Intracranial pressure and conductance to outflow of cerebrospinal fluid in normal-pressure hydrocephalus.

Forty patients with clinical evidence of normal-pressure hydrocephalus were studied by monitoring intraventricular pressure during a 24-hour period, and by a lumboventricular perfusion test for measurement of the conductance to outflow of cerebrospinal fluid (CSF). The purpose of the study was to investigate whether there is a relationship between intraventricular pressure and conductance to outflow of CSF, and whether it is possible to use the results from pressure monitoring in the selection of patients who may be expected to benefit from shunting therapy. The conductance to outflow was used

Standardizing Normal Pressure Hydrocephalus Diagnosis: The Interdisciplinary Lausanne's Protocol.

INTRODUCTION: Idiopathic normal pressure hydrocephalus (iNPH) is a reversible neurological condition characterized by gait disorders, cognitive impairment, and urinary incontinence with ventriculomegaly at brain imaging. Due to the diagnostic complexity, we developed a structured multidisciplinary protocol to improve iNPH diagnosis. We present here the results of the first year after its implementation. METHODS: A multidisciplinary team involving neurosurgeons, physical therapists, neuropsychologists, and neurologists at the Lausanne University Hospitals (Switzerland) organized a diagnostic p

Idiopathic normal pressure hydrocephalus concomitant with progressive supranuclear palsy.

INTRODUCTION: Recent studies by us and others have unveiled a frequent coexistence of idiopathic normal pressure hydrocephalus (iNPH) with neurodegenerative movement disorders, including progressive supranuclear palsy (PSP). This study aims to explore the clinical and radiological characteristics of patients with iNPH who also had comorbid with PSP, referred to as iNPHc + PSP. METHODS: We retrospectively analyzed patients with iNPH admitted to our department between 2009 and 2024. We adhered to the established clinical criteria for iNPH and the Movement Disorder Society (MDS)-PSP criteria for

Familial vs sporadic normal pressure hydrocephalus: a comparative study.

INTRODUCTION: Normal pressure hydrocephalus (NPH) is a syndrome characterized by the buildup of cerebrospinal fluid that results in the clinic triad of gait impairment, urinary incontinence, and cognitive impairment. NPH can be categorized as secondary, idiopathic, or familial. Here, we report a comparison of sporadic to familial types from clinical, radiological, and surgery response aspects as well as a novel gene mutation as a cause of familial NPH. METHOD: We analyzed 139 patients evaluated for NPH at our center from 2010 to 2022. Ninety-five patients diagnosed with probable (n = 26) or d

Hakim's disease: an update on idiopathic normal pressure hydrocephalus.

INTRODUCTION: Idiopathic normal pressure hydrocephalus (iNPH) increases with age but is still underdiagnosed and undertreated. In the last decade, iNPH research has expanded into understanding broader contributions to iNPH, the role of cerebrospinal fluid (CSF), and imaging biomarkers to aid early detection, help diagnosis and differentiation from iNPH mimics, and aid with outcome prediction. EVIDENCE ACQUISITION: We performed a literature search on the PubMed database. English language articles published between 2015-2024 were included. The strategies focused on iNPH and specific terms relat

Determinants of clinical response in possible normal pressure hydrocephalus.

OBJECTIVES: This study aims to identify which clinical factors should be considered when determining a diagnosis of probable idiopathic Normal Pressure Hydrocephalus (iNPH) in patients with possible iNPH, and to explore alternative assessments that can complement lower extremity motor evaluations. PATIENTS AND METHODS: We conducted a prospective observational study involving 41 patients with possible iNPH. The assessments included the 10-Meter Walk Test (10MWT), a comprehensive neuropsychological battery, the Finger Tapping Test, and radiological measurements. Patients who exhibited a 20% or

Knowledge of idiopathic normal pressure hydrocephalus among healthcare professionals.

Idiopathic normal pressure hydrocephalus (iNPH) is a prevalent, yet greatly underdiagnosed condition in older adults, which can cause significant disability if left untreated. Reduced knowledge among healthcare professionals could contribute to the existing mismatch between the prevalence and treatment rates of iNPH. This study aimed to determine the level of knowledge on iNPH amongst healthcare professionals working in Neurology, Neurosurgery, Geriatrics and other relevant acute services, as well as primary and community care. A cross-sectional study, based on retrospectively extracted data f

[Movement Disorders and Rehabilitation in Idiopathic Normal Pressure Hydrocephalus].

Movement disorders, particularly gait and balance disturbances can lead to falls and reduced daily activities in patients with idiopathic normal pressure hydrocephalus (iNPH). In this study, we investigate movement disorders from both the pathophysiological and kinematic perspectives in patients with iNPH. Additionally, we discuss essential factors that should be evaluated before and after cerebrospinal fluid tap tests and shunt surgeries and considerations for assessment of fall risk in patients with iNPH. Additionally, we describe the most recent findings on rehabilitation of iNPH patients.

Automated Segmentation and Connectivity Analysis for Normal Pressure Hydrocephalus.

Objective and Impact Statement. We propose an automated method of predicting Normal Pressure Hydrocephalus (NPH) from CT scans. A deep convolutional network segments regions of interest from the scans. These regions are then combined with MRI information to predict NPH. To our knowledge, this is the first method which automatically predicts NPH from CT scans and incorporates diffusion tractography information for prediction. Introduction. Due to their low cost and high versatility, CT scans are often used in NPH diagnosis. No well-defined and effective protocol currently exists for analysis of

Alpha-Synuclein RT-QuIC in Idiopathic Normal Pressure Hydrocephalus.

This study quantified the occurrence of an underlying synucleinopathy in 50 patients with idiopathic normal pressure hydrocephalus by means of real-time quaking-induced conversion, a highly sensitive and specific technique capable of detecting and amplifying misfolded aggregated forms of α-synuclein in the cerebrospinal fluid. Seven patients were positive and they did not differ from negative cases, except for a more frequent L-dopa responsiveness and gait characterized by a wider base. The two groups did not differ in terms of response rate to tap test or shunt surgery, although step length a

Normal-pressure hydrocephalus: A rare cause of reversible dementia.

Normal-pressure hydrocephalus (NPH) is characterized by the clinical triad of dementia, gait instability, and urinary incontinence. The estimated annual incidence is 1.8 cases in 100,000 persons, making NPH a rare diagnosis and uncommon cause of dementia. NPH is a form of communicating hydrocephalus that can easily be missed in older adults with multiple comorbidities, so clinicians must exclude reversible causes of dementia before diagnosing irreversible causes such as Alzheimer disease.

A prospective study on gait dominant normal pressure hydrocephalus.

OBJECT: We characterize idiopathic normal pressure hydrocephalus (NPH) following treatment with lumbar puncture (LP) and shunt placement through clinical evaluation and quantitative ProtoKinetics Zeno walkway assessments. We evaluate the symptomology by determining gait characteristics altered by treatment. METHODS: Patients at Barrow Neurological Institute (BNI) who underwent a LP, removing 30-32 mL cerebrospinal fluid) followed by ventriculoperitoneal shunt placement in February 2015 to February 2017 were analyzed for gait impairments. Inclusion in the study required a diagnosis of NPH, no
