Lack of CD27-CD45RA-V gamma 9V delta 2+ T cell effectors in immunocompromised hosts and during active pulmonary tuberculosis.

In humans, the circulating pool of mycobacteria-reactive Vgamma9Vdelta2+ T cells is expanded with age and may contribute to Mycobacterium tu … icate that monitoring and boosting gammadelta T cell effectors could be clinically relevant both in immunocompromised hosts and during active tuberculosis disease.

Osteoarticular Infection by Mycobacterium bovis Following Intravesical Bacillus Calmette-Guérin (BCG) Therapy: A Rare Case of Septic Arthritis and Osteomyelitis in an Immunocompromised Host.

Osteoarticular infections caused by Mycobacterium bovis are rare complications associated with intravesical Bacillus Calmette-Guérin (BCG) therapy and often cause subtle, nonspecific symptoms, which can delay the diagnosis and treatment.  We present the case of a 72-year-old male with a history of urothelial carcinoma treated with intravesical BCG therapy administered four years earlier, who developed persistent fever and progressive worsening polyarthralgia with a two-month duration prior to hospital admission. Imaging presented extensive joint and soft tissue inflammatory involvement. Early

Morganella Morganii Respiratory Infection as an Initial Presentation of Acute Myeloid Leukemia With Monocytic Differentiation: A Rare Opportunistic Pathogen in a Newly Diagnosed Immunocompromised Host.

Infective manifestations of hematological malignancies can be traced to a variety of pathogens, necessitating broad spectrum empiric antibiotic coverage, and prompt identification of pathogenic microbes to improve clinical outcomes. We present a case of Morganella morganii leading to pneumonia as the presenting illness in a previously undiagnosed, functionally neutropenic adult presenting with acute myeloid leukemia with monocytic differentiation. Our patient was a 64 year old man with no prior medical care who presented with complaints of progressive abdominal pain, productive cough and fever

Hepatitis C virus antigen detection is an appropriate test for screening and early diagnosis of hepatitis C virus infection in at-risk populations and immunocompromised hosts.

Early diagnosis remains key for effective prevention and treatment. Unfortunately, current screening with anti-hepatitis C virus antibody (anti-HCV Ab) test may have limited utility in the diagnosis of HCV infection and reinfection. This is of special concern to at-risk population, such as immunocompromised hosts and end-stage renal failure patients on hemodialysis. HCV antigen (Ag) could be useful in identifying the ongoing infection in such clinical scenarios. Hence, we aimed to study the utility of HCV Ag testing for the diagnosis of acute and chronic hepatitis C. Of 89 samples studied, 19 …

Development and Validation of a Machine Learning-Based Bedside Score (IPA-2T2C2DW) for Invasive Pulmonary Aspergillosis in Critically Ill Non-Neutropenic Immunocompromised Hosts.

OBJECTIVE: To develop and validate machine learning-based diagnostic models for IPA using data available within 24 hours of ICU admission, construct three model versions for different healthcare settings, and derive a bedside score for rapid risk stratification. METHODS: This prospective multi-center cohort study enrolled immunocompromised patients with pneumonia complicated by respiratory failure admitted to the ICU at China-Japan Friendship Hospital (formed the derivation cohort) and 15 additional centers nationwide (comprised the external validation cohort). RESULTS: The derivation cohort

Is early combination therapy associated with lower risk of respiratory failure in severely immunocompromised hosts with COVID-19? a target trial emulation study from the omicron-dominant era.

INTRODUCTION: COVID-19 is still a potentially serious infection in immunocompromised individuals, with a significant risk of respiratory failure; nevertheless, the optimal treatment strategy is uncertain. Herein, the impact of combination therapy versus monotherapy on risk of respiratory failure was evaluated. METHODS: This study is a pragmatic target trial emulation performed on observational data collected between 01/01/2022 and 31/03/2024 in 4 tertiary-care hospitals. PATIENTS: immunocompromised patients with mild/moderate COVID-19. INTERVENTION: (arm A) monotherapy with antivirals (DAAs

First Case of Trichoderma longibrachiatum CIED (Cardiac Implantable Electronic Device)-Associated Endocarditis in a Non-immunocompromised Host: Biofilm Removal and Diagnostic Problems in the Light of the Current Literature.

