Hypergastrinemia and enterochromaffin-like cell hyperplasia.

UNLABELLED: The enterochromaffin-like (ECL) cells, the most frequent endocrine cells of the oxyntic mucosa of the stomach, are under the tro … multiple biopsies of the gastric body mucosa for the evaluation of qualitative pattern of ECL cells hyperplasia, and basal fasting gastrin determination. A sample of hypergastrinemic patients from each group was further investigated by meal-stimulation of gastrin secretion and quantitative morphometry for CgA positive gastric body endocrine cells. RESULTS: A …

[Condylar hyperplasia. Early detection is desired].

When single-sided excessive condylar growth is detected in time, removal of the affected condyle is sufficient. In cases of late detection, extensive surgery of upper and lower jaw and the chin may be necessary.

Lymphoid hyperplasia causing recurrent rectal prolapse.

An 8-year-old girl had a 5-month history of recurrent rectal prolapse. On colonoscopy, two submucosal masses were noted in the distal rectum and diagnosed by biopsy as benign lymphoid hyperplasia. These were excised by limited dissection superficial to the submucosa, and the histologic diagnosis was confirmed. The child has done well after removal of the nodules, with no subsequent prolapse for more than 2 years.

Prostatic stents for benign prostatic hyperplasia.

Treatment options for benign prostatic hyperplasia (BPH) have developed rapidly over the past several years. These options include surgical intervention, medical therapy, and minimally invasive modalities such as prostatic stents. Prostatectomy remains the treatment of choice for symptoms that are the result of bladder outflow obstruction, with success rates of 85-90%. Medical therapy with selective alpha-1 antagonists, 5 alpha-reductase inhibitors, LHRH antagonists, antiandrogens, and aromatase inhibitors have increased in popularity and are mainly used to treat patients with mild to moderate…

Benign prostatic hyperplasia. Practical treatment guidelines.

Symptomatic benign prostatic hyperplasia (BPH) is a common condition in older men and has a significant impact on their daily lives. Transurethral resection of the prostate (TURP) or open prostatectomy are currently the most effective therapies for BPH. TURP is, however, associated with clinically significant adverse events in 20% of patients. Therefore, patients who need treatment for BPH based on the presence of symptoms should also be offered other therapy options. Transurethral incision of the prostate is an effective therapy with minimal adverse effects in patients with a prostate not lar…

[Drug therapy of benign prostatic hyperplasia].

BPH patients with Vahlensieck stage II or III disease are suitable for drug treatment. The points of attack are reduction of testosterone, conversion of testosterone to dihydrotestosterone, conversion of testosterone to estrogen using GnRH analogues, antiandrogens and alpha reductase inhibitors or aromatose inhibitors. Furthermore a reduction in obstruction is achieved through the use of phytopharmaceuticals containing 5-lipoxygenase and cyclooxygenase inhibitors. At present, Curcurbitae pepo seeds, Urtica dioica root, Pollinis siccae extract and Sabal serrulata seed extract are approved for t

[Pituitary enlargement in congenital adrenal hyperplasia].

The aim of the suppression of pituitary adrenal axis in the management of patients with congenital adrenal hyperplasia is to reduce the hypersecretion of adrenal androgens. However, the local consequences of the hyperreactivity of corticotropin cells during inadequate therapy is not well defined. In this case report, we present the observation of a 28-year-old patient with congenital adrenal hyperplasia with inadequate treatment in whom a pituitary enlargement was detected after MR imaging for headaches. This observation underlines the need for a pituitary follow-up in these specific patients.

Malignant transformation of nasopharyngeal lymphoid hyperplasia.

Lymphadenopathy and nasal obstruction are very common in the HIV-positive patient and may or may not reveal a nasopharyngeal tumor. Biopsy is warranted if there is evidence suggestive of lymphoma or other neoplastic disease. This would include progressive rapid enlargement observed on examination or CT scan, an asymmetric growth pattern, and evidence of invasion of normal anatomic boundaries. It is certain that not all cases of adenoid hypertrophy that occurs early in the course of HIV disease represent a malignancy. Further study is needed to determine the proper timing for nasopharyngeal bio

Managing patients with benign prostatic hyperplasia.

