Navigated unicompartmental knee replacement.

Computer-aided systems have been developed recently to improve the precision of implantation of unicompartmental knee replacement (UKR) or total knee replacement. Minimal invasive techniques have been developed to decrease the surgical trauma related to the prosthesis implantation. However, there might be a concern about the potential of minimal invasive techniques for a loss of accuracy. Navigation systems might address this issue. We are currently using routinely a nonimage-based navigation for total knee replacement. We developed a modified system for UKR, suitable for either a conventional…

Managing osteoarthritic knee pain.

Osteoarthritis is one of the most common forms of arthritis seen in primary care practice. Pain associated with this condition is the chief complaint of most patients, prompting them to seek medical attention. Pain can originate from the synovial membrane, joint capsule, periarticular muscles and ligaments, and periosteum and subchondral bone, among other sources. Although osteoarthritis is traditionally thought of as a noninflammatory type of arthritis, inflammatory mechanisms can be present. Therefore, management of osteoarthritic pain involves both nonpharmacologic and pharmacologic modes o

[Pathogenesis of knee osteoarthrist].

More than 20% of the population of over 60-year olds suffers from degenerative joint diseases of the lower extremities. The cause of primary osteoarthritis of the knee is still unknown. A multifactorial genesis is presumed that includes genetic, nutritional, hormonal and age-related factors. On the other hand, secondary osteoarthritis is a sequela of predisposing factors. The most frequent are axial deformities, pre-existing conditions or injuries. Pre-osteoarthritis appears as dysplasias and dystopias (abnormal presentation) of the patella and axial misalignments, incongruities and joint dama…

Multiple-ligament injured knee.

OBJECTIVE: To explore the clinical characteristic of the multiple-ligament injured knee and evaluate the protocol, technique and outcome of treatment for the multiple-ligament injured knee. METHODS: From October 2001 to March 2005, 9 knees with combined anterior cruciate ligament (ACL) and posterior cruciate ligament (PCL) tears in 9 patients were identified with clinical and arthroscopic examinations. Of them, 5 knees were combined with ruptures of posteromedial corner (PMC) and medial collateral ligament (MCL), 4 with disruptions of posterolateral corner (PLC), 2 with popliteal vascular inj…

[Jumper's knee--a review].

Jumper's knee has been defined as painful chronic overuse injury of the extensor mechanism of the knee joint. The disease has a high incidence in jumping sports and depends on training frequency and level of performance. Its natural course is protracted, repetitive, and often bilaterally occurring. Its etiology is a chronic overload of the knee extensor mechanism which is triggered by jumping sports (volleyball, basketball etc.) as well as different intrinsic (ligamentous laxity, Q-angle, patella height, tenderness, pattern of force development) and extrinsic dispositions (frequency of trainin…

Imaging of knee arthroplasty.

Knee replacement surgery, either with unicompartmental or total systems, is common. The purpose of this manuscript is to review the appearance of normal knee arthroplasty and the appearances of complications such as infection, polyethylene wear, aseptic loosening and particle-induced osteolysis, patellofemoral abnormalities, axial instability, and periprosthetic and component fracture. Knowledge of the potential complications and their imaging appearances will help the radiologist in the diagnostic evaluation of the patient with a painful knee arthroplasty.

Locked knee during pregnancy.

An acutely locked knee is a painful and debilitating orthopaedic condition usually caused by a loose body or bucket-handle meniscal tear that requires surgery to remove or repair. We describe 2 cases of acute locking of the knee that occurred during pregnancy. Both patients underwent urgent arthroscopic treatment under spinal anesthesia without complication to the patient or fetus. Concerns regarding surgery during pregnancy revolve around the safety for the mother and fetus. Although local and regional anesthetics are safe with proper management, there is a slightly increased risk of spontane…

Floating knee in children.

The outcomes of 18 ipsilateral displaced femoral and tibial fractures in 17 children are assessed and a new classification system is proposed. Average age was 8.75 years, and follow-up averaged 3.2 years. In the modified Bohn and Durbin classification used, eight cases were type I, four were type II, three were type IIIa, one was type IIIb, and two were type IV. In tibial fractures there was angulation in three cases, and in femoral fractures there were dislocation and angulation in four cases, refracture in one case, leg length discrepancy in four cases, and asymptomatic knee ligament injury …

[Knee arthroscopy in children].

PURPOSE OF THE STUDY: Between 1990 and 1998, 110 knee arthroscopies were performed in children. We analyzed the epidemiology and diagnostic data and studied the correlation between clinical and radiographic findings and the final diagnosis after arthroscopy in order to establish a consensus on use of knee arthroscopy in children. MATERIAL AND METHODS: We made a retrospective analysis of 110 knee arthroscopies performed in children, classing the patients in three age groups: 0-5 years, 5-10 years, 10-17 years. Clinical and radiological findings were compared with the arthroscopy findings. RES…

[Para-articular knee osteotomies].

