A HiBiT-Tagged influenza A virus reporter system reveals cytokine-inhibitory micropeptide 53 as a modulator of viral infection and immunopathology.

Influenza A virus (IAV) remains a major threat to human and animal health, highlighting the need for efficient approaches to identify host factors and antivirals. Reporter viruses are critical tools for these efforts, but incorporating large reporter genes often compromises viral fitness and genetic stability. Here, we developed a replication competent reporter IAV by fusing an 11-amino acid HiBiT tag into the nonstructural protein 1 (NS1) of the A/WSN/1933 (H1N1) backbone. The modified virus retained parental virion morphology, comparable replication kinetics, and in vivo tissue tropism, whil

Recent advances in flavivirus encephalitis.

PURPOSE OF REVIEW: Flaviviruses are an increasing public health concern, responsible for a broad spectrum of human disease ranging from asymptomatic or mild febrile illness to severe neuroinvasive infections such as encephalitis. Flavivirus encephalitis is associated with substantial mortality and long-term neurological sequelae, yet no specific antiviral therapy is currently available. Diagnosis remains challenging because of transient viremia and serological cross-reactivity, and preventive strategies are unevenly implemented. This review summarizes recent advances in the diagnosis, prognost

Comparative evaluation of multimodal point-of-care tests to differentiate gram-negative from gram-positive infections in critically ill adults: a diagnostic accuracy study.

PURPOSE: Patients with suspected sepsis often receive broad-spectrum antibiotics before culture results are available. A rapid point-of-care test (POCT) that indicates Gram-negative (GN) versus Gram-positive (GP) infection could help tailor empiric therapy. We systematically compared available POCT for GN/GP differentiation and, for biomarkers, examined clinically usable thresholds. METHODS: PubMed, Embase, Web of Science and the Cochrane Library (January 2005 to August 2025) were searched for studies in adults with sepsis that evaluated rapid tests to distinguish GN from GP infection. We poo

Human Cytomegalovirus Infection in Haematopoietic Stem Cell Transplant Recipients and CAR-T Cell Recipients-PART 2: Antiviral Therapy and Virus-Specific T Cell Therapy for HCMV in Allo-HSCT.

Human cytomegalovirus (HCMV) reactivation is a common and significant complication after allogeneic haematopoietic stem cell transplantation (allo-HSCT), causing potentially life-threatening disease. Antiviral therapy approaches for HCMV are often limited by toxicities and the risk of developing antiviral drug resistance. This review article (Part 2) provides an overview of current antiviral pharmacotherapies for HCMV and the application of virus-specific adoptive T cell therapies for the prevention or treatment of HCMV reactivation in allo-HSCT recipients. The number of available antiviral dr

A quorum sensing antagonist with dual bacterium- and host-directed anti-pathogenic activity against Pseudomonas aeruginosa.

Pseudomonas aeruginosa is a major opportunistic pathogen whose clinical management is increasingly compromised by antibiotic resistance and biofilm-associated persistence. Anti-pathogenic strategies that attenuate virulence without inhibiting bacterial growth offer a promising alternative to conventional antibiotics. Here, we identify the Spongosorites sp-derived bisindole alkaloid (R)-6'-debromohamacanthin B (RDB) as a potent dual bacterium- and host-directed anti-pathogenic agent against P. aeruginosa. RDB suppressed quorum sensing (QS) activity and markedly inhibited biofilm formation at gr

The single-cell immune atlas of bacterial pneumonia: from inflammatory cell infiltration to treatable cell states.

Bacterial pneumonia remains a major cause of infectious mortality, and antimicrobial resistance has intensified interest in host-directed therapy (HDT). Recent bronchoalveolar lavage fluid (BALF) and peripheral blood atlases (2024-2025) provide single-cell evidence from human disease, but the main translational gap is prioritization rather than catalog expansion. In this review, we reorganize published datasets from bacterial pneumonia and related lung infection cohorts (>500,000 cells; 2020-2026) into a state-prioritization scheme termed "treatable cell states." Evidence is currently stronges…

Sirtuin2 blockade inhibits replication of human immunodeficiency virus-1 and Mycobacterium tuberculosis in macrophages and humanized mice.

Co-infections with Mycobacterium tuberculosis (Mtb) and human immunodeficiency virus-1 (HIV-1) present a critical health challenge and require treatment for patient survival. We found that human M1 macrophages inhibit Mtb growth, while M2 macrophages, characterized by elevated Sirt2 expression, permit Mtb growth. Further, we found that HIV-1 augmented Sirt2 gene expression in MФs. Therefore, we explored the therapeutic potential of sirtuin-modulating drugs using human MФs. Sirtinol, a Sirt2 inhibitor, significantly reduced HIV-1 growth in M0, M1, and M2-MФs by >1log10 over 7 days. Conversely,

Pharmacological reduction of neutrophil infiltration reduces Clostridioides difficile infection severity.

