Study of antipsychotic drug use in long-term care facilities.

We chose to conduct this study because neuroleptic use has not been well defined in nursing homes. To our knowledge, it represents the only attempt at implementing the AIMS procedure in a group of nursing homes. We have provided evidence for what appears to be reasonably typical use of antipsychotic medication in skilled and intermediate care facilities. Senility and dementia were the most common diagnoses noted for neuroleptic recipients. Inservice education had little, if any, impact on antipsychotic drug use. Efforts to reduce the incidence of inappropriate use of neuroleptics should be aim

[Injuries at day care centers and family day nurseries. A two-year case-load of a casualty department involving children under 7 years of age].

With the object of demonstrating whether injuries in children cared for outside their homes differ from injuries in children as a whole, the authors registered a material of children in the age group 0-6 years from a casualty department. Ninety injuries which had occurred during care outside the home were compared with the remaining 782 injuries. An enormous increase in the proportion of injuries occurring in day care centres and in family day nurseries was observed in contrast to the findings in previous investigations. This can be explained by the increasing number of children cared for outs

Longitudinal changes in functional capacity among surviving old people continuously resident in hospitals and homes.

A group of surviving, continuously resident elderly people was identified through linkage of data from two cross sectional studies (three years apart) of all types of hospital and homes provided within a defined geographical area. Although 60-70% were unchanged in their ability to perform basic activities of daily living, any change was most often deterioration: between three and four to one. The exception was for faecal incontinence: a higher proportion remained unchanged but of those who changed, more deteriorated than was so for other activities. Change was greater when all aspects of funct

Risk factors for toxocaral ocular larva migrans: a case-control study.

Twenty-four patients with serologically confirmed toxocaral ocular larva migrans (OLM) and 24 age- and sex-matched controls were the subjects of an epidemiologic study. Information on the exposure to dogs and other factors possibly associated with transmission of toxocariasis was obtained by contacting the parents of the subjects. The odds of having had a dog present in the household within the year previous to onset of illness were 3 to 1 for cases vs controls. An association of pups (< 3 months) in the households of OLM patients within one year of onset was statistically significant (P < .05

Expanded home-based care for the impaired elderly: solution or pipe dream?

Benefits assumed by those calling for massive expansion of home health care to include preventive, health-supportive services for the dependent aged--reduced institutionalization, reduced stress among family caregivers, and enhanced life-satisfaction for the dependent elderly--have been the objects of inadequate research and reflection. Advocates are unable to specify exactly what kind of person could benefit or who would seek to use such services. The little evidence that exists suggests that these benefits may be realized only partially and/or only among select segments of the population aff

Associations Between Hearing Impairment and Dysphagia with Cognitive Frailty in Nursing Homes Older Adults.

OBJECTIVE: To establish the prevalence of cognitive frailty (CF) among older adults residing in nursing homes and to evaluate the relationship between this condition and the prevalence of hearing impairment and dysphagia. MEASUREMENTS: A cross-sectional study of 451 individuals 60+ years of age living in nursing homes in Chengdu, Sichuan, China. The FRAIL and Chinese Mini Mental Status (CMMS) scales were respectively employed to measure physical frailty (PF) and cognitive functionality, with the results being used to separate subjects into four groups: cognitively intact non-PF individuals ("

Dementing illness: the patient, caregiver and community.

Care of the elderly patient with dementing illness involves not only the patient but also the family and other caregivers, and the community. An evaluation and treatment plan must go beyond the patient's immediate medical problems to examine the caregiver's needs and the community services available.

Occupational therapy for care home residents with stroke.

BACKGROUND: Stroke is a worldwide problem and is a leading cause of adult disability, resulting in dependency in activities of daily living (ADL) for around half of stroke survivors. It is estimated that up to 25% of all care home residents in the USA and in the UK have had a stroke. Stroke survivors who reside in care homes are likely to be more physically and cognitively impaired and therefore more dependent than those able to remain in their own home. Overall, 75% of care home residents are classified as severely disabled, and those with stroke are likely to have high levels of immobility,

Developing and evaluating online COVID-centric advance care planning training and information resources for nursing staff and family members in nursing homes: the necessary discussions study protocol.

