Diatrizoate absorption from enemas in newborn puppies.

Serum iodine levels increased slightly 15 min. after Gastrografin enemas were given to 20 healthy neonatal puppies. The systemic dehydration which occurs in the neonate following an hypertonic enema primarily reflects an osmotic drawing of water into the bowel, although systemic absorption of diatrizoate through the intact gut wall enhances the alteration in serum osmolarity.

Effect of urea concentration of pelvic fluid on renal concentrating ability.

The role of the pelvic urine in the renal concentration process was examined in anesthetized antidiuretic rats. The left ureter was cannulated with perfusion and collection catheters so that the composition of the fluid bathing the papilla could be altered by superperfusing solution of known composition without opening the renal pelvis, exposing the papilla, or altering intrapelvic pressure. The effect on urinary osmotic pressure (Uosmol) of papillary superperfusion with solutions of varying urea concentrations and total osmolality was examined. Papillary superperfusion with solutions containi

Elevated spinal monoamine neurotransmitters after antenatal hypoxia-ischemia in rabbit cerebral palsy model.

We hypothesized that a deficiency in the descending serotonergic input to spinal cord may underlie postnatal muscle hypertonia after global antenatal hypoxic-ischemic injury in a rabbit model of cerebral palsy. Neurotransmitter content was determined by HPLC in the spinal cord of newborns with and without muscle hypertonia after fetal global hypoxic-ischemic brain injury and naïve controls. Contrary to our hypothesis, serotonin levels in both cervical and lumbar expansions and norepinephrine in cervical expansion were increased in hypertonic kits relative to non-hypertonic kits and controls, w

Liquid volume, Na+ and mannitol concentration in a hypertonic mannitol-Ringer hydrothorax.

In anesthetised rabbits with a 2 ml hypertonic mannitol-Ringer hydrothorax in the right space 30 mM/L mannitol were required for an unchanged volume of the hydrothorax after 60 min. [Na+] in the pleural liquid 10, 30 and 60 min after this hydrothorax was 8, 7 and 5 mEq/L, respectively, lower (P less than 0.01) than the initial one and that in a Ringer-hydrothorax. This seems due to the active transport of Na+ out of the pleural space followed by little water because of the osmotic pressure exerted by mannitol. This finding provides further evidence for an active transport without using inhibit

[Proceedings: Home treatment of chronic respiratory insufficiency: carrier solutions for aerosol inhalations].

The excellent bronchodilatory effect of inhalation is widely accepted today. In order to obtain good results, correct instruction of the patient and correct choice of the nebulizer and inhalation solution are necessary. The performance requirements of the nebulizer and the most frequently used carrier solutions are critically discussed.

The anorectic and thermogenic effects of pharmacological lactate in male mice are confounded by treatment osmolarity and co-administered counterions.

Lactate is a circulating metabolite and a signalling molecule with pleiotropic physiological effects. Studies suggest that lactate modulates energy balance by lowering food intake, inducing adipose browning and increasing whole-body thermogenesis. Yet, like many other metabolites, lactate is often commercially produced as a counterion-bound salt and typically administered in vivo through hypertonic aqueous solutions of sodium L-lactate. Most studies have not controlled for injection osmolarity and the co-injected sodium ions. Here, we show that the anorectic and thermogenic effects of exogenou

The preliminary results of a comparative effectiveness evaluation of adhesiolysis and caudal epidural injections in managing chronic low back pain secondary to spinal stenosis: a randomized, equivalence controlled trial.

BACKGROUND: Lumbar surgery and epidural injections for spinal stenosis are the most commonly performed interventions in the United States. However, there is only moderate evidence to the effectiveness of surgery and caudal epidural injections. The next sequential step is adhesiolysis and hypertonic neurolysis with targeted delivery. There have not been any randomized trials evaluating the effectiveness of percutaneous adhesiolysis and targeted delivery of local anesthetic, steroid and hypertonic sodium chloride solution in lumbar spinal stenosis. STUDY DESIGN: A randomized, equivalence, contr

The involvement of cyclic adenosine monophosphate in the control of schistosome miracidium cilia.

This study examined the possible involvement of cyclic adenosine monophosphate (cAMP) in the control of ciliary action of Schistosoma mansoni miracidia. Miracidia immobilized in hypertonic NaCl solution were treated with 3 compounds that are known to increase intracellular cAMP concentrations. Forskolin, at a concentration of 50 microM, induced 50.1% of the miracidia to swim in hypertonic solution. The corresponding values obtained for 3-isobutyl-1-methylxanthine (IBMX) at 1 mM and 8-bromo-cAMP at 10 mM were 42.2 and 50.4%, respectively. The motility-enhancing effect of these compounds was dos

Symposium on nutritional requirements of the surgical patient. 2. Peripheral parenteral nutrition.