Clostridioides difficile is the leading cause of nosocomial infections and an urgent public health threat. This bacterial pathogen is challenging to treat due to antibiotic resistance and high recurrence rates, highlighting the need for additional therapeutic strategies. Host inflammatory responses are major drivers of disease severity during C. difficile infection (CDI), yet few therapeutic strategies target these pathways. Here, we show that the prostaglandin E1 (PGE1) analog misoprostol markedly reduces CDI severity by modulating host immune responses. During CDI, misoprostol decreases circ

Marine Streptomyces-derived 2,4-di-tert-butylphenol enhances autophagy to eliminate mycobacteria in human macrophages.

Mycobacterium tuberculosis (M. tb), the causative agent of tuberculosis (TB), remains a major global health threat, highlighting the need for novel therapeutic strategies. Host-directed therapies (HDTs), particularly autophagy induction via natural or pharmacological agents, offer promising strategies for combating mycobacteria. This study assessed the anti-mycobacterial potential of marine-derived Streptomyces spp. Briefly, the isolated marine Streptomyces spp. underwent primary and secondary screenings to assess their inhibitory activity against M. smegmatis. Notably, the cell-free extract o

Understanding HBV immunopathogenesis to inform future immune-based therapies.

Chronic hepatitis B virus (HBV) infection remains a major cause of cirrhosis and hepatocellular carcinoma (HCC) worldwide, with more than 250 million people living with chronic infection despite effective prophylactic vaccination. While current treatments provide durable suppression of viral replication and reduce liver-related complications, they rarely achieve functional cure, defined as sustained hepatitis B surface antigen (HBsAg) loss after treatment cessation. Viral persistence is maintained by intrahepatic covalently closed circular DNA and HBV DNA integration, which sustain antigen pro

Current and emerging therapies for pulmonary tuberculosis in adults.

Tuberculosis remains the biggest global killer from infectious causes, with an estimated 1.25 million deaths per year. The quadruple drug regimen of rifampicin, isoniazid, pyrazinamide, and ethambutol has been standard therapy for 40 years, with good outcomes in patients with drug susceptible tuberculosis who are able to tolerate the drug regimen. However, the frequency of adverse drug reactions and emergence of drug resistant tuberculosis has created demand for new treatment strategies: there is now a surge in novel tuberculosis agents and regimens entering human trials. For decades, drug res

Septic shock and sepsis syndrome in obstetric patients.

Septic shock is a life-threatening clinical syndrome that, despite its rare occurrence in obstetrics, remains a leading cause of maternal mortality. Its pathophysiology is explained by a profound systemic response to a complex variety of host cellular and humoral mediators elaborated after exposure to microbial toxins. Early recognition, prompt diagnostic workup, and immediate initiation of therapy improve outcomes. Therefore, recent publications have popularized the concept of the "sepsis syndrome," a preshock list of clinical criteria associated with progressive sepsis. Needed diagnostic stu

Treatment of Tuberculous Meningitis and Its Complications in Adults.

PURPOSE OF REVIEW: Tuberculous meningitis (TBM) is a global health problem. In this review, we systematically evaluate the evidence for current and emerging antimicrobials, host-directed therapies and supportive managements. RECENT FINDINGS: Current antimicrobial regimes do not factor the differing ability of drugs to cross the blood-brain barrier. Rifampicin may be more effective at higher doses yet the most recent clinical trial failed to demonstrate survival benefit at 15 mg/kg/day. Dose finding studies suggest that higher doses still may be safe and more effective. Fluoroquinolones are cu

IFN-γ-related mRNA profile predicts clinical response to PD-1 blockade.

Programmed death-1-directed (PD-1-directed) immune checkpoint blockade results in durable antitumor activity in many advanced malignancies. Recent studies suggest that IFN-γ is a critical driver of programmed death ligand-1 (PD-L1) expression in cancer and host cells, and baseline intratumoral T cell infiltration may improve response likelihood to anti-PD-1 therapies, including pembrolizumab. However, whether quantifying T cell-inflamed microenvironment is a useful pan-tumor determinant of PD-1-directed therapy response has not been rigorously evaluated. Here, we analyzed gene expression profi

Impact of SARS-CoV-2 Resistance to Antiviral Monoclonal Antibody Therapy on Neutralizing Antibody Response.

BACKGROUND: Anti-SARS-CoV-2 monoclonal antibodies (mAbs) have played a key role as an anti-viral against SARS-CoV-2, but there is a potential for resistance to develop. The interplay between host antibody responses and the development of monoclonal antibody (mAb) resistance is a critical area of investigation. In this study, we assessed host neutralizing antibody (nAb) responses against both ancestral virus and those with treatment-emergent E484K bamlanivimab resistance mutations. METHODS: Study participants were enrolled in the ACTIV-2/Advancing Clinical Therapeutics Globally (ACTG) A5401 ph

The Use of Adjunctive Interferon-γ Therapy for Complex Mycobacterial Disease in Patients with Advanced HIV: A Case Series.

Management of mycobacterial opportunistic infection remains challenging in patients living with HIV, particularly when infection persists despite antiretroviral therapy (ART) and optimised antimycobacterial therapy (AMT). Interferon-γ is a key mediator of macrophage activation and intracellular pathogen killing, providing a rationale for its use as adjunctive host-directed therapy in selected patients. We describe three patients with advanced HIV and complex mycobacterial disease treated with adjunctive interferon-γ. One patient had disseminated tuberculosis with recurrent psoas abscesses afte…