Benign prostatic hyperplasia is the usual cause of prostatism--irritative and obstructive urinary symptoms. Steps in making the diagnosis include a focused neurologic examination, urinalysis, serum creatinine determination and, possibly, catheterization to detect urinary retention. It is difficult to predict the likelihood of future complications, such as complete urinary obstruction, even for patients with severe symptoms. The natural course of prostatism is a waxing and waning of symptoms. Treatment options are watchful waiting, medication, transurethral prostatectomy, and newer surgical tre…

[Hormonal therapy of benign prostatic hyperplasia].

In the last 30 years transurethral prostatectomy (TURP) has been the standard treatment for benign prostatic hyperplasia. While the mortality rate of TURP has been reduced to zero, the postoperative morbidity rate has remained unchanged at 15-18% for several decades. Increasing costs and the preference of many patients for non-surgical treatment have now led to the development of new pharmacological treatment options. All hormone therapeutic modalities known to date, such as GnRH-agonists, cyproteronacetate and flutamide, have acted on the male hypothalamic-pituitary-gonadal system. Because of…

Pathophysiology of clinical benign prostatic hyperplasia.

The pathophysiology of clinical BPH has been attributed to bladder outlet obstruction resulting from the enlarged prostate. A direct relationship does not exist between prostate size, symptom severity, or bladder outlet obstruction. The pathophysiology of prostatism is most likely multifactorial. Elucidating the factors contributing to symptoms will likely provide the foundation for the development of new pharmacological alternatives for the medical treatment of BPH.

Angiofollicular lymph node hyperplasia (Castleman's disease).

This review provides a comprehensive assessment of angiofollicular lymph node hyperplasia (ALNH) or Castleman's disease including pathogenesis, clinical presentation, histomorphologic and immunophenotypic findings, laboratory results, treatment, and prognosis. A division of ALNH into clinically relevant subtypes provides a framework for the consideration of the disorder. A comprehensive search of the medical literature involving ALNH using Medline was performed. Reports judged to be significant for the understanding of the disorder were analyzed and their findings incorporated into this review…

[Adenomatoid hyperplasia of minor salivary glands].

Adenomatoid hyperplasia of minor salivary glands in an uncommon clinicopathologic entity, first reported, in 1971, by Giansanti and cols. The condition mimics a neoplasm because of its swelling, but the histologic picture agrees with that of normal appearing salivary gland tissue. The interest of this entity is that although benign pseudotumoral lesion, it can be clinically confused with benign or malignant tumors and even, through fine needle aspiration cytology, with low grade mucoepidermoid tumors. We present one case of this condition arising in the soft palate in a patient with unilateral…

Proliferative verrucous leukoplakia and verrucous hyperplasia.

Proliferative verrucous leukoplakia and hyperplasia are irreversible clinicopathologic lesions of the oral cavity and upper respiratory tract with considerable potential for evolving into verrucous or other forms of squamous cell carcinoma. Proliferative verrucous leukoplakia is a disease of the oral cavity in which verrucous hyperplasia is a part of its developmental spectrum. Verrucous hyperplasia, at other head and neck mucosal sites, may be associated with papillomas or be a de novo lesion. Human papillomavirus, as a cofactor, plays a role in some of the lesions. In the instance of verruco…

[Laser treatment of benign prostatic hyperplasia].

This review article describes the different methods of laser treatment of benign prostatic hyperplasia and their development. Published treatment results are compared with our own results obtained with different procedures. The aim of therapy is to reduce the volume of the gland by coagulation, with subsequent secondary ablation or primary vaporization. Due to the desired volume effects Nd:-YAG lasers are used almost exclusively. The technique most frequently used is transurethral laser coagulation of the prostate. Radiation is done in the non-contact mode with beam-detecting applicators, with…

Adenomatous hyperplasia of equine allantoic epithelium.

Adenomatous hyperplasia with or without cystic formation of allantoic epithelium was observed in 63/954 equine placentas examined from 1 February 1988 to 31 January 1990. In 61/63 placentas, the adenomatous hyperplasia was associated with other placental lesions: 49 with chronic or chronic-active placentitis, six with placental edema, three with fetal diarrhea, one with placentitis and fetal diarrhea, one with fetal diarrhea and placental edema, and one with hyperplasia of chorionic epithelium. When lesions were less severe, the hyperplastic lesions were not grossly visible, but when lesions w…
