Ultrasound-based evaluation revealed reliable postoperative knee stability after combined acute ACL and MCL injuries.

PURPOSE: Anterior cruciate ligament (ACL) injuries are often combined with lesions of the medial collateral ligament (MCL). The aim of this study was to evaluate treatment outcome of combined acute ACL and MCL lesions using functional US and clinical examination. METHODS: Patients aged > 18 years undergoing primary ACL reconstruction with concomitant operative (group 1) or non-operative treatment of the MCL (group 2) between 2014 and 2019 were included after a minimum follow-up of 12 months. Grade II MCL injuries with dislocated tibial or femoral avulsions and grade III MCL ruptures underwent

In recreational alpine skiing, the ACL is predominantly injured in all knee injuries needing hospitalisation.

PURPOSE: The knee joint still represents the most frequent anatomical injury location accounting for about one-third of all injuries in recr … level and risk-taking behaviour. RESULTS: The anterior cruciate ligament (ACL) was injured in all knee injuries recorded. Of the total study sample, 64.5% (n = 182) were ACL injuries with concomitant injuries and about 35.5% (n = 100) were isolated ACL injuries, not involving any other structures of the knee joint. In general, most common concomitant injury d …

Eccentric Knee Flexor Strength and Risk of Hamstring Injuries in Rugby Union: A Prospective Study.

BACKGROUND: Hamstring strain injuries (HSIs) represent the most common cause of lost playing time in rugby union. Eccentric knee flexor weakness and between-limb imbalance in eccentric knee flexor strength are associated with a heightened risk of HSIs in other sports; however, these variables have not been explored in rugby union. PURPOSE: To determine if lower levels of eccentric knee flexor strength or greater between-limb imbalance in this parameter during the Nordic hamstring exercise are risk factors for HSIs in rugby union. STUDY DESIGN: Cohort study; Level of evidence, 2. METHODS: Th

[External fixation with motion capacity in complex dislocation of the knee joint and associated injuries].

Closed and open dislocations of the knee joint with vascular and nerve injuries are treated immediately and may present an indication for external fixation. In acute trauma definitive treatment and reconstruction of the complex capsule and ligamentous injuries are often impossible. The application of a medial transarticular external fixator with motion capacity combines the treatment of remaining joint instability with the benefit of early joint movement. Disadvantages of prolonged joint immobilisation until definitive stabilisation of the knee are reduced.

Treatment of acute and chronic anterior cruciate ligament-posterior cruciate ligament-lateral side knee injuries.

Knee dislocations are rare but devastating injuries. The ACL-PCL-lateral side injury combination is representative of the challenges these injuries present. Early management is focused on vascular integrity. When possible, acute repair and reconstruction within 3 weeks from injury is preferred. Chronically deficient knees generally will require lateral side reconstruction rather than repair and may require limb realignment. Addressing all injured structures is imperative to afford the best chance at a reasonable outcome. Good results with surgery are possible, but some degree of persistent dis

A systematic approach to magnetic resonance imaging interpretation of sports medicine injuries of the knee.

Magnetic resonance imaging is performed more commonly on the knee than on any other joint, and it is an excellent diagnostic tool that can aid in the evaluation of a host of sports-related injuries involving the ligaments, tendons, menisci, osseous structures, and articular surfaces. A thorough evaluation of the images, however, can be a daunting task, as the study often contains dozens of images obtained with multiple pulse sequences and in several imaging planes. A systematic approach will facilitate an accurate and timely evaluation of this complex examination and will ensure that all of th

[Proprioceptive abilities of surgically and conservatively treated knee joints with injuries of the cruciate ligament].

UNLABELLED: In the present study we evaluated proprioceptive capabilities of the knee joint with a balance test and correlated these findings to parameters which document mechanical stability. We compared 8 conservatively treated and 12 surgically treated patients with ACL-deficient knee joints with a control group of 12 subjects. The balance test was performed with a Kistler force plate. Both the conservatively treated and the surgically treated patients showed significantly higher deviations of their centre of gravity than the control group. This was true not only for the injured leg but als

Complete dislocation of the knee: spectrum of associated soft-tissue injuries depicted by MR imaging.