Osteotomy close to the knee is an established method to treat degenerative osteoarthritis of the knee with varus or valgus deformity. However, indications, surgical techniques and results are discussed controversially. This article gives an overview of those surgical procedures most commonly used for correcting valgus and varus deformities. Supported by our results it is shown that osteotomy of the tibia for degenerative osteoarthritis together with operative arthroscopy in the same operative session gives better results compared to osteotomy alone. Despite the success of arthroplasty of the k…

[Post-traumatic knee degeneration].

Degenerative changes and arthritis are one of the most important sequelae after knee injury. The reasons of deterioration of the cartilage are different in each injury. After fractures the most important pejorative factors are incongruity of the articular surface and malunion specially in varus deformities. With long term follow up the arthrosis is observed between 15 and 50%, dependent of the localisations and the gravity of the injury. After menisco-ligamentous injury the kinematic of the knee is disturbed and the medial femoro-tibial compartment is too overlead specially when the meniscus w…

[Knee injury in children].

The diagnosis approach after knee injury in children must be carefully taken care of the growth plates but it is necessary to emphasize the frequency of the ligament and intra-articular lesions, and on the other hand to consider their benign reputation. Clinical and conventional radiologic examination give in most of the cases the correct diagnosis. For the others an examination under general anesthesia ++ is necessary to distinguish the ligament and the growth plate lesions. MRI must not replace the arthroscopy which is necessary for an haemarthrosis with a stable knee or a little isolated an…

Acute traumatic knee hemarthrosis.

A rapidly developing hemarthrosis in an acutely injured knee is reported to be associated with "surgically significant" lesions, such as anterior cruciate ligament tears, meniscus tears, and osteochondral fractures, in < or = 90% of cases. We report two cases of adult men who showed signs of such acute hemarthroses in stable knees after significant trauma. Neither had significant intraarticular pathology, such as meniscal, articular, or ligamentous injury, at arthroscopy. Persistent postarthroscopic bleeding led to additional diagnostic testing that found mild factor VIII deficiency. Hemophili…

Knee injuries in volleyball.

The knee is the most frequent site of injury in volleyball players. More than 40% of high level players suffer overuse injuries during this activity; this particularly painful syndrome is caused by the amount of jumping typical in volleyball play, and in its training which aims at strengthening the quadriceps muscle. In volleyball players the extensor apparatus is subject to continuously high stress and the bone tendon junction, being the weakest point, is susceptible to lesion. The prevention and treatment of 'jumper's knee' requires a high degree of cooperation among trainers, doctors and at…

Epidemiology of jumper's knee.

Jumper's knee is a typical functional overload injury because it affects those athletes who submit their knee extensor mechanisms to intense and repeated stress, e.g. volleyball and basketball players, high and long jumpers. According to the classification of Perugia and colleagues, it is an insertional tendinopathy affecting, in order of frequency, the insertion of the patellar tendon into the patella (65% of cases), attachment of the quadriceps tendon to the patella (25%) and the attachment of the patellar tendon to the tibial tuberosity (10%). The frequent occurrence of this injury in athle…

Rehabilitation following knee injury.

Over the past decade, there has been a resurgence of interest in techniques of rehabilitation of the injured knee. Technologic advances have provided instruments that yield objective measurements so that appropriate staging can be done as part of the rehabilitation plan. Research in multidisciplinary fields has provided data that currently allow a rational physiologic approach to progressive care of the injured lower extremity. No longer is the adage "work it out and let's see how it goes" appropriate care for the injured knee. Starting with the proper diagnosis, rehabilitation begins at the t…

Evaluating acute knee injuries.

The diagnosis of acute knee injuries should be as precise as possible. A wait-and-see attitude is condemned. A complete diagnosis cannot always be made on the basis of history, physical examination and x-rays. Additional aids, such as stress films, joint aspiration, arthrography, examination under anesthesia and arthroscopy, increase diagnostic accuracy to nearly 100 percent. Only then can the physician properly manage the patient so that long-term disability is minimized.

Anametric total knee arthroplasty.

The early experience with the anametric knee prosthesis shows it to be effective in achieving the aims desired in total knee arthroplasty. Decreased pain and improved function have been noted in most patients, with relatively few complications. In osteoarthritics, patellar replacement increases the level of improvement. This prosthetic system has been improved by the addition of further sizes and additional tibial fixation options. Detailed long-term follow-up evaluation of these patients will be necessary to judge the effectiveness of the newly introduced design modifications.
