: A retrospective review was conducted of 35 children (<18 years) who underwent upper-limb FFMT with a minimum follow-up of 24 months. All reconstructions used

Anatomical variability of the pes anserinus tendons: prevalence and patterns of accessory bands of the semitendinosus, gracilis, and sartorius tendons and a new classification.

BACKGROUND: The pes anserinus constitutes a complex anatomical region on the medial side of the knee, formed by the tendons of the semitendinosus, gracilis, and sartorius muscles. Accessory tendon bands within this region have been described in numerous anatomical studies; however, their reported prevalence, morphology, and distal attachment patterns vary considerably. These inconsistencies largely result from non-standardized terminology and classification systems that often fail to capture the anatomical complexity of this structure. MATERIALS AND METHODS: A total of 62 formalin-fixed adult

Hypokalemic periodic paralysis associated with the atypical CACNA1S c.2690G>A (p.Arg897Lys) variant: description of 14 affected individuals from five families.

This study describes five families (14 individuals) with hypokalemic periodic paralysis carrying a heterozygous pathogenic variant NM_000069.3:c.2690G>A (p.Arg897Lys) in the Calcium Voltage-Gated Channel Subunit Alpha1 S (CACNA1S) gene. The clinical exam showed pelvic weakness was common (10/14, with three being too young to exclude this age-dependent myopathy). Electromyography showed myogenic changes, and the long exercise test did not reveal a significant reduction of compound muscle action potential amplitude. Muscle MRI in three patients demonstrated involvement of axial musculature, the

Hamstring muscle recovery following arthroscopic anatomic lateral ankle ligament reconstruction.

PURPOSE: Arthroscopic anatomic lateral ankle ligament reconstruction using gracilis autograft has become common practice in the treatment of chronic lateral ankle instability. While morbidity of hamstring harvesting has been extensively studied in anterior cruciate ligament reconstruction, no study has assessed the impact of this autograft on muscle recovery in ankle ligament reconstruction. The purpose of this study was to evaluate short-term hamstring recovery using isokinetic tests. METHODS: All patients operated on for chronic lateral ankle instability by arthroscopic anatomic ligament re

Repair of the femoral nerve by two motor branches of the obturator nerve: A case report.

Complete femoral nerve palsies are uncommon but devastating injuries when they are caused by large nerve defects. Direct repair is usually not possible and nerve grafting renders uncertain outcomes. Recent studies proposed different peripheral nerve transfers as treatment strategies for large femoral nerve defects. We report a clinical application of a nerve transfer to reinnervate the quadriceps muscle with two motor branches of the obturator nerve in a 48 years-old man that was diagnosed with a femoral nerve palsy after resection of a retroperitoneal schwannoma. The branches supplying the gr

Paralytic dislocation of the hip in myelodysplasia. The role of the adductor transfer.

In thirty-nine patients with myelodysplasia at the third, fourth, or fifth lumbar level and subluxation or dislocation of the hip (fifty-nine hips involved), the following surgical approaches were utilized to balance the muscles about the hip: In one group (twenty hips in eleven patients) the origins of the adductors longus, brevis, and gracilis were transferred to the ischial tuberosity, and in fifteen hips in that group the iliopsoas also was transferred to the greater trochanter when the abductors were severely weakened. In the second group (thirty-nine hips in twenty-eight patients), only

LGMD R18 Mimicking Wilson Disease: 6-Year Longitudinal Muscle MRI Evolution and Clinical Insights into TRAPPC11 Mutation.

Background and Clinical Significance: Limb-girdle muscular dystrophy R18 is a rare autosomal recessive disorder that may present with both muscular and hepatic involvement, potentially leading to diagnostic confusion. Longitudinal MRI data documenting this disease from childhood remain scarce. Case Presentation: We report a female patient with disease onset that occurred at 8 years of age, initially presenting with elevated liver enzymes and decreased ceruloplasmin levels, which raised suspicion of Wilson's disease. Genetic testing confirmed TRAPPC11 mutation. Serial MRI scans of the thigh mus

Skeletal muscle insulin unresponsiveness during chronic hyperdynamic sepsis in the dog.

Recent reports from our laboratory and others have documented changes in insulin unresponsiveness and electrolyte and hormonal changes characteristic of hypodynamic shock states in anesthetized animals. Since most acute shock protocols do not adequately mimic the clinical profile of sepsis, the present study was undertaken to document the hemodynamic and metabolic changes associated with chronic hyperdynamic peritonitis in dogs. Mongrel dogs of either sex weighing 20 +/- 2 kg were surgically instrumented with an electromagnetic aortic flow probe for monitoring cardiac output determinations, an

CT muscle imaging and the clinical assessment of neuromuscular disease.

