Iatrogenic Articular Cartilage Injury in Arthroscopic Hip and Knee Videos and the Potential for Cartilage Cell Death When Simulated in a Bovine Model.

PURPOSE: To determine the incidence and characterize the severity of iatrogenic cartilage injuries. METHODS: Technique videos of arthroscopic femoral acetabular impingement procedures and meniscus repairs on VuMedi (n = 85) and Arthroscopy Techniques (n = 45) were reviewed and iatrogenic cartilage injuries were identified and graded (minor, intermediate, and major injury) by 2 independent reviewers. To demonstrate that even minor injuries on a cellular scale result in damage, a bovine osteochondral explant was used to create comparable minor iatrogenic injuries at varied forces that do not di

Left recurrent laryngeal palsy after left total hip arthroplasty; stretch injury due to inappropriate positioning in the lateral approach?

An 80-year-old man is described with a left-sided recurrent laryngeal nerve palsy after primary total hip arthroplasty. Intubation was uncomplicated. Aetiology is discussed. It might be caused by intubation injury, stretch-induced injury due to cervical hyperextension or inappropriate positioning during surgery in the lateral approach or a combination.

Evaluation of Preoperative Left Ventricular Relative Wall Thickness for Predicting Postoperative Acute Kidney Injury in Elderly Hip Fracture Patients.

Objectives: This study aimed to explore the association between left ventricular relative wall thickness (RWT) and postoperative acute kidney injury (AKI) in elderly patients who underwent hip fracture surgery. Additionally, we evaluated the prognostic value of RWT for postoperative clinical outcomes in this high-risk group. Methods: This prospective study included 131 patients aged ≥ 65 years who underwent surgery for femoral neck or intertrochanteric hip fractures. Preoperative echocardiographic parameters, including RWT, were recorded and their associations with postoperative AKI were analy

Endovascular repair of external iliac vein injury caused by acetabular screw penetration during total hip arthroplasty: a case report.

INTRODUCTION: Total hip arthroplasty (THA) is a common orthopedic procedure with low complication rates. However, vascular injuries, though rare (0.1%-0.3%), can lead to life-threatening hemorrhage and hemodynamic instability. External iliac vein (EIV) injuries during THA are exceptionally uncommon, and optimal management remains a clinical challenge. PRESENTATION OF CASE: We report a 62-year-old woman with diabetes and hypertension who underwent right THA for severe osteoarthritis. Intraoperatively, a significant hemorrhage occurred due to the acetabular screw penetration into the right EIV.

Characterizing peroneal nerve injury clinicoradiological patterns with MRI in patients with sciatic neuropathy and foot drop after total hip replacement.

OBJECTIVE: Sciatic nerve injury following total hip arthroplasty (THA) predominantly affects the peroneal division of the sciatic nerve, often causing a foot drop. This can result from a focal etiology (hardware malposition, prominent screw, or postoperative hematoma) or nonfocal/traction injury. The objective of this study was to compare the clinicoradiological features and define the extent of nerve injury resulting from these two distinct mechanisms. METHODS: Patients who developed a postoperative foot drop within 1 year after primary or revision THA with a confirmed proximal sciatic neuro

The effect of intra-operative hypotension on acute kidney injury, postoperative mortality and length of stay following emergency hip fracture surgery.

The association between intra-operative hypotension and postoperative acute kidney injury, mortality and length of stay has not been comprehensively evaluated in a large single-centre hip fracture population. We analysed electronic anaesthesia records of 1063 patients undergoing unilateral hip fracture surgery, collected from 2015 to 2018. Acute kidney injury, 3-, 30- and 365-day mortality and length of stay were evaluated to assess the relationship between intra-operative hypotension absolute values (≤ 55, 60, 65, 70 and 75 mmHg) and duration of hypotension. The rate of acute kidney injury wa

A Retrospective Analysis to Evaluate Seasonal Pressure Injury Incidence Differences Among Hip Fracture Patients in a Tertiary Hospital in East China.

