The proximal ureter atresia as a cause of a congenital hydronephrosis with functioning normal kidney is to a certain extent unknown. The histological elaboration of an uretero-pelvic resection preparation of a 7-year-old boy after Anderson-Hynes-plasty showed the transition of a stenosis of the ureter passage into a ureter atresia. Cause of the development of a proximal ureter atresia are secondary changes of the structure in the ureteropelvic segment due to the disturbed urodynamics in stenosis of the ureter passage. The possibilities of the genesis of atresia are discussed and it is referred…

A retrospective review of 63 children with primary pelvic hydronephrosis has revealed many features similar to those in adults. A plea is made for the greater use of pressure/flow studies in the renal pelvis in the assessment of hydronephrosis. The value of a modified and simplified nephroplasty as an adjunct to pyeloplasty in the more advanced obstructive cases is described and its greater use encouraged.

Congenital hydronephrosis of the lower segment in duplex kidney was treated in 12 patients. This uncommon abnormality carries the risk of calculous formation, chronic urinary tract infection, and functional failure of the involved segment. Diagnostic problems encountered included differentiation from tumor, obstruction by a calculus, and hydronephrosis due to reflux. Treatment was individualized according to severity of hydronephrosis and presence or absence of specific complications. Mild obstruction could be treated nonsurgically while the patient was closely observed for evidence of stone, …
