Feasibility and safety of transfemoral transcatheter aortic valve implantation performed with a percutaneous coronary intervention-like approach.

BACKGROUND: Transfemoral percutaneous transcatheter aortic valve implantation (TF-TAVI) is a safe, reproducible and established procedure, mainly performed under local anaesthesia, which is mostly administered and monitored by a dedicated anaesthesia team (regular approach). Our centre has developed a standardized pathway of care, and eligible patients are selected for a minimalist TF-TAVI, entirely managed by operators without the presence of the anaesthesia team in the operating room, like most interventional coronary procedures ("percutaneous coronary intervention-like" approach [PCI approa

Ferritin screening and Iron treatment for maternal anemia and fetal growth restriction prevention - A multicenter randomized controlled trial (FAIR Study).

BACKGROUND: Non-anemic iron deficiency precedes iron deficiency anaemia and has an estimated prevalence of 1-2 billion worldwide. Few studies have comprehensively researched the idea of treating non-anemic iron deficiency (NAID) with iron to improve the outcome of the mother and the offspring. METHODS AND ANALYSIS: FAIR will be a multicenter randomized controlled trial that will be conducted in multiple clinical academic obstetrics units in Lahore (including Services Institute of Medical Sciences, Lahore, Allama Iqbal Medical College, Lahore and Fatima Jinnah Medical University). Sedation Management in the Intubated Pediatric Patient as a Method to Reduce Neuromuscular Blockade Utilization Rate During Transport: A Quality Improvement Project.

OBJECTIVE: Models recommending continuous sedation combined with specific tools to assess sedation depth during pediatric transport do not exist. Published studies demonstrate that nurse-driven sedation protocols yield more consistent levels of appropriate sedation. METHODS: A retrospective review in 2020 of mechanically ventilated pediatric transport patients at this institution demonstrated that 60.7% received neuromuscular blockade. This higher than anticipated neuromuscular blockade usage indicated an opportunity to improve sedation management. The primary aim of this quality improvement

Team Approach: Hip Preservation Surgery.

The evaluation and treatment of adolescents and young adults with hip pain has seen tremendous growth over the past 20 years. Labral tears are well established as a common cause of hip pain but often occur because of underlying bony abnormalities. Femoroacetabular impingement (FAI) and acetabular dysplasia are now well-established causes of hip osteoarthritis and are increasingly treated in the prearthritic stage in hopes of improving symptoms and prolonging the longevity of the native hip. Beyond FAI and acetabular dysplasia, this patient population can present with a complex and variable gro

Latex allergy. An orthopaedic case presentation and considerations in patient care.

Latex allergy has become a challenging phenomenon in health care delivery for the last several years given the numerous products containing latex. The number of individuals with latex allergy has grown dramatically since the institution of standard precautions issued by the Centers for Disease Control and Prevention (CDC) in 1987 in response to the AIDS epidemic. The allergic reaction to latex ranges from a minor skin rash to anaphylactic shock. Preventing exposure to latex is the key to managing and preventing this allergy. Providing a safe environment for patients with latex allergy is the r

Twenty years of university-graduated osteopaths in Belgium: A cross-sectional survey.

BACKGROUND: In 2004, the Université libre de Bruxelles (ULB, Belgium) launched Europe's first comprehensive university‑based osteopathy program, a six‑year curriculum intended to set an international standard for training osteopaths in the primary care management of functional musculoskeletal pain and disorders. This study describes the socio‑professional profile of ULB‑trained osteopaths in Belgium by graduation cohort. METHODS: All contactable ULB graduates (n = 277; 83.9%) were invited to complete a voluntary, online cross‑sectional survey (49 items) disseminated via cohort‑personalized in

Barriers to primary care physicians prescribing buprenorphine.

PURPOSE: Despite the efficacy of buprenorphine-naloxone for the treatment of opioid use disorders, few physicians in Washington State use this clinical tool. To address the acute need for this service, a Rural Opioid Addiction Management Project trained 120 Washington physicians in 2010-2011 to use buprenorphine. We conducted this study to determine what proportion of those trained physicians began prescribing this treatment and identify barriers to incorporating this approach into outpatient practice. METHODS: We interviewed 92 of 120 physicians (77%), obtaining demographic information, curr

Error Traps in Pediatric Adenotonsillectomy: Clinical Patterns, Cognitive Pitfalls, and Evidence-Informed Mitigation Strategies.

