Unexpected Peritoneal Loose Body Discovered During Laparoscopic Varicocelectomy: A Case Report.

BACKGROUND Peritoneal loose bodies (PLBs) are rare, benign intraperitoneal entities usually discovered incidentally during abdominal surgery or imaging. Their occurrence during urologic laparoscopy appears to be infrequently reported and may create intraoperative diagnostic uncertainty when encountered unexpectedly. CASE REPORT A healthy 22-year-old Jordanian man with no history of abdominal or pelvic surgery underwent bilateral laparoscopic varicocelectomy for symptomatic varicoceles. Dense sigmoid adhesions over the left internal inguinal ring required careful adhesiolysis to expose the sper

M1 segment morphology is associated with far lateral lenticulostriate arteries in middle cerebral artery bifurcation surgery: a cadaveric and clinical anatomic study.

Lenticulostriate arteries (LSAs) arising from the middle cerebral artery (MCA) bifurcation or M2 segment and coursing medially toward the anterior perforated substance (APS), defined here as "far lateral LSAs", represent a surgically vulnerable anatomical variant during procedures such as MCA aneurysm clipping and insular glioma resection. When preoperative imaging is limited, intraoperative recogniton of these branches becomes important. We sought to identify M1 segment morphologic features associated with the presence of far lateral LSAs through analysis of 42 cadaveric hemispheres and 14 cl

Intraoperative Augmented Reality Fiber Tractography for Primary Motor Area Glioma Resection.

The implementation of intraoperative augmented reality fiber tractography (iAR-FT) into the surgical workflow for high-grade supratentorial gliomas has been shown to be effective and safe in maximizing the extent of resection and progression-free survival through the surgeon's enhanced 3-dimensional awareness of the spatial localization of fiber tracts.1-3 Primary motor area tumors present special challenges due to the high eloquence of the precentral gyrus and risk of postoperative onset or worsening of motor deficits, as well as limited postoperative plasticity.4 Although essential, electric

Navigation-assisted resection of tumoral calcinosis of the lumbosacral spine: illustrative case.

BACKGROUND: Tumoral calcinosis is an uncommon disease resulting from dystrophic calcium phosphate crystal deposition, with only 7% of cases involving the spine, and it may diagnostically mimic neoplasms. OBSERVATIONS: In this case, a 54-year-old woman with history of systemic scleroderma presented with 10 months of progressive left lumbosacral pain. Imaging revealed an expansile, 4 × 7-cm, well-circumscribed mass in the lumbosacral spine with L5-S1 neuroforaminal compression. Because intractable pain and computed tomography (CT)-guided needle biopsy did not entirely rule out malignancy, opera

Diagnostic Challenges of Abdominal Tumors Without Typical Imaging Features: From Misdiagnosis of Pancreatic Cystadenoma to Adrenal Pheochromocytoma and the Intraoperative Response.

BACKGROUND Pheochromocytoma is a rare neuroendocrine tumor. Some patients are asymptomatic, and misdiagnosis is common, especially when it lacks typical imaging features. In asymptomatic patients misdiagnosed with pheochromocytoma, hemodynamic instability often occurs during surgical procedures, posing significant challenges to surgeons and anesthesiologists. This case report presents a patient who was misdiagnosed as having a pancreatic cystadenoma prior to surgery, aiming to explore the intraoperative decision-making and considerations for such cases. CASE REPORT A 48-year-old woman with abd

Monitoring with head-mounted displays: performance and safety in a full-scale simulator and part-task trainer.

BACKGROUND: Head-mounted displays (HMDs) can help anesthesiologists with intraoperative monitoring by keeping patients' vital signs within view at all times, even while the anesthesiologist is busy performing procedures or unable to see the monitor. The anesthesia literature suggests that there are advantages of HMD use, but research into head-up displays in the cockpit suggests that HMDs may exacerbate inattentional blindness (a tendency for users to miss unexpected but salient events in the field of view) and may introduce perceptual issues relating to focal depth. We investigated these issu

Nasogastric medication for perioperative Parkinson's rigidity during anaesthesia emergence.

We present a case of severe rigidity during emergence from general anaesthesia in a 64-year-old man who had suffered from Parkinson's disease for nine years. Controversy still exists over how to optimally manage these patients perioperatively. We successfully managed his Parkinsonism with administration of crushed Sinemet" and amantadine via a nasogastric tube. This case report serves as a reminder of the importance that patients receive their anti-Parkinsonian medications perioperatively, and highlights the potential benefits of inserting a gastric tube to continue anti-Parkinson's medication

What is the Marginal Intraoperative Cost of Using an Exoscope or Operative Microscope for Anterior Cervical Discectomy and Fusion? A Time-Driven Activity-Based Cost Analysis.

OBJECTIVE: Our primary objective was to compare the intraoperative costs of 3 different surgical visualization techniques for anterior cervical discectomy and fusion (ACDF). Specifically, we used time-driven activity-based costing (TDABC) methodology to compare costs between ACDFs performed with operative microscopes (OM-ACDF), exoscopes (EX-ACDF), and loupes (loupes-ACDF). METHODS: Total cost was divided into direct and indirect costs. Individual costs were obtained by direct observation, electronic medical records, and through querying multiple departments (business operations, sterile proc

High complication rate with anterior total hip arthroplasties on a fracture table.

BACKGROUND: Recent attention in THA has focused on minimally invasive techniques and their short-term outcomes. Despite much debate over the outcomes and complications of the two-incision and the mini-lateral and mini-posterior approaches, complications arising from use of the anterior THA on a fracture table are not well documented. QUESTIONS/PURPOSES: We determined the intraoperative and postoperative complications with the anterior approach to THA through an extended single-surgeon patient series. METHODS: We reviewed 800 primary THAs performed anteriorly with the aid of a fracture table

Efficacy of intravenous dexmedetomidine-lignocaine infusion compared to morphine for intraoperative haemodynamic stability in modified radical mastectomy: A randomised controlled trial.

BACKGROUND AND AIMS: In recent times, non-opioid analgesic-based anaesthesia has been gaining popularity as it can achieve the goals of hypnosis, amnesia, and haemodynamic stability while avoiding opioid side effects. Our study compares the efficacy of opioid-free anaesthesia and opioid-based general anaesthesia regarding intraoperative haemodynamic stability, anaesthetic requirements, awareness, and recovery profile. METHODS: After receiving ethical approval and registering the trial, we conducted this randomised, single-blinded study on American Society of Anesthesiologists (ASA) physical s