During total parenteral nutrition hypertonic (25%) dextrose combined with 5% amino acid solutions must be used to achieve the necessary non-protein calorie/nitrogen ratio of 150:1. The resultant hyperosmolarity prohibits utilization of peripheral veins and makes cannulation of the subclavian vein mandatory. This exposes the patient to the risks of infection and technical complications, but these are uncommon and less important than the deleterious effects of chronic starvation. However, under certain clinical conditions it is possible to supply partial parenteral nutrition through peripheral v

Hyperosmolar therapies for neurological deterioration in mild and moderate traumatic brain injury: towards new research.

The scoping review by Nicolò Marchesini and colleagues about the use of hyperosmolar therapies (HTs) in patients with traumatic brain injury (TBI) points out a significant gap in scientific literature regarding this topic. Although there are few high-quality recommendations, it is important to provide care under certain physiologic parameters. Through this letter we comment on the importance of guidelines to administer and monitor the use of HTs in the Neuro-ICU.

Overcorrection and undercorrection with fixed dosing of bolus hypertonic saline for symptomatic hyponatremia.

OBJECTIVE: Current guidelines recommend treating symptomatic hyponatremia with rapid bolus-wise infusion of fixed volumes of hypertonic saline regardless of body weight. We hypothesize that this approach is associated with overcorrection and undercorrection in patients with low and high body weight. DESIGN: Single-center, retrospective cohort study. METHODS: Data were collected on patients treated with ≥1 bolus 100 or 150 mL 3% NaCl for symptomatic hyponatremia between 2017 and 2021. Outcomes were overcorrection (plasma sodium rise > 10 mmol/L/24 h, > 18 mmol/L/48 h, or relowering therapy) a

[A young woman who did not awake after extensive surgery].

BACKGROUND: Cystic echinococcosis (CE) caused by Echinococcus granulosus, significantly impacts health globally, but is a rare disease in Norway. CE is treated with a combination of anthelmintics and surgery, or percutaneous drainage. CASE PRESENTATION: A woman in her thirties underwent extensive surgery due to disseminated CE in the abdominal cavity and liver. Due to intraoperative cyst rupture with contamination of the abdominal cavity, peritoneal lavage with hypertonic saline (20 % NaCl), a scolicidal agent, was performed for ten minutes before irrigation with physiological saline. Cerebral salt wasting versus SIADH: what difference?

The term cerebral salt wasting (CSW) was introduced before the syndrome of inappropriate antidiuretic hormone secretion was described in 1957. Subsequently, CSW virtually vanished, only to reappear a quarter century later in the neurosurgical literature. A valid diagnosis of CSW requires evidence of inappropriate urinary salt losses and reduced "effective arterial blood volume." With no gold standard, the reported measures of volume depletion do not stand scrutiny. We cannot tell the difference between CSW and the syndrome of inappropriate antidiuretic hormone secretion. Furthermore, the disti

A blood culture study comparing the new Bactec high-volume resin media with hypertonic media.

A blood culture study comparing the performance of the new high-volume Bactec resin media to hypertonic medium was conducted in adult patients seen in the emergency department. Twenty milliliters of blood was divided equally among three media: hypertonic (NR8A), aerobic high-volume resin (HV26), and anaerobic high-volume resin (HV27). During a 5.5-month period, 2,724 blood cultures were collected; there were 417 (15.3%) positive cultures, of which 233 (8.6%) yielded pathogens and 184 (6.8%) contaminants. For the 257 isolates recovered from 233 cultures yielding pathogens, the isolation rates w

[Diagnosis and therapy of hyponatremic syndromes].

Hyponatremia is the most common abnormality in electrolyte and water metabolism. In adult patients it is related to high morbidity and mortality. The degree of CNS-damage depends 1st on the absolute serum sodium concentration (NaS) and 2nd on the rapidity with which NaS is lowered. The most frequent etiology of hyponatremia in pediatric patients is dilutional hyponatremia (SIADH, infusion-therapy). Nephrotic syndromes and congestive heart insufficiency associated with cardiac low output are further causes. Being aware of the different pathophysiological mechanism prevention of hyponatremia is
