The relation between mindfulness and fear of negative evaluation over the course of cognitive behavioral therapy for social anxiety disorder.

OBJECTIVES: This study examined the relation between mindfulness and fear of negative evaluation over the course of nonmindfulness based cognitive-behavioral therapy (CBT) for social anxiety disorder (SAD). We expected that higher levels of mindfulness would be associated with a more positive response to treatment. METHOD: This study is a secondary report from a randomized controlled trial in which participants (N = 65) diagnosed with SAD were randomly assigned to receive 8 weeks of 1 of 2 manualized treatments (exposure group therapy, n = 33; or virtual reality exposure therapy, n = 32) eith

Anxiety-control strategies: is there room for neutralization in successful exposure treatment?

Cognitive-behavioral theory suggests that anxiety-control strategies such as neutralization, distraction and various forms of safety behavior have the potential to diminish the effectiveness of and/or interfere with exposure treatment. Yet, it is common practice when treating individuals with anxiety disorders to employ various anxiety-control strategies as a means of assisting clients/patients with difficult exposure situations. Questions surrounding the issue of which anxiety-control strategies help vs. hinder exposure-based treatments (and under which circumstances) have been a topic of muc

Treatment of acrophobia in virtual reality: the role of immersion and presence.

In this study the effects of virtual reality exposure therapy (VRET) were investigated in patients with acrophobia. Feelings of presence in VRET were systematically varied by using either a head-mounted display (HMD) (low presence) or a computer automatic virtual environment (CAVE) (high presence). VRET in general was found to be more effective than no treatment. No differences were found in effectiveness between VRET using an HMD or CAVE. Results were maintained at 6 months follow-up. Results of VRET were comparable with those of exposure in vivo (Cyberpsychology and Behavior 4 (2001) 335). I

Severe depersonalization treated by behavior therapy.

The author reports the use of behavior therapy in the treatment of two cases of severe intractable depersonalization. Outcome criteria included full psychiatric assessment, patients' self-ratings, and psychometric test scores. Treatment by flooding was highly effective in one case, and associated obsessive symptoms and anticipatory anxiety were substantially decreased in the other. Behavioral techniques may prove especially helpful in cases in which anticipatory anxiety, phobic avoidance, and obsessive perseveration are exacerbating features.

A Randomized Controlled Trial of Self-Guided Digital Written Imaginal Exposure Therapy for Eating Disorders in Patients Recently Discharged From Higher Level Specialty Eating Disorder Care.

OBJECTIVE: Imaginal exposure therapy facilitates the approach of feared stimuli (e.g., fear of weight gain) not accessible in everyday life (e.g., rapidly gaining weight) and has initial promising data for the treatment of eating disorders (EDs). METHODS: In the current randomized controlled trial (RCT) participants (N = 130) were randomly assigned to a five-session digital imaginal exposure condition or a control online journaling condition within 4 months of discharge from intensive (e.g., inpatient) treatment. We examined the feasibility and acceptability of the treatment conditions, targe

Sweetening fear exposure: study protocol for a multi-day, randomized controlled trial to study effects of glucose in exposure sessions of participants with public speaking anxiety.

BACKGROUND: Public speaking anxiety (PSA) is present in most individuals with social anxiety disorder (SAD). As for SAD, exposure therapy is considered the gold-standard treatment for PSA. However, it is known that not all patients benefit from it. Recent research (Hauck Behav Res Ther: 104553, 2024) suggests that glucose facilitates fear extinction and consolidation, processes crucial for psychotherapeutic success, demonstrating its potential as an adjuvant to improve the efficacy of exposure therapy. The present study aims to translate previous findings into a clinical context, by studying t

Pretreatment anhedonia as a predictor of exposure-based anxiety treatment outcomes.

Reward processes can reinforce extinction learning and aide cognitive processes involved with inhibitory learning. Anhedonia, characterized by deficits in reward incentivization, hedonic response, and learning, may therefore predict worse treatment outcomes. The current study examined associations between pretreatment self-reported anhedonia and several measures of anxiety symptom severity and treatment outcomes in a naturalistic exposure-based treatment setting. We used multilevel models to examine the relationship between pretreatment anhedonia with change in general treatment response and d

Acceptability and Preliminary Effects of Intensive Brief Trauma-Focused PTSD Treatment for Refugees.

BACKGROUND: Post-Traumatic Stress Disorder (PTSD) is a significant mental health concern in refugee populations exposed to trauma and displacement. Traditional treatments for PTSD often involve lengthy interventions. However, there's a growing interest in exploring more condensed, intensive treatments to improve outcomes and accessibility for refugees. OBJECTIVE: This study aimed to evaluate the acceptability, and preliminary effects of an intensive brief trauma-focused PTSD treatment (ITT) program delivered to refugees at the Swedish Red Cross Treatment Center for Persons Affected by War and

An investigation of the role of estradiol in fear reduction during a single session of exposure therapy.

Research suggests that estradiol may moderate fear extinction. It is unclear whether these results generalize to exposure therapy. The aim of the current study was to determine whether estradiol moderates outcomes in exposure therapy among women with anxiety disorders. Participants were 35 women with a primary diagnosis of an anxiety disorder who participated in the study as part of routine care at an anxiety specialty clinic. Endogenous estradiol was assessed via saliva. They provided subjective distress ratings before (pre) and after (post) an exposure session, as well as after a brief delay

The efficacy of 90-min versus 60-min sessions of prolonged exposure for PTSD: A randomized controlled trial in active-duty military personnel.

