BACKGROUND: Clival metastases of adenocarcinomas are exceptionally rare tumors, especially when they arise from the small intestine. Endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA) is being increasingly used in the diagnostic workup of mediastinal diseases. Here, we report a patient with cystic lesions located in the mediastinum, the sampling of which facilitated the diagnosis of renal neoplasm. This paper confirms the usefulness and safety of EBUS/TBNA on cystic lesions and describes a rare presentation of intrathoracic metastases from carcinoma of the kidney.

Extramedullary plasmacytoma is a rare malignant neoplasm typically arising outside the bone marrow of patients who show no clinical evidence of multiple myeloma. Kidney is a rare site for plasmacytoma. We present here a case of primary renal plasmacytoma confirmed on histopathology of the specimen and immuno-histochemistry studies. Patient was treated with radical nephrectomy followed by radiotherapy. The case is presented due to its rarity.

Multilocular cystic renal cell carcinoma (MCRCC) is an uncommon type of cystic renal neoplasm with characteristic histologic findings and a good prognosis. Three cases are reported. One case involves enucleation of an MCRCC in a kidney donor with a 10-year follow-up and no recurrence in the transplant recipient. Nephron sparing surgery should be considered when the diagnosis of MCRCC is suspected preoperatively and confirmed intraoperatively.

Angiomyolipoma is a well described but relatively uncommon benign renal neoplasm composed of varying admixtures of mature adipose tissue, smooth muscle, and thick-walled blood vessels. The incidence of angiomyolipoma is about 0.3% overall. It frequently occurs in patients with tuberous sclerosis. Even more uncommon is the simultaneous occurrence of angiomyolipoma and renal cell cancer in the same kidney in a patient without tuberous sclerosis.

Renomedullary interstitial cell tumors (RMICTs) are often incidentally detected lesions at autopsy or resection of the kidney for other reasons. The RMICTs have not been reported in renal allograft biopsy. Histochemical analysis of frozen, thin sections revealed the distribution of alkaline phosphatase (ALPase) in 47 primary intracranial neoplasms in humans. Expression of SV40 large T antigen was correlated with the development of pathology in three lines of transgenic mice. Syrian golden hamsters were treated with N delta-(N-methyl-N-nitrosocarbamoyl)-L-ornithine (MNCO), a nitrosourea amino acid, which has induced a high incidence of breast, kidney and skin neoplasms and a low incidence of pancreatic carcinoma in rats. MNCO induced a few breast and skin carcinomas, and a high incidence of foci of atypical acinar cells and of focal ductular abnormalities in the exocrine pancreas. The latter were similar to lesions observed in rats treated with MNCO. MNCO was less toxic and less effective as a carcinogen in hamster than in the rat.
