Targeted Therapy for Prostate Cancer by Prostate-Specific Membrane Antigen-Targeted Small-Molecule Drug Conjugates.

In the aging global population, prostate cancer is a worldwide health problem because the incidence rate of this disease increases at advanced ages. Although early-stage prostate cancer can be treated by total prostatectomy, the surgery causes side effects, such as incontinence and dysuria, that lower QOL. Once the disease progresses to metastatic castration-resistant prostate cancer (mCRPC), there are no effective chemotherapeutic agents without systematic side effects. Therefore, targeted therapies for mCPRC are urgently needed. Traditional antibody-drug conjugate treatments for prostate can

Practical clinical management of ocular adverse events related to Antibody-Drug Conjugates in gynaecological malignancies.

BACKGROUND: The advent of Antibody-Drug Conjugates (ADC) represents a significant advancement in targeted therapy for gynaecological malignancies. However, the ocular toxicities associated with ADCs, particularly Tisotumab Vedotin (TV) and Mirvetuximab Soravtansine (MIRV) necessitate effective mitigation in order to optimise patient care. METHODS: This review synthesises findings from clinical trials to delineate the spectrum of ocular adverse events induced by ADCs. The analysis focuses on the incidence, onset, severity and reversibility of adverse events. It examines the underlying mechanis

Minimizing Cross-Reactivity for the Chemiluminescent Lateral Flow Immunoassay of Cardiac Troponin I Based on PEGylation of Gold Nanoparticles.

Lateral flow immunoassays (LFIAs) have been employed extensively for the rapid, accurate, and portable detection of protein biomarkers in healthcare. However, the cross-reactivity, especially in the multiplexed detection, leads to false-positive errors that would further limit their practical applications. In this work, we report a highly sensitive and accurate chemiluminescent LFIA based on the synthesis of the Au nanoparticle-antibody-horseradish peroxidase-polyethylene glycol conjugate for detecting cardiac troponin I (cTnI), a major biomarker of acute myocardial infarction. Due to the pres

Anti-CD79B Antibody-Drug Conjugate DCDS0780A in Patients with B-Cell Non-Hodgkin Lymphoma: Phase 1 Dose-Escalation Study.

PURPOSE: Targeting CD79B using antibody-drug conjugates (ADC) is an effective therapeutic strategy in B-cell non-Hodgkin lymphoma (B-NHL). We investigated DCDS0780A, an anti-CD79B ADC with THIOMAB technology (TDC) that consistently conjugates two anti-neoplastic molecules per antibody, in contrast with ADCs with heterogeneous loads. PATIENTS AND METHODS: This phase 1 study enrolled 60 patients with histologically confirmed B-NHL that had relapsed/failed to respond following ≥1 prior treatment regimens; 41 (68%) had diffuse large B-cell lymphoma (DLBCL). Fifty-one patients received DCDS0780A m

Strategies for persistent retention of macromolecules and nanoparticles in the blood circulation.

The enhanced permeability and retention (EPR) effect has been the gold standard in developing drug delivery systems for passive tumor targeting. Although the importance of this concept remains unchanged, some controversies have arisen. In this review, various strategies for tumor targeting using macromolecules and nanoparticles based on the EPR effect are discussed from the viewpoint of pharmacokinetics. Overall, such strategies seek to retain therapeutic material in the blood circulation, which is a key factor for successful targeting. Strategies using macromolecules, including antibody-drug

Antibody-Mediated Enzyme Therapeutics and Applications in Glycogen Storage Diseases.

The use of antibodies as targeting molecules or cell-penetrating tools has emerged at the forefront of pharmaceutical research. Antibody-directed therapies in the form of antibody-drug conjugates, immune modulators, and antibody-directed enzyme prodrugs have been most extensively utilized as hematological, rheumatological, and oncological therapies, but recent developments are identifying additional applications of antibody-mediated delivery systems. A novel application of this technology is for the treatment of glycogen storage disorders (GSDs) via an antibody-enzyme fusion (AEF) platform to

Optimizing the role of brentuximab vedotin in classical Hodgkin lymphoma therapy.

The US Food and Drug Administration approval of brentuximab vedotin (BV) in 2011 marked an important milestone in the management of classical Hodgkin lymphoma (HL). Although initially approved for use in the relapsed or refractory setting, its high efficacy and favorable toxicity profile led to numerous studies evaluating BV in the front-line, second-line, and posttransplant settings. BV is now approved for use (in combination with chemotherapy) as frontline treatment of advanced-stage patients and as maintenance therapy following autologous stem cell transplant. Additional studies demonstrate

Antibody radiolabeling techniques to optimize cellular retention.

Radiolabeling of antibodies and antibody fragments facilitates the development of new targeted therapeutics or tracking and validation of biosimilars. The typical metal ion chelators that can be used for radiolabeling reactions have residualizing properties in tissues/tumors. A team at Genentech has developed an elegant new technique for combining iodine radiolabeling with an azamacrocyclic chelator to confer residualizing properties on the radioiodine metabolites. Robust protocols, such as this example, are required for the future development of protein based drugs.

Current and experimental antibody-based therapeutics: insights, breakthroughs, setbacks and future directions.

The premise of targeted therapy was born from an intimate understanding of the unique biological pathways and endpoints which are implicated in the development of different disease states and conditions. In addition, the identification of the most appropriate drugs to use for targeted drug therapy has aided in growing interest of the pharmaceutical industry to allocate more resources to monoclonal antibody (mAb) therapeutics. This being the case, it is important to understand antibody based therapeutics, some of the currently Food and Drug Administration (FDA)-approved mAbs in different diseas

Mesangial immunoglobulin (Ig)A glomerulonephritis in a patient with rheumatoid arthritis treated with abatacept.

We report observations of a 47-year-old seropositive woman with rheumatoid arthritis (RA) suffering from mesangial immunoglobulin (Ig)A glomerulonephritis (GN) after initiation of abatacept, a selective T-cell co-stimulation modulator cytotoxic T-lymphocyte antigen 4 (CTLA4)-Ig. She was initially treated by corticosteroids, followed by methotrexate associated with a TNF inhibitor (adalimumab then switched to etanercept), finally switched to abatacept monotherapy, after secondary failure of these two forms of TNF inhibitors. Due to a progressively increased hematuria and proteinuria after abata

Belatacept.

Immunosuppressive therapy designed to prevent kidney graft rejection usually consists of a triple-drug combination including a corticosteroid, a calcineurin inhibitor (ciclosporin or tacrolimus) and a drug that inhibits cell proliferation (azathioprine or mycophenolate mofetil). Belatacept is closely related to abatacept, an immunosuppressant marketed for rheumatic diseases. It is now authorised in the European Union for the prevention of kidney graft rejection, as a replacement for the calcineurin inhibitor. Two randomised controlled trials compared belatacept (2 doses) versus ciclosporin as

Antibody-drug conjugates and tumor immunity in breast cancer.

Antibody-drug conjugates (ADCs) combine antigen-directed delivery with cytotoxic or immunostimulatory payloads. Their antitumor activity can involve direct tumor-cell killing, but the extent to which conventional ADCs remodel human breast-cancer immunity remains incompletely defined. This review critically separates established cytotoxic mechanisms from immune-mediated effects and incorporates emerging STING agonist ADCs, neonatal Fc receptor (FcRn) trafficking, and quantitative antigen measurement. A focused narrative synthesis of mechanistic, translational, and clinical literature was perfor
