Fasciola hepatica in the Differential Diagnosis of a Patient with Obstructive Jaundice and Eosinophilia.

Fasciola hepatica is a trematode that is visible to the naked eye. The diagnosis can be made by serology or by clinical improvement and decrease of eosinophilia after triclabendazole treatment or by finding parasite eggs in the stool. Sometimes the diagnosis is possible during unnecessary surgery, laparoscopic cholecystectomy, and endoscopic retrograde cholangiopancreatography (ERCP). F. hepatica infection should be considered in patients with typical clinical findings; the elevation of liver enzymes indicating cholestasis, eosinophilia, and characteristic computed tomography (CT) or ultrasono

An unusual finding of obstructive jaundice-a case report and review of the literature.

Biliary ascariasis is a tropical infectious disease, endemic in regions such as the Far East, Indian subcontinent and parts of the Middle East. The responsible organism is the Ascaris lumbricoides worm. This case study presents a 54-year-old British Caucasian female patient, admitted with a 1-week history of generalized abdominal pain. Ultrasound scan of the abdomen showed a collapsed and abnormal image of the gallbladder. Further imaging for better visualization by magnetic resonance cholangiography showed a 6 cm linear structure in the gallbladder. The morphology was characteristic of gallbl

[Obstructive jaundice after surgery of the biliary tract, a benign stricture or not?].

Obstructive jaundice occurred in 3 patients, 2 women, aged 44 and 60 years, and one man aged 62 years: 17 years after choledochojejunostomy following a complicated cholecystectomy, 8 years after hepatojejunostomy following choledochal cyst removal, and 1 year after laparoscopic cholecystectomy. Percutaneous or endoscopic stent placement was performed in all patients to relieve biliary obstruction. However, stent dysfunction repeatedly resulted in re-stenting of the obstruction. Imaging techniques, as well as endoscopic brush pathology could not distinguish between a benign or malignant stenosi

Bile salts and cholesterol in the pathogenesis of target cells in obstructive jaundice.

Free cholesterol is in rapid equilibrium between serum lipoproteins and red cells. The level of red cell cholesterol is influenced by bile salts, which shift the serum/cell partition of free cholesterol to the cell phase and which inhibit the cholesterol-esterifying mechanism. During incubation in normal serum possessing an active cholesterol-esterifying mechanism, red cells lose cholesterol and surface area and thereby become more spheroidal and less resistant to osmotic lysis. When exposed to serum from patients with obstructive jaundice or to normal serum with added bile salts, red cells ac

A case of obstructive jaundice due to autoimmune pancreatitis and a review of the literature.

A 33-year-old man presented with jaundice, mild pancreatitis, and some weight loss. Laboratory tests showed cholestatic icterus. Imaging revealed distal bile duct obstruction and a mass in the head of the pancreas suspicious for cancer. Besides the enlargement of the head, the whole pancreatic parenchyma was altered. Therefore, autoimmune pancreatitis was considered. Endoscopic ultrasound biopsy found pancreatic tissue with lymphoplasmacytic inflammation and fibrosis. IgG4 serum concentration was within normal range. Due to the benign histology, a therapeutic attempt with prednisolone was init

The effects of erythropoietin on bacterial translocation and inflammation in rats with obstructive jaundice.

INTRODUCTION: Obstruction of the common bile duct is associated with hepatic paranchymal damage and increased susceptibility to subsequent bacterial infections. Erythropoietin has antiinflammatory and cytoprotective effects and it induces antiinflammatory cytokines and suppresses the production of proinflammatory cytokines. In this study, we aimed to investigate the effect of Erythropoietin on bacterial translocation, inflammation and tissue damage in rats with obstructive jaundice. MATERIALS AND METHODS: Thirty-two Wistar albino rats (200-250 g) were divided into 4 groups as follows: Group 1

[The role of biliary decompression and correction of dysbiosis in the treatment of obstructive jaundice].

The article providing in-depth analysis of pathogenesis of obstructive jaundice shows that this disease is manifest not only as changes at the hepatic level (cholestasis, cholehemia, cholangitis, cholangio- and lymphovenous shunts, hepatic encephalopathy) but also as marked dysbiotic disturbances due to anacholia and toxic metabolites that cause bacterial translocation and endotoxemia complicating liver insufficiency. Based on the literary data and original observations, a new scheme for the treatment of obstructive jaundice is proposed including simultaneous correction of both components of h

[Serous microcystic adenoma of the head of the pancreas causing an obstructive jaundice].

BACKGROUND: Serous microcystic adenoma is a rare benign tumor of the exocrine pancreas originating from the ductal system and composed of a large number of small cysts covered by cuboid cells, filled with clear serous fluid and separated with fibrocolagenous stroma. Most frequently it appears in women in 7th and 8th decades, in the distal pancreas. It shows a very low malignant potential. In 2/3 of patients symptoms are uncharacteristic and in 1/3 they are absent When localised within the head of the pancreas it rarely causes an obstructive jaundice. CASE REPORT: We presented a 61-year-old fe

[A rare case of obstructive jaundice and cholecystitis in hepatic fascioliasis in Italy].

The human infestation caused by Fasciola hepatica is a rare zoonosis, with an incidence of about 10 cases/year in Italy. We report a case of cholecystitis and obstructive jaundice in a patient affected by fascioliasis in which the diagnosis was secondary to the extraction of viable flukes from the bile duct during ERCP. The endoscopic examination permits, in addition to a rapid, correct diagnosis, direct clearance of the bile ducts. Oral drug therapy, when carried out following the endoscopic treatment, is aimed at killing any flukes potentially evading mechanical clearance. The healing achiev

Effects of obstructive jaundice on the peripheral nerve: an ultrastructural study in rats.

Obstructive jaundice (OJ) and hepatic disorders have been shown to be associated with peripheral neuropathy in several clinical studies. The study evaluated the effect of OJ on the ultrastructure of the rat sciatic nerve. In the OJ group, jaundice was created by ligation of common bile duct in Wistar-Albino rats. In the sham-operated control group the same procedure was performed without ligation of the bile duct. On day 7, all rats were re-operated and sciatic nerves were explored to harvest 2-cm-long nerve segments for quantitative and qualitative histopathological analysis by light and elec
