Ethical Challenges Posed by HIV-Vaccine Induced Seropositivity for the Informed Consent Process and HIV Testing Practices in Maputo City, Mozambique.

Mozambique has a robust HIV control program and currently conducts HIV vaccine trials. One measure of a vaccine's success is development of antibodies, the vaccine-induced seropositivity(VISP). This study assesses the understanding and practices regarding VISP from trial participants and healthcare providers, as well as ethical challenges encountered during the informed consent. We conducted in-depth, semi-structured interviews using open and closed-ended questions with three categories of participants: healthcare professionals [n = 26], researchers [n = 5] and trial participants [n = 20]. For…

Ensuring Care for Transgender Adolescents Who Need It: Response to 'Reconsidering Informed Consent for Trans-Identified Children, Adolescents and Young Adults'.

Adolescent transgender care is increasingly surrounded by controversies and criticism. One of these concerns expressed in a review article by Levine et al. entitled 'Reconsidering Informed Consent for Trans-Identified Children, Adolescents and Young Adults' is the limited evidence base, especially of the Dutch studies which provided the first and mostly cited basis for medical intervention at a young age. This Response is written by the first author of two of those studies that showed effectiveness of the approach that included puberty blockers. The author rebuts several of the concerns that L…

Would you like to participate in this trial? The practice of informed consent in intrapartum research in the last 30 years.

BACKGROUND: Informed consent is the cornerstone of the ethical conduct and protection of the rights and wellbeing of participants in clinical research. Therefore, it is important to identify the most appropriate moments for the participants to be informed and to give consent, so that they are able to make a responsible and autonomous decision. However, the optimal timing of consent in clinical research during the intrapartum period remains controversial, and currently, there is no clear guidance. OBJECTIVE: We aimed to describe practices of informed consent in intrapartum care clinical resear…

Ethical considerations of transparency, informed consent, and nudging in a patient with paediatric aortic stenosis and symptomatic left ventricular endocardial fibroelastosis.

A 9-year-old boy who was born with bicuspid aortic stenosis underwent two unsuccessful aortic valvuloplasty interventions, and by 2 years of age he developed restrictive cardiomyopathy caused by left ventricular endocardial fibroelastosis and diastolic dysfunction. The attending cardiologist referred the patient to a high-volume, high-profile congenital cardiac surgical programme 1000 miles away that has a team with considerable experience with left ventricular endocardial fibroelastosis resection and a reputation of achieving good results. Owing to problems with insurance coverage, the parent

Patient-Reported Perceptions of Surgical Informed Consent Communication in Elective General Surgery: A Cross-Sectional Study in a Chinese Tertiary Hospital.

BACKGROUND: Informed consent supports voluntary surgical decision-making, yet patients' routine perceptions of consent communication remain insufficiently described. In China, where family involvement and time-pressured care are common, understanding how patients retrospectively perceive written and oral consent communication is important. METHODS: We conducted a cross-sectional survey of 627 consecutive adult patients undergoing elective general surgery in a tertiary general hospital in Fuzhou, Fujian Province, China. Participants were interviewed face-to-face on postoperative days 2-7 using…

The role of virtual reality in enhancing informed consent for shockwave lithotripsy: A mixed-methods study of physician and patient perspectives.

OBJECTIVE: The integration of immersive technologies such as virtual reality (VR) in healthcare is gaining momentum, particularly in patient education and the informed consent process. Traditional patient information leaflets often fail to meet the needs of diverse populations due to literacy barriers and a lack of visual engagement. This study aimed to evaluate the impact of a VR platform (Surgassists®) on patient understanding, anxiety, and satisfaction during the consent process for shockwave lithotripsy (SWL), alongside surgeon perspectives on VR's feasibility and utility. METHODS: A mixe…

Analyzing patient perspectives with large language models: a cross-sectional study of sentiment and thematic classification on exception from informed consent.

Large language models (LLMs) can improve text analysis efficiency in healthcare. This study explores the application of LLMs to analyze patient perspectives within the exception from informed consent (EFIC) process, which waives consent in emergency research. Our objective is to assess whether LLMs can analyze patient perspectives in EFIC interviews with performance comparable to human reviewers. We analyzed 102 EFIC community interviews from 9 sites, each with 46 questions, as part of the Pediatric Dose Optimization for Seizures in Emergency Medical Services study. We evaluated 5 LLMs, includ…

A Comparison of the Quality of Informed Consent for Clinical Trials of an Experimental Hookworm Vaccine Conducted in Developed and Developing Countries.

Informed consent is one of the principal ethical requirements of conducting clinical research, regardless of the study setting. Breaches in the quality of the informed consent process are frequently described in reference to clinical trials conducted in developing countries, due to low levels of formal education, a lack of familiarity with biomedical research, and limited access to health services in these countries. However, few studies have directly compared the quality of the informed consent process in developed and developing countries using the same tool and in similar clinical trials. T…

The Quality and Perception of Preoperative Informed-Consent Discussions with Older Patients: A Questionnaire From Both the Patients' and the Physicians' Perspectives.

BACKGROUND: Informed consent requires the provision and comprehension of adequate information, as well as autonomous decision-making by the patient. We studied whether routine preoperative informed-consent discussions fulfill these prerequisites, and to what extent patients and physicians differ in their assessments of the informed-consent process. METHODS: In a single-center observational study, we asked patients aged 65 and above about the information they had been given, their comprehension of it, and their decision-making autonomy, as well as their personal preference for degree of partic…

Knowledge and associated factors of patients toward informed consent in obstetric and gynecologic surgical procedures at Debre Markos Comprehensive Specialized Hospital, Ethiopia.

BACKGROUND: Informed consent (IC) is a principle in medical ethics and medical law that a patient must have adequate information and understanding before making decisions about their medical care. It is very essential for both clinician and patient in surgery and should be seen as a usual activity. Knowledge is one of the main intervention tools to improve patient comprehension in informed consent for surgical patients. In Ethiopia, little is known about the knowledge level and its determinant factors toward obstetric and gynecologic surgical informed consent. Due to this reason, this study ai…