BACKGROUND: Nursing home residents are typically older adults with high levels of chronic illness and impairment. As such, they are particularly susceptible to severe complications and mortality from COVID-19. Since all nursing home residents are at increased risk, nursing home care staff need to know what residents would want to happen should they become infected with COVID-19. This study aims to develop and evaluate advance care planning (ACP) COVID-centric online training and information resources for nursing home staff and family members of residents, to improve care at the end of life dur

Cost consequence analysis of transcutaneous tibial nerve stimulation (TTNS) for urinary incontinence in care home residents alongside a randomised controlled trial.

BACKGROUND: Urinary incontinence (UI) is prevalent in more than half of residents of nursing and residential care homes and can have a detrimental impact on dignity and quality of life. Care homes predominantly use absorbent pads to contain UI rather than actively treat the condition. Transcutaneous tibial nerve stimulation (TTNS) is a non-invasive, safe, low-cost intervention with demonstrated effectiveness for reducing UI in adults. We examined the costs and consequences of delivering TTNS to care home residents in comparison to sham (inactive) electrical stimulation. METHODS: A cost conseq

Eliciting and Honoring What Matters Most in the Nursing Home: A Primary Palliative Care Approach to Improving End-of-Life Outcomes.

BACKGROUND: Nursing homes provide a substantial proportion of end-of-life care but often face fragmented communication, inconsistent care planning, and limited access to specialty palliative care. These gaps highlight the need for scalable primary palliative care models. METHODS: The authors conducted a descriptive quality improvement project across 28 community nursing homes in northern California. A multicomponent primary palliative care model included clinician education, nurse care navigation, and system-level supports. Outcomes included documented goals-of-care conversations, hospice len

Participatory science to action: Radon literacy assessment and testing in an African American community.

Low radon testing rates in communities of color are health concerns with possible elevated lung cancer risks. Our objective was to use a participatory science approach to assess environmental health literacy specific to radon, i.e., radon literacy, and testing practices in a primarily African American community in Atlanta, Georgia. A leadership team of two academic researchers and a community organization leader engaged an intergenerational group of residents that included one adult team lead, three community elders, and three youth as community scientists, who contributed to research design,

[A comparative study of the characteristics and social backgrounds of frail and elderly persons at home, long-stay elderly hospital patients, and residents of welfare homes for the frail elderly].

Three studies were carried out in Mino City, Osaka Prefecture, on 188 frail and elderly persons living at home (the home group), 61 elderly patients who had been hospitalized more than six months (the inpatient group), and 72 residents of welfare homes for the frail elderly (the resident group). The characteristics and social backgrounds of the three groups were compared. About 30% of each group had suffered a stroke. As for ADL score, moderate disability was dominant in the home group, severe disability in the inpatient group, and slight disability in the resident group. The proportions of th

Intervention for burnout and irrational beliefs in parents of couples seeking a divorce: A critical reflection of Igbo-African marital discord.

BACKGROUND: Looking at the sociocultural dimension of marriage in the sub-region of Nigeria like Igbo, parents allow their sons and daughters to marry so that they can have homes. The expectation is for them to have permanent homes. If there is anything contrary to that such as divorce, parents usually frown at it. To some extent, the psychological effects of it may be deeply rooted in parents who hear that their children are seeking a divorce. Given this reason, this study investigated the effect of rational emotive family health therapy (REFHT) on burnout and irrational beliefs in parents of

Theoretical and hands-on guidance from dental hygienists promotes good oral health in elderly people living in nursing homes, a pilot study.

OBJECTIVES: Oral health in nursing homes for elderly is often unsatisfactory, and oral health education to nursing staff has not shown sufficient results why there is need for novel approaches. The aim of the study was to trial a new oral healthcare educational programme and to evaluate the effects on residents' oral health. In addition, attitudes among the nursing staff in the intervention nursing home were explored. METHODS: In a controlled clinical trial, two comparable nursing homes were randomly assigned for intervention or control. Interventions included weekly theoretical and hands-on