BACKGROUND: Trichoderma species are saprophytic filamentous fungi producing localized and invasive infections that are cause of morbidity and mortality, especially in immunocompromised patients, causing up to 53% mortality. Non-immunocompromised patients, undergoing continuous ambulatory peritoneal dialysis, are other targets of this fungus. Current molecular diagnostic tools, based on the barcode marker ITS, fail to discriminate these fungi at the species level, further increasing the difficulty associated with these infections and their generally poor prognosis. CASE REPORT: We report on th

Management of infections caused by Gram-negative bacteria in severely immunocompromised hosts employing Outpatient Parenteral Antimicrobial Therapy (OPAT): Preliminary evidence from a real-world observational study.

INTRODUCTION: Infections caused by Gram-negative bacteria pose a significant challenge in immunocompromised patients. Outpatient Parenteral Antimicrobial Therapy (OPAT) has emerged as a promising alternative to inpatient care, allowing patients to receive intravenous antimicrobial therapy at home while potentially improving their quality of life. However, data on the efficacy and safety of OPAT in immunocompromised patients remain limited. This study aims to evaluate the effectiveness, safety, and clinical outcomes of OPAT in this vulnerable population. METHODS: We conducted a retrospective o

Evolution of central nervous system multidrug-resistant Mycobacterium tuberculosis and late relapse of cryptic prosthetic hip joint tuberculosis: complications during treatment of disseminated isoniazid-resistant tuberculosis in an immunocompromised host.

We report a case of disseminated isoniazid-resistant tuberculosis in an immunocompromised patient with evolution of rifampin (rifampicin) resistance in the central nervous system. This was cured with intraventricular and oral treatment but was followed by a late relapse of the original infection in a prosthetic hip joint. We provide drug levels in cerebrospinal fluid and serum.

Comparing the profile of respiratory fungal pathogens amongst immunocompetent and immunocompromised hosts, their susceptibility pattern and correlation of various opportunistic respiratory fungal infections and their progression in relation to the CD4+T-cell counts.

INTRODUCTION: Invasive fungal infections are increasingly common in the nosocomial setting. MATERIALS AND METHODS: The patients were divided into two groups immunocompetent and immunocompromised that is, patients with significant neutropenia <500 neutrophils/μl for longer than 10 days. microscopy, culture, identification of isolates were done and some specilised tests on serum and BAL for antigen detection were performed. RESULTS: Majority of the patients were young adult males in this study. A higher prevalence of 26.7% was seen in immunocompromised patients. Amongst yeasts, Candida albican

Successful management of persistent COVID-19 using combination antiviral therapy (nirmatrelvir/ritonavir and remdesivir) and intravenous immunoglobulin transfusion in an immunocompromised host who had received CD20 depleting therapy for follicular lymphoma.

The management of persistent symptomatic coronavirus disease 2019 (COVID-19) infections in immunocompromised patients remains unclear. Here, we present the first case of successful antiviral therapy (nirmatrelvir/ritonavir and remdesivir) in combination with intravenous immunoglobulin (IVIg) in a patient who had received CD20 depleting therapy for follicular lymphoma and experienced recurrent COVID-19 relapses. After the patient received IVIg treatment, the viral load decreased without recurrence. Subsequently, it was found that the anti-spike antibody titer in the administered immunoglobulin

When to Consider the Possibility of a Fungal Infection: An Overview of Clinical Diagnosis and Laboratory Approaches.

Fungal infections are an important and increasingly prevalent cause of disease in certain patient populations. These infections can occur both in immune-compromised and immune-competent individuals. Because the number of patients who are immunocompromised is steadily growing, it is vital for clinicians to consider fungal disease in the differential diagnosis of these patients. This article reviews the epidemiology and approach to diagnosis of a variety of fungal infections.