OBJECTIVE: Prolonged exposure (PE) therapy is a first-line posttraumatic stress disorder (PTSD) treatment, but the manualized 90-min session format constitutes a barrier to adopting PE in most settings because they use 60-min sessions for scheduling and billing. We examined whether 60-min PE sessions were as effective and efficient as 90-min PE sessions. METHOD: In total, 160 active-duty military personnel with PTSD were randomized to 8-15 sessions of 60- or 90-min PE sessions and assessed pre- and posttreatment, and 3- and 6-month posttreatment, using the Clinician Administered PTSD Scale fo

Genome-wide expression and response to exposure-based psychological therapy for anxiety disorders.

Exposure-based psychological treatments for anxiety have high efficacy. However, a substantial proportion of patients do not respond to therapy. Research examining the potential biological underpinnings of therapy response is still in its infancy, and most studies have focussed on candidate genes. To our knowledge, this study represents the first investigation of genome-wide expression profiles with respect to treatment outcome. Participants (n=102) with panic disorder or specific phobia received exposure-based cognitive behavioural therapy. Treatment outcome was defined as percentage reductio

Affect labeling enhances exposure effectiveness for public speaking anxiety.

Exposure is an effective treatment for anxiety but many patients do not respond fully. Affect labeling (labeling emotional experience) attenuates emotional responding. The current project examined whether affect labeling enhances exposure effectiveness in participants with public speaking anxiety. Participants were randomized to exposure with or without affect labeling. Physiological arousal and self-reported fear were assessed before and after exposure and compared between groups. Consistent with hypotheses, participants assigned to Affect Labeling, especially those who used more labels durin

Bottom-up and top-down influences of beliefs on emotional responses: fear of heights in a virtual environment.

According to cognitive approaches in emotion research, emotions hinge on beliefs that can be true or false. We suggest that emotionally relevant beliefs can be influenced bottom-up e.g. by the depth cues of a virtual environment or top-down e.g. by reappraisal strategies. Our research question is if bottom-up and top-down processes influence the same belief structure or if different belief structures are responsible for bottom-up and top-down influences on emotions. To test these assumptions we exposed participants to a virtual environment that is able to elicit fear of heights and manipulated

Empirically supported psychological interventions for social phobia in adults: a qualitative review of randomized controlled trials.

BACKGROUND: Social phobia is a chronic disorder that results in substantial impairment. We conducted a qualitative review of randomized controlled trials (RCTs) of psychological interventions for social phobia. METHOD: Articles were identified through searches of electronic databases and manual searches of reference lists. They were classified by psychological interventions evaluated. Data regarding treatment, participants and results were then extracted and tabulated. We identified which psychological interventions are empirically supported, using the scheme proposed by Chambless & Hollon (J

[Treating specific childhood phobia in a single session? A systematic review of the literature].

OBJECTIVE: The essential feature of specific phobia is a persistent fear of clearly discernable, circumscribed objects or situations. The DSM-IV distinguishes four subtypes: animal, natural environment, blood-injection-injury, and situational. Specific phobias frequently co-occur. Specific phobia is one of the most common psychiatric disorders with a lifetime prevalence of 12.5% and is about twice as common in women as in men. Most phobias have a childhood onset except for the situational subtype which usually occurs during patients' twenties. It is well known that childhood anxiety increases

[Individual virtual reality trigger exposure as an add-on in the treatment of komplex PTSD - A case study].

We present the case of a patient who, as a result of multiple traumatic experiences during his military deployments and associated moral injuries, suffered from complex post-traumatic stress disorder, alienation, increased aggression potential and, primarily, intense feelings of guilt. He participated in a feasibility and acceptance study on individualised virtual reality trigger exposure in order to work with these feelings of guilt. The advantages and disadvantages of this adjuvant therapeutic approach are subsequently discussed.

Study adaptation, design, and methods of a web-based PTSD intervention for women Veterans.

Women Veterans are a rapidly growing population with high risk of exposure to potentially traumatizing events and PTSD diagnoses. Despite the dissemination of evidence-based treatments for PTSD in the VA, most women Veteran VA users underutilize these treatments. Web-based PTSD treatment has the potential to reach and engage women Veterans with PTSD who do not receive treatment in VA settings. Our objective is to modify and evaluate Delivery of Self Training and Education for Stressful Situations (DESTRESSS), a web-based cognitive-behavioral intervention for PTSD, to target PTSD symptoms among

Complex regional pain syndrome: An optimistic perspective.

Complex regional pain syndrome (CRPS) presents with clinical symptoms that can no longer be explained by the initial trauma, including pain, sensory, motor, and trophic symptoms, and impairment of autonomic control of the limb. These symptoms spread distally and go beyond single nerve innervation territories. Typically, the symptoms change through the course of CRPS as a result of the varying pathophysiology. Diagnosis is made clinically after the rigorous elimination of other possible causes, and 3-phase bone scintigraphy can be a useful tool for confirming CRPS. In acute stages, inflammatory

A Randomized Clinical Trial of Prolonged Exposure Therapy With and Without Topiramate for Comorbid PTSD and Alcohol Use Disorder.

OBJECTIVE: Posttraumatic stress disorder (PTSD) and alcohol use disorder (AUD) frequently co-occur. Prolonged exposure (PE) is an effective treatment for PTSD but shows smaller effects in patients with co-occurring AUD. Topiramate may help reduce alcohol use and PTSD symptoms. This double-blind, placebo-controlled outpatient clinical trial compared 12 sessions of PE plus either topiramate or placebo. METHODS: One hundred U.S. veterans (mean age=45 years [SD=12], 84% men) with PTSD+AUD were randomly assigned to 16 weeks of treatment with PE+topiramate (up to 250 mg) or PE+placebo to examine ef