OBJECTIVE: Complete knee dislocation is a rare injury. The purpose of this study was to evaluate the spectrum of injuries that are depicted by MR imaging in patients with a dislocation of the knee and to determine if there is any predictive factor that might indicate those patients who may be at risk for popliteal nerve injury. MATERIALS AND METHODS: A retrospective search for patients sustaining traumatic knee dislocations who had radiographs and an MR imaging examination of the knee as part of their initial evaluation was done at three level I trauma centers for the period between 1989 and

Knee laxity and function after conservative treatment of anterior cruciate ligament injuries. A prospective study.

Knee laxity and function were investigated in a prospective study including 40 patients with acute anterior cruciate ligament injury and 11 patients with minor knee injury not involving the cruciate ligaments (controls). None of the patients had primary ligament repair. Instrumental testing (OSI, Hayward, California) of the static laxity of both knees was performed at one, three, six, and 12 months after the injury. The initial laxity of the anterior cruciate-deficient knees was greater than that of the controls. At 12 months the laxity at 180N tibial load for the patients with anterior crucia

Polygenic Collagen Variants and Their Association With Ligament Injury Patterns in Patients With Knee Ligament Injuries.

Knee ligament injuries, particularly anterior cruciate ligament (ACL) tears, represent a major cause of functional limitation in physically active individuals. While biomechanical and environmental factors are well-established contributors, increasing evidence suggests that genetic variability in collagen-related genes may influence individual susceptibility to ligament injury. Most previous studies, however, have focused on elite athletic populations, limiting their applicability to routine clinical settings. A prospective cross-sectional study was conducted in 94 adult patients with clinical

OPTIKNEE 2022: consensus recommendations to optimise knee health after traumatic knee injury to prevent osteoarthritis.

The goal of the OPTIKNEE consensus is to improve knee and overall health, to prevent osteoarthritis (OA) after a traumatic knee injury. The consensus followed a seven-step hybrid process. Expert groups conducted 7 systematic reviews to synthesise the current evidence and inform recommendations on the burden of knee injuries; risk factors for post-traumatic knee OA; rehabilitation to prevent post-traumatic knee OA; and patient-reported outcomes, muscle function and functional performance tests to monitor people at risk of post-traumatic knee OA. Draft consensus definitions, and clinical and res…

Changes in Health-Related Quality of Life and Knee Function After Knee Injury in Young Female Athletes.

BACKGROUND: Recent literature has called for greater attention to evidence-based practice in sports medicine with the documentation of overall status and impairments following injury. The prospective documentation of impairments associated with knee injuries in female athletes regarding their health-related quality of life (HRQoL) and knee function (KF) of high school and collegiate athletes is limited. Assessing the effect knee injuries have on young female athletes may allow clinicians to better understand the perspectives of the athletes who sustain these injuries. PURPOSE: To document the…

The extent and risk of knee injuries in children aged 9-14 with Generalised Joint Hypermobility and knee joint hypermobility - the CHAMPS-study Denmark.

BACKGROUND: Generalised Joint Hypermobility (GJH) is suggested as an aetiological factor for knee injuries in adolescents and adults. It is presumed that GJH causes decreased joint stability, thereby increasing the risk of knee injuries during challenging situations like jumping and landing. The aim was to study the extent and risk of knee injuries in children with GJH and knee hypermobility. METHODS: In total, 999 children (9-14 years) were tested twice during spring 2012 and 2013 with Beighton's Tests (BT) for hypermobility, a 0-9 scoring system. GJH was classified with cut-point ≥5/9 on bo…

Decreased Knee Joint Loading Associated With Early Knee Osteoarthritis After Anterior Cruciate Ligament Injury.

BACKGROUND: Anterior cruciate ligament (ACL) injury predisposes individuals to early-onset knee joint osteoarthritis (OA). Abnormal joint loading is apparent after ACL injury and reconstruction. The relationship between altered joint biomechanics and the development of knee OA is unknown. HYPOTHESIS: Altered knee joint kinetics and medial compartment contact forces initially after injury and reconstruction are associated with radiographic knee OA 5 years after reconstruction. STUDY DESIGN: Case-control study; Level of evidence, 3. METHODS: Individuals with acute, unilateral ACL injury compl
