Diagnostic electrocardiographic trap: A case of conduction system pacing.

His-bundle pacing (HBP) is a relatively new method of cardiac pacing, with recent studies showing an association between HBP and a lower risk of developing right ventricular heart failure compared to classical pacing methods. However, HBP is also associated with a higher risk of lead dislodgement, undersensing, and loss of capture. As such, a detailed assessment of pacing effectiveness in pacemaker patients is vital. In the presented case, an electrocardiogram (ECG) recording seems to present successful selective His-bundle pacing, while pacemaker follow-up demonstrated the loss of ventricular

Problems and prospects for finding new pharmacological agents among adenosine receptor agonists, antagonists, or their allosteric modulators for the treatment of cardiovascular diseases.

A1-adenosine receptors (A1AR) are widely distributed in the human body and mediate many different effects. They are abundantly present in the cardiovascular system, where they control angiogenesis, vascular tone, heart rate, and conduction. This makes the cardiovascular system A1AR an attractive target for the treatment of cardiovascular diseases (CVD). The review summarizes the literature data on the structure and functioning of A1AR, and analyzes their involvement in the formation of myocardial hypertrophy, ischemia-reperfusion damage, various types of heart rhythm disorders, chronic heart f

The atrioventricular junctions in Ebstein malformation.

OBJECTIVE: To review the anatomical structure of the right atrioventricular junction, including the specialised atrioventricular conduction system, in hearts with Ebstein's malformation, to identify potential substrates for the abnormalities in conduction. METHODS: Five heart specimens representing the morphological spectrum of Ebstein malformation were examined grossly and histologically. RESULTS: On the endocardial surface, the atrioventricular junction was marked by a faint line in two hearts, and by a small ridge in the other three. Analysis of the right parietal junction in four hearts

The pathophysiologic aspects and clinical implications of electrocardiographic parameters of ventricular conduction delay in repaired tetralogy of Fallot.

The 12-lead surface electrocardiogram is a valuable and feasible clinical tool in the management of patients following tetralogy of Fallot (TOF) repair. The importance of QRS duration in TOF patients has long been acknowledged. A prolonged QRS complex has been associated with increased risk for subsequent life-threatening ventricular arrhythmia and sudden cardiac death. Our current ability to risk-stratify TOF patients for malignant arrhythmogenic events primarily on the basis of QRS duration is rather limited. Nevertheless, increasing evidence suggests that QRS morphology and duration may be

Successful in utero therapy of fetal heart block.

OBJECTIVE: Congenital complete heart block is associated with maternal autoantibodies to Ro and La proteins, which injure the fetal cardiac conduction system. We administered dexamethasone to the mothers of five fetuses with heart block caused by maternal antibodies. STUDY DESIGN: We diagnosed five cases of fetal heart block at 20 to 23 weeks and treated all mothers with dexamethasone 4 mg orally each day for the remainder of the pregnancy. All patients were positive for anti-SS-A (anti-Ro) and/or anti-SS-B (anti-La) antibodies. RESULTS: Four patients had complete heart block, and one had se

Management of conduction disorders after transcatheter aortic valve implantation: results of the EHRA survey.

Conduction disorders such as left bundle branch block (LBBB) are common after transcatheter aortic valve implantation (TAVI). Consensus regarding a reasonable strategy to manage conduction disturbances after TAVI has been elusive. The European Heart Rhythm Association (EHRA) conducted a survey to capture contemporary clinical practice for conduction disorders after TAVI. A 25-item online questionnaire was developed and distributed among the EHRA electrophysiology (EP) research network centres. Of 117 respondents, 44% were affiliated with university hospitals. A standardized management protocol

Surgical bifascicular block.

Electrophysiological studies were performed in 18 patients who developed bifascicular block after repair of ventricular septal defect (VSD) or tetralogy of Fallot (TF). Two had transient complete heart block (CHB) in the immediate postoperative period. The P-A intervals were normal in all. A-H and H-V intervals were prolonged in three and four cases, respectively. Atrial pacing at progressively increasing heart rates was performed in 15 patients; two developed type II block distal to the His bundle (H). The effective and functional refractory periods (ERP and FRP) of the atrium (11 cases) were

[Transient atrioventricular conduction disorder: a potential cause of sudden death in myocarditis due to varicella. A case report].

The case of a patient with varicella skin lesions presenting with heart failure at 4 years of age is described. A transitory atrioventricular conduction disturbance due to myocardial impairment was documented during hospital stay. Despite the clinical improvement with conventional therapy, the patient died from respiratory failure due to a lung infection but with an almost normal electrocardiogram. Necropsy showed myocardial lesions compatible with acute myocarditis. A possible viral specific affinity for the conduction system of the heart is suggested and judged to be a potential cause of dea

Case report: anaplasma-related myocardial damage in a dog.

We present the case of a female dog that was evaluated following an episode of heart failure and was subsequently diagnosed with anaplasmosis. Cardiac assessment revealed evidence of myocardial injury, systolic dysfunction, and conduction system abnormalities. This case highlights the importance of considering Anaplasma phagocytophilum infection as a potential cause of myocarditis, especially in instances of unexplained heart failure and elevated troponin levels in the absence of other underlying conditions.

Severe Symptomatic Bradycardia Following First-Dose Oxcarbazepine in a Very Elderly Patient on Long-Term Carbamazepine: A Case Report.

We report a 91-year-old male patient who developed acute symptomatic bradycardia shortly after receiving his first standard dose of oxcarbazepine for trigeminal neuralgia, despite prior long-term tolerance to carbamazepine. A clear temporal association between oxcarbazepine administration and the onset of bradycardia was observed, while alternative causes were systematically excluded. Electrocardiography at baseline showed a marked intraventricular conduction delay. A subsequent ECG after drug discontinuation demonstrated an improved QRS duration, as well as new evidence of intra-atrial conduc

Complete atrioventricular block caused by pulmonary embolism: A case report and review of literature.

INTRODUCTION: Acute electrocardiogram abnormalities are common in patients afflicted with pulmonary embolism (PE). Rarely, a patient with acute pulmonary embolism and electrocardiogram changes may have an underlying conduction abnormality that complicates their course. CASE PRESENTATION: We describe a patient with known history of left bundle branch block who presented with acute PE and developed a right bundle branch block, which devolved into complete third degree heart block without ventricular escape rhythm and led to concomitant hemodynamic compromise. DISCUSSION: Given the rarity of th

Torsades de Pointes in Intravenous Drug Abuse-Associated Infective Endocarditis.

Intravenous drug abuse rates in the United States have increased exponentially in recent years. Ohio is one of 5 states with the highest age-adjusted drug overdose death rates, with drug overdose causing 39.1 of every 100,000 deaths. In patients who survive, the associated morbidity poses a significant public health burden. Infective endocarditis, defined as an infection of the endocardium of the heart, is a potentially lethal consequence of bacteremia related to intravenous drug abuse. Methicillin-sensitive Staphylococcus aureus (MSSA) is the most commonly implicated organism and may affect t

What is the attributing electrocardiographic phenomenon underlying a concomitant 2:1 and Wenckebach atrio-ventricular block?

We herein describe the case of a spontaneously observed bidirectional block (both entrance and exit block) at the sino-atrial junction. This bidirectional block at the sino-atrial junction associated with the failure of conduction to the ventricle revealed the Wenckebach periodicity of the atrio-ventricular conduction.