Patients with a hip fracture are at high risk for pressure injury. A retrospective review of the electronic medical records of consecutive patients with a hip fracture treated in 2016 in a single tertiary hospital in east China were examined to investigate whether time of year affected the incidence of hospital-acquired pressure injury. Data collected included demographic characteristics (patient name, hospital number, age, gender, and body mass index); possible risk factors for pressure injury, such as presence of diabetes mellitus, admission hemoglobin, admission albumin, length of surgery,

Knee and hip loading patterns at different phases in the menstrual cycle: implications for the gender difference in anterior cruciate ligament injury rates.

BACKGROUND: Menstrual cycle phase has been correlated with risk of noncontact anterior cruciate ligament injury in women. The mechanism by which hormonal cycling may affect injury rate is unknown. HYPOTHESES: Jumping and landing activities performed during different phases of the menstrual cycle lead to differences in foot strike knee flexion, as well as peak knee and hip loads, in women not taking an oral contraceptive but not in women taking an oral contraceptive. Women will experience greater normalized joint loads than men during these activities. STUDY DESIGN: Controlled laboratory stud

Pre- and post-alpha motoneuronal control of the soleus H-reflex during sinusoidal hip movements in human spinal cord injury.

The aim of this study was to establish the contribution of hip-mediated sensory feedback to spinal interneuronal circuits during dynamic conditions in people with incomplete spinal cord injury (SCI). Specifically, we investigated the effects of synergistic and antagonistic group I afferents on the soleus H-reflex during imposed sinusoidal hip movements. The soleus H-reflex was conditioned by stimulating the common peroneal nerve (CPN) at short (2, 3, and 4 ms) and long (80, 100, and 120 ms) conditioning test (C-T) intervals to assess the reciprocal and pre-synaptic inhibition of the soleus H-r

Predicting the effects of muscle activation on knee, thigh, and hip injuries in frontal crashes using a finite-element model with muscle forces from subject testing and musculoskeletal modeling.

In a previous study, the authors reported on the development of a finite-element model of the midsize male pelvis and lower extremities with … The simulation results suggest that lower-extremity muscle tension has little effect on the risk of hip injuries, but it increases the bending moments in the femoral shaft, thereby increasing the risk of femoral shaft fractures by 20%-40%. However, these findings may be affected by the inability of the AnyBody Model to appropriately predict antagonistic muscle …

Delayed Ischemic Hepatocellular Injury Following Cemented Total Hip Arthroplasty: A Case Report Within the Spectrum of Bone Cement Implantation Syndrome.

Bone cement implantation syndrome (BCIS) is classically associated with acute intraoperative cardiopulmonary disturbances during cemented arthroplasty. However, accumulating clinical observations suggest that its systemic manifestations may extend beyond the immediate peri-cementation period. Hepatic involvement remains rarely reported and is poorly characterized, particularly in frail elderly patients with limited physiological reserve. We report the case of an 82-year-old woman who developed severe but reversible ischemic acute liver failure with concomitant acute kidney injury following cem

A Rare Case of Ipsilateral Floating Hip with Femoral Neck Fracture and Contralateral Floating Knee Injury - Proposal for a Management Flowchart and Literature Review.

INTRODUCTION: Femoral neck fractures can complicate by non-union and/or hip avascular necrosis (AVN); moreover, the incidence of AVN remains obscure when it is accompanied by an ipsilateral acetabular fracture. Measures to reduce non-union or hip AVN with this complex injury pattern are not well addressed in the literature. CASE REPORT: We report a case of a young male patient who sustained ipsilateral acetabular, femoral neck, and shaft fractures in addition to a contralateral floating knee injury following a motor vehicle collision. To the best of our knowledge, this is the first report tha

A COMBINED TREATMENT APPROACH EMPHASIZING IMPAIRMENT-BASED MANUAL THERAPY AND EXERCISE FOR HIP-RELATED COMPENSATORY INJURY IN ELITE ATHLETES: A CASE SERIES.

BACKGROUND/PURPOSE: Athletes experiencing hip, groin, and low back pain often exhibit similar clinical characteristics. Individuals with hip, groin and low back pain may have the presence of multiple concurrent pathoanatomical diagnoses. Regardless, similar regional characteristics and dysfunction may contribute to the patient's chief complaint, potentially creating a sub-group of individuals that may be defined by lumbopelvic and hip mobility limitations, motor control impairments, and other shared clinical findings. The purpose of this case series is to describe the conservative management o
