There are multiple neurocutaneous syndromes that may show hair alterations such as the interglabellar peak or 'widow's peak', which is an alteration of the hair implantation, in addition to the genohypotrichosis, hypertrichosis and hair shaft dysplasias. This review highlights advances in our understanding of the genetic basis for alopecia areata. New findings in trichothiodystrophy, Bazex syndrome, and Netherton's syndrome are also discussed. Finally, abnormalities of the hair, such as atrophic midline bald patches, generalized follicular hamartoma, and loose anagen syndrome, may be associated with developmental defects.

The major hair shaft abnormalities are reviewed in two parts, discussing clinical characteristics, light and electron microscopy and, where information is available, the pathogenesis of these rare conditions. The manageability of the hair is dependent on the surface composition of the hair shaft. Exogenously caused damage leads to unmanageability by massive superficial defects in the hair cuticle. The symptoms of trichorrhexis nodosa and trichoptilosis are present, and superficial horny scales are bent up and curve under in tunnel-like fashions. Appropriate hair care can improve these defects. This study assessed the contribution of the apical hair cells to hearing. Guinea pigs, chinchillas and monkeys were behaviorally trained using positive reinforcement to respond to pure-tone stimuli. When a stable audiogram had been determined, each subject received one of three experimental treatments: ototoxic drug administration, low-frequency noise exposure, or the application of a cryoprobe to the bony wall of the cochlear apex. Menkes steely hair disease (MSHD) is a rare disorder which typically results in severe mental retardation and death in early childhood. A 21-month-old boy with an atypical milder form was presented by Procopis et al. [1981]. A second child with the atypical form is presented here who has survived to age 9 years and is doing well clinically.

Hair transplantation between monozygotic twins was used to repair a marked congenital, nonprogressive alopecia of the scalp. The alopecia was due to a follicular aplasia that, along with several other abnormalities of the integumentary system, affected only one of the twins. This case represents a unique application of isograft transplantation.

There has been no polytypical variation in human hairforms for at least the last 10,000 years. In view of the wide variety of hair forms, it must be assumed that biological adaptation has occurred. Several features of the scalp hair, such as form, color, thickness, density, maximal length, tensile strength, etc. are clearly genetically determined. When dealing with diseases of the hair, it may be useful for the dermatologist to distinguish between "hair" and "hair".

The problem of infection during hair transplant surgery in one dermatologist's office is described herein. Careful epidemiologic investigation identified the causative organism and its source, and the infection was effectively controlled by the use of "semi-sterile" technique. Infection may be more common than generally suspected and it should be considered as a possible cause of otherwise unexplained disappointing results as reflected by decreased numbers of viable hairs per graft.