Twenty patients with neurogenic disorders, polymyositis, or muscular dystrophies were assessed clinically and by CT imaging of limb, limb girdle, and trunk muscles, using a standard protocol. On each side of these patients 26 movements were graded by the MRC scale, and 20 muscles were assessed by CT imaging. The clinical and CT findings could be compared, in a blind evaluation, in 10 muscles on each side. A quantitative assessment of the CT muscle images were also made. The CT images showed striking abnormalities, even in many muscles of normal strength by clinical testing. Asymmetrical involv

Oxidant stress-induced increase in myogenic activation of skeletal muscle resistance arteries in obese Zucker rats.

This study characterized myogenic activation of skeletal muscle (gracilis) resistance arteries from lean (LZR) and obese Zucker rats (OZR). Arteries from OZR exhibited increased myogenic activation versus LZR; this increase was impaired by endothelium denudation or nitric oxde synthase inhibition. Treatment of vessels with 17-octadecynoic acid impaired responses in both strains by comparable amounts. Dihydroethidine microfluorography indicated elevated vascular superoxide levels in OZR versus LZR; immunohistochemistry demonstrated elevated vascular nitrotyrosine levels in OZR, indicating incre

Microvascular Gracilis Free Flap: Single and Double Innervation.

Facial paralysis (FP) is a devastating condition that can lead to significant aesthetic, social, and emotional morbidities for patients. For some patients with FP, free gracilis muscle transfer (FGMT) is the best option for smile restoration. Masseteric-driven FGMT produces a reliable voluntary smile. Cross-face nerve graft-driven FGMT can produce a spontaneous smile, but this technique has higher failure rates. Early studies suggest dual-innervation FGMT can produce a spontaneous smile while maintaining the reliability of masseteric-driven procedures. Great care should be taken during FGMT su

Motor nerve biopsy: clinical usefulness and histopathological criteria.

Early differential diagnosis of motor neuropathies (MN) and lower motor neuron diseases (LMND) is important, as prognosis and therapeutic approaches are different. We evaluated the diagnostic contribution of the biopsy of the motor branch of the obturator nerve and gracilis muscle in 21 consecutive patients in which, after proper clinical and neurophysiological studies, the differential diagnosis was still open. At baseline, motor biopsy was performed; diagnostic confirmation was obtained by 2-year clinical follow-up. Our results support the usefulness of this diagnostic procedure for selected

The possible link between venison poisoning and Sarcocystis spp. infection in Lithuania: a case report.

There have been report of possible food poisoning in Lithuania following the consumption of raw steaks from a roe deer (Capreolus capreolus). Two people experienced symptoms of abdominal pain, watery diarrhea, loss of appetite, and nausea. The parasitological examination of the roe deer revealed macrocysts of Sarcocystis sp. to the naked eye. Microscopic sarcocysts of Sarcocystis spp. were detected in different muscles, and a high sarcocyst load was observed in the roe deer muscles by a light microscope. Based on molecular results, S. capreolicanis, S. gracilis, S. linearis, S. oviformis, and

Gracilis microneurovascular transfer for facial paralysis.

Facial nerve dysfunction occurs in varying degrees of severity due to several causes, and leads to asymmetric or absent facial movements. Regardless of the etiology, facial nerve dysfunction can be functionally and psychologically devastating. Many techniques to restore facial symmetry both at rest and with motion have been pursued throughout history. Within the past 30 years, free muscle microneurovascular transfer techniques have been developed to provide symmetric motion to the face. The aim of this article is to describe one of the most common and reliable techniques to restore midface mob

Characteristic findings of skeletal muscle MRI in caveolinopathies.

Caveolinopathies, caused by CAV3 mutations, can include several phenotypes such as rippling muscle disease, limb-girdle muscular dystrophy type 1C, distal myopathy, familial hypertrophic cardiomyopathy, and idiopathic hyperCKemia. Here we present characteristic skeletal muscle imaging findings in four patients with genetically defined childhood-onset RMD caused by CAV3 mutations and in one patient with congenital generalized lipodystrophy type 4 with muscular dystrophy due to polymerase I and transcript release factor (PTRF) mutations, which may have caused secondary deficiency of caveolin-3.