Adenotonsillectomy is one of the most common elective pediatric surgeries. However, its routine occurrence can mask substantial preventable risks arising from practitioner inexperience, underappreciated comorbidities, airway proximity to the surgical field, and challenging postoperative pain management. This review highlights common "error traps" that contribute to perioperative adverse events and outlines evidence-based mitigation strategies. Key preoperative challenges include unrecognized moderate-to-severe obstructive sleep apnea (OSA) and recent upper respiratory infections (URIs), both o

The Acute Perioperative Pain Service: Impact, Organization, and Future Directions.

PURPOSE OF REVIEW: The Acute Perioperative Pain Service has been established as a relatively new but important concept and service in clinical patient care. Many surgical institutions have dedicated inpatient acute pain services with variable compositions. This review aims to investigate the purpose, clinical and economic implications, and future direction of the Acute Perioperative Pain Service (APPS). RECENT FINDINGS: There is growing evidence of the multiple benefits of a dedicated APPS, especially pertaining to patients at higher risk of poorly controlled postoperative pain. Healthcare pr

Perioperative Management of Parturient Women With Severe Pulmonary Arterial Hypertension Secondary to Atrial Septal Defect.

At our institution, we occasionally see pregnant patients in the later stages of pregnancy who present with severe pulmonary arterial hypertension caused by congenital heart disease. The physiological changes in pregnancy may worsen the cardiovascular status leading to heart failure which is associated with a high incidence of morbidity and mortality. A scheduled caesarean section in such patients ensures delivery in a controlled environment, avoiding prolonged labour, which is detrimental. Perioperative complications that may worsen pulmonary arterial hypertension should be prevented. The per

Unforeseen rupture of pseudoaneurysm of common carotid artery: An arduous anesthetic challenge.

Pseudoaneurysm of the common carotid artery (CCA) is exceptionally unstable and unpredictable; it mandates quick medical attention in order to circumvent neurologic sequelae or hemorrhage. Unanticipated rupture is extremely lethal and a potential provocation for the anesthesia caregiver. It is an arduous challenge for an anesthetist to establish emergency airway when a huge bleeding pseudoaneurysm is compressing and deviating the trachea, securing invasive lines in collapsing vessels, volume and vasopressor resuscitation in deteriorating hemodynamics in order to maintain cerebral perfusion wit

Opportunities to improve inpatient services and reduce rates of patient-direct discharge among people who use substances.

PURPOSE: Patients who use substances (PWUS) report experiencing stigmatizing encounters and undertreatment of pain and withdrawal symptoms that increase the likelihood of patient-directed discharge (PDD). This scoping review examines North American literature to gain insights about how institutional factors intersect with patient experiences and contribute to PDD. METHODS: A scoping review was conducted using MEDLINE, CINAHL, Scopus, and EMBASE databases. Screening was completed by two reviewers. A data extraction tool developed by the research team was used to collect demographic information

Urinary tract injuries during treatment of patients with morbidly adherent placenta.

INTRODUCTION: Urinary tract injuries (UTI) are a frequent complication of morbidly adherent placenta (MAP) management. In this study, we aim to characterize the type of UTI that occurs and to define if their incidence varies after establishing a fixed interdisciplinary group for the protocolized management of patients with MAP. METHODOLOGY: All patients with confirmed MAP attended between 2011 and 2019 in our institution, were included. We analyzed the effect of a change in the surgical protocol including rigid ureteral catheters, vesicouterine dissection before hysterotomy and interdisciplin

Management of motor neurone disease.

Motor neurone disease is a progressive neurodegenerative disorder leading to severe disability and death. It is clinically characterised by mixed upper and lower motor neurone involvement affecting bulbar, limb, and respiratory musculature. Recent guidelines have established diagnostic criteria and defined management of the condition. In a proportion of familial amyotrophic lateral sclerosis there is a mutation in the gene encoding the enzyme copper/zinc superoxide dismutase 1; this has allowed mutation screening and generated considerable laboratory based research. The diagnosis must be given

Breakroom Culture and Nurse Burnout: A Multilevel Moderated Mediation Analysis.

AIM: To examine whether perceived organisational support (POS) mediates the association between breakroom culture and nurse burnout (low psychological well-being and disengagement) and whether unit-level teamwork engagement moderates this mediation. BACKGROUND: Nurse burnout threatens care quality and workforce retention globally. Informal social resources embedded in the everyday work environment, such as breakroom culture (shared supportive peer interactions during rest periods), remain underexplored-particularly in nonWestern healthcare contexts. Grounded in the Job Demands-Resources theor
