Role of ultrasound in the assessment of male infertility.

The use of ultrasonography has become an important component in the evaluation and treatment of male reproductive tract disorders. From the use of color flow Doppler ultrasonography for the assessment of varicoceles to transrectal ultrasonography combined with seminal vesiculography for the evaluation of ejaculatory duct obstruction, ultrasonography has practical clinical applications. In this article, the authors review the indications and use of ultrasonography in the assessment and treatment of the infertile male. The recent advances in diagnostic transrectal ultrasonography for ejaculatory

Cytogenetic study in idiopathic infertile males.

Sex chromatin investigations, including X-chromatin and Y-chromatin, was carried out on one hundred idiopathic infertile males with marked oligospermia or azoospermia. Seven cases (7%) were X-chromatin positive, 18% of the cases had aberrant Y-body (10% big Y, 7% small Y and 1% double Y). Such Y-chromosome abnormalities were frequent among azoospermic than oligospermic males. Chromosomal analysis of patients with positive X-chromatin and/or abnormalities of the Y-chromosome showed chromosomal abnormalities in the seven azoospermic cases (7%) which were X-chromatin positive. These chromosomal a

[The treatment of male infertility with the preparation vitaton].

Thirty men with fertility disorders, 21 to 42 years of age (mean 29 years) and mean duration of sterile matrimony 4 years 7 months received Vitaton treatment. The patients were divided in three groups: 8 with cryptorchidism, 12 having undergone resection of v. testicularis interna sin for varicocele and 10 with idiopathic oligospermia. In the first group of patients with bilateral cryptorchidism and azoospermia the control spermatograms showed persistence of azoospermia, i.e. in none of these patients was fertility reestablished. The second group of patients who had been operated for idiopathi

The testicular biopsy in the evaluation of male infertility.

A testicular biopsy is often useful in male infertility to evaluate the degree of damage and the patient's prognosis. The major issues in the interpretation of the histologic specimens are the adoption of a simple reproducible classification of testicular abnormalities, and the recognition of specific causes of damage. This review is concerned with the clinical scope of testicular biopsy for infertility, the technical handling of the specimen, and a practical stepwise approach to the histologic diagnosis.

Genetic Testing for Monogenic Forms of Male Infertility Contributes to the Clinical Diagnosis of Men with Severe Idiopathic Male Infertility.

PURPOSE: In recent years, many genes have been associated with male infertility; however, testing of monogenic forms has not yet been clinically implemented in the diagnosis of severe forms of idiopathic male infertility, as the diagnostic utility has not been established yet. The aim of this study was therefore to answer if the implementation of genetic testing for monogenic forms of male infertility could contribute to the clinical diagnosis of men with severe forms of idiopathic male infertility. MATERIALS AND METHODS: Based on the ClinGene curation protocol, we defined a panel of genes wi

Genetic aspects of human male infertility: the frequency of chromosomal abnormalities and Y chromosome microdeletions in severe male factor infertility.

OBJECTIVE: The main purpose of this study is to detect the frequency and type of both chromosomal abnormalities and Y chromosome microdeletions in patients with severe male factor infertility and fertile control subjects. The association between the genetic abnormality and clinical parameters was also evaluated. METHODS: This study was carried out in 208 infertile and 20 fertile men. Results of 208 patients, 119 had non-obstructive azoospermia and 89 had severe oligoasthenoteratozoospermia (OAT). Seventeen out of 119 (14.3%) azoospermic patients and two out of 89 (2.2%) patients with OAT had

Unfolding the Semen Microbiota: Implications for Male Infertility.

In recent years, the growing interest in microbiota, supported by increasing evidence from next-generation sequencing (NGS)-based studies, has led to the hypothesis that multifactorial conditions such as male infertility may have their bases in its imbalance. Despite rapid advances in the field over the past decade, findings remain fragmented and sometimes inconsistent. Furthermore, analyses frequently exclude non-bacterial components, such as viruses (e.g., human papillomavirus and herpes simplex virus), protozoa (e.g., Trichomonas vaginalis) and fungi (e.g., Candida spp.), with a few studies

State Laws and Insurance Coverage for Male Infertility.

OBJECTIVE: To assess coverage of male infertility in state laws. Infertility is experienced by 12% of couples, unfortunately, however, infertility treatments are expensive and infrequently covered by insurance. Furthermore, many states lack explicit laws on fertility coverage. METHODS: The National Conference of State Legislatures and individual state legislature websites were used to assess fertility coverage. These were queried for health insurance mandates or fertility coverage by evaluating healthcare-related legislation. RESULTS: Less than half (21/50) of states require coverage. Among

Novel INSL3 variants cause male infertility with cryptorchidism.

PURPOSE: Cryptorchidism is one of the most prevalent male congenital abnormalities, affecting 1.6%-9% of newborn males, and it poses substantial risks to male fertility. INSL3 and its receptor RXFP2 play a major role in the first phase of the biphasic testicular descent process. The genetic etiology of cryptorchidism has long remained controversial, and the association between INSL3 gene mutations and cryptorchidism still requires robust evidence to substantiate. This study aims to clarify that the novel homozygous frameshift variants of INSL3 are the genetic cause of cryptorchidism in patient

How much does mitochondrial dysfunction affect male infertility?

Infertility is usually defined as the inability of a couple to conceive even after 1 year of unprotected, frequent sexual intercourse. The male is solely responsible in about 20% of cases. Several risk factors as well as urogenital abnormalities, endocrinological disease, enviromental toxines, genetic disorders, immunological factors, urogenitaly infections, sexual dysfunction  have been identified as causes of male infertility. Nowadays different studies reported mithocoldrial dysfunction as cause of male infertility and alteration of semen parameters.

Insight toward inflammasome complex contribution to male infertility.

During the last decades, a wide range of factors involved in the physiopathology of male infertility disease have been discussed. The inflammation role in some of the main infertility-related diseases has been studied, such as varicocele, spinal cord injury and obesity. Inflammation is the main response of the immune system to infection or cell damage, leading to intense inflammatory cytokine release during the loss of homeostasis. One of the first steps toward pro-inflammatory cytokines release is the recognition of dangerous signals by the immune cells, including pathogen-associated molecula

Microbiology and immune mechanisms associated with male infertility.

Up to 50% of infertility is caused by the male side. Varicocele, orchitis, prostatitis, oligospermia, asthenospermia, and azoospermia are common causes of impaired male reproductive function and male infertility. In recent years, more and more studies have shown that microorganisms play an increasingly important role in the occurrence of these diseases. This review will discuss the microbiological changes associated with male infertility from the perspective of etiology, and how microorganisms affect the normal function of the male reproductive system through immune mechanisms. Linking male in

[Oxidative stress and male infertility a clinical perspective].

The review of recent literature suggests that oxidative stress plays a significant role in the etiology of male infertility, life style and environmental factors contributing to oxidative stress are common in males. Antioxidant defence system is described as a complex network of internally synthesised and externally supplied components. In men with oxidative stress infertility antioxidants (-carotine, vitamin E, vitamin C, selenium, and zinc) intake decreases reactive oxygen species production, improves sperm parameters and pregnancy rate.
