Non-tobacco nicotine dependence is associated with increased complications following clavicle open reduction internal fixation.

BACKGROUND: Tobacco dependence is well established to impair bone healing and increase postoperative complications. However, the independent effects of nicotine, separate from combustion-related factors, remain unclear. This study aimed to evaluate the impact of non-tobacco nicotine dependence (NTND) following open reduction and internal fixation (ORIF) of clavicle fractures, including midshaft and lateral fracture subtypes. METHODS: A retrospective cohort study was performed using a large national database. Patients undergoing ORIF of clavicle fractures were identified using procedural and d

Metal implantation and high-grade osteosarcoma of the femur: a case report and review of the literature.

BACKGROUND: Osteosarcoma is the most common primary malignant bone tumor, typically affecting adolescents and young adults. While its development is often associated with rapid bone growth and genetic factors, the potential link between metal implants and osteosarcoma remains unclear and rarely reported. This case highlights the unusual occurrence of high-grade osteosarcoma arising at the site of a long-standing metal implant, emphasizing the importance of long-term vigilance in patients with orthopedic hardware. Because implant-associated sarcomas are exceptionally uncommon and causality cann

Nine months after surgery, imaging revealed cage displacement. Intraoperative cultures during hardware removal identified C. glabrata. The patient underwent repeat irrigation and debridement with hardware exchange and was treated with intravenous micafungin and oral fluconazole and later transitioned to posaconazole. Eight patients suffered from persistent trochanteric pain and three patients underwent hardware removal. CONCLUSIONS: Early results for PF-LCP osteosynthesis show major complications in 7 of 19 …

The value of subtalar arthroscopy in the management of intra-articular calcaneus fractures.

UNLABELLED: A wealth of clinical and experimental data suggests, that anatomical restoration of the posterior calcaneal facet is a major predictor of outcome after intra-articular calcaneus fractures. The value of subtalar arthroscopy was examined in three clinical study groups (I-III. MATERIALS AND METHODS: (I) The subtalar joint was inspected arthroscopically in 28 patients during hardware removal one year after open reduction and internal fixation (ORIF). This procedure was accompanied by subtalar arthrolysis in all cases. (II) Open subtalar arthroscopy was performed to evaluate the qualit…

Distal Radius Fracture Fixation Devices and Their Radiographs.

Background: Distal radius fractures are among the most common fractures encountered in orthopedic practices. If treated operatively, most implants are retained after the fracture heals unless there is hardware failure, limitation of wrist motion, pain, infection, tendon rupture, or tenosynovitis. Complications have been reported during hardware removal, including not knowing the exact implant prior to its removal. If a patient presents for plate removal to a surgeon who did not perform the initial fracture fixation, having a preoperative visual aid can help the treating surgeon choose the righ…

Headless Compression Screw for Metacarpal Nonunion: Description of a New Technique and Report of 4 Cases.

Nonunion is a rare complication of metacarpal fractures and is usually associated with compound, open fractures with tissue loss and infection, or after failed surgical treatment. Repair by rigid osteosynthesis with plate and screws combined with autologous bone grafting has always been the treatment of choice for most patients. Such method allows early motion, although it has been proven that the presence of hardware may cause local soft tissue irritation. Often, hardware removal and tendon releases are necessary. We describe an original technique with a fixation method for the treatment of m…

A modified cement spacer technique for infected total hip arthroplasties with significant bone loss.

Complications related to femoral spacers are common during addressing infected total hip arthroplasties by 2-stage revision. We evaluated 11 patients who had 2-stage revisions with massive bone loss after removal of the infected components by a trochanteric osteotomy. All femoral cement spacers were assembled on intramedullary nails. Femurs were protected by a plate, whereas acetabuli were augmented by a cage or roof ring depending on the remaining bone stock. This additional hardware was covered with antibiotic-impregnated cement. Infection was eradicated in 10 (90.9%) of 11 patients within 3

Radial artery injury in association with fractures of the trapezium.

Three cases of trapezial fractures in which the radial artery was injured are reported. The injuries occurred during the initial trauma in one, when internal fixation was done in the second, and during hardware removal in the third. Six cadaveric wrists were dissected to study the relationship between the course of the artery and the trapezium.

[Results of modern osteosynthesis procedures in fractures of the radial head--are conservative therapy, bone pin osteosynthesis or primary resection still allowed?].

In a retrospective study, we evaluated the outcome of 38 patients who underwent open reduction internal fixation for displaced radial head fractures. We used resorbable material, as well as titanium implants which need no hardware removal. Our results were 84% good, 6% satisfying and 10% unsatisfying. The latter were due to more severe damage to the elbow joint.

Early Retrieval of Spanning Plates Used for Fixation of Complex Fractures of the Distal Radius.

Background  Spanning plates are being increasingly used for the treatment of complex fractures of the distal radius. The traditional recommendation is to leave the hardware in place for at least 12 weeks. Questions/Purpose  This study assesses the comparative outcomes of spanning plates removed at or before 10 weeks. We hypothesized that acceptable healing and functional outcomes can be achieved with earlier hardware removal to allow for earlier range of motion, rehabilitation, and return to function. Patients and Methods  All patients treated for a comminuted, intra-articular distal radius fr…
