[Magnetic resonance imaging research progress of hydropic ear disease].

Hydropic ear disease is an otological disease caused by hydrops in the labyrinth of the inner ear membrane. According to the medical history and imaging classification system, it is mainly divided into two types: primary hydropic ear disease and secondary hydropic ear disease. The clinical manifestations vary depending on the anatomical localization of hydrops. Cochlear endolymphatic hydrops can lead to hearing loss, tinnitus, and ear fullness. Vestibular endolymphatic hydrops can cause dizziness, vertigo, nausea, vomiting, and gait instability. A number of studies on magnetic resonance imagin

Osteogenesis imperfecta of the temporal bone: CT and MR imaging in Van der Hoeve-de Kleyn syndrome.

We report the progressive otic capsule demineralization around the membranous labyrinth and facial nerve in an adult patient with osteogenesis imperfecta tarda. Whereas the initial CT scan showed bandlike, undermineralized pericochlear areas, 2 years later, repeat CT performed because of hearing deterioration showed progression of these findings to the promontory, the round window niche, and the labyrinthine and tympanic segments of the facial nerve canal. MR imaging demonstrated enhancement of the abnormal otic capsule and of the intratemporal and canalicular facial nerve. The differential di

Multiple otopathologic disorders.

Temporal bones (1,383) from 713 patients were studied systematically for multiple pathologic lesions. Eleven percent (152 temporal bones) were found to have more than one pathologic finding. Males (60.5%) had multiple diseases more commonly than did females (37.7%). The most frequently occurring findings were otitis media (71.1%), otosclerosis (43.4%), endolymphatic hydrops (38.8%), labyrinthitis (25.0%), and cancer (24.3%). We conclude that multiple coexisting pathologic conditions can have coincidental or causative relationships. The otolaryngologist should consider multiple pathologic condi

[Endocrine cells of the internal ear in the normal state and in specific labyrinthine lesion in rabbits].

Using light and electron microscopes, apudocytes of internal ear in normal and syphilis-infected rabbits were examined. Temporal bone blocks were dissected from 16 healthy (control) and 32 infected (experimental) animals. Some of the preparations were hematoxylin and eosin stained for histological examinations; some were silver stained according to Griemelius for histochemical examinations; and others were used to prepare ultrathin sections for electron microscopy. It was demonstrated that apudocytes were normally found in different labyrinthine structures and consistently seen in the vascular

[ORL selection of French astronauts].

Strict criteria were used, under the control of the Centre National d'Etudes Spatiales (CNES) to select seven French spationautes for future participation in human cargo space flights to take place between 1987-1991 within the framework of the scientific missions of the USA and USSR. Data and results of the clinical O.R.L. examinations and vestibular tests conducted during medical operations of this selection procedure are presented.

Mechanisms of nystagmus.

The slow phases of nystagmus can be divided into four types based on their wave forms: linear, exponentially decaying, exponentially increasing, and sinusoidal. The former three give risk to "jerk" nystagmus; the last to "pendular" nystagmus. The drift of the eyes that creates the slow phase of nystagmus can be analyzed by considering the mechanisms that normally hold the eyes still so that images are stable upon the retina for best visual acuity. Thus, we have a vestibular-optokinetic system to keep images of stationary objects stable on the retina during head rotation; a pursuit system to ke

Diagnosis and Treatment of Perilymphatic Fistula.

Perilymphatic fistula (PLF) is defined as an abnormal communication between the fluid (perilymph)-filled space of the inner ear and the air-filled space of the middle ear and mastoid, or cranial spaces. PLF is located in the round or oval window, fractured bony labyrinth, microfissures, anomalous footplate, and can occur after head trauma or barotrauma, chronic inflammation, or in otic capsule dehiscence. This clinical entity was initially proposed more than a century ago, yet it has remained a topic of controversy for more than 50 years. The difficulty of making a definitive diagnosis of PLF

[Tumors of the inner ear and adjacent structures].

Tumors of the inner ear and adjacent structures often present with hearing loss, tinnitus and vertigo due to compression of the traversing cranial nerves. More than 90% of the tumors of the inner ear with or without expansion into the cerebellopontine angle are histologically diagnosed as vestibular schwannomas. Less common tumorous lesions include ectopic meningiomas located in the petrous bone, glomus tympanicum paragangliomas or endolymphatic sac tumors (ELST) originating in the vestibular recess. Most tumors are sporadic, but hereditary disorders have to be considered. Bilateral vestibular

What can posturography tell us about vestibular function?

Patients with balance disorders want answers to the following basic questions: (1) What is causing my problem? and (2) What can be done about my problem? Information to fully answer these questions must include status of both sensory and motor components of the balance control systems. Computerized dynamic posturography (CDP) provides quantitative assessment of both sensory and motor components of postural control along with how the sensory inputs to the brain interact. This paper reviews the scientific basis and clinical applications of CDP. Specifically, studies describing the integration of

Drugs affecting the inner ear. A review of their clinical efficacy, mechanisms of action, toxicity, and place in therapy.

Many drugs have a site of action within the inner ear. The list includes therapeutic, diagnostic and ototoxic agents. Therapeutic agents are most useful in cases of infections, endolymphatic hydrops, vascular insufficiency, vertigo of peripheral origin, autoimmune disease, otosclerosis (otospongiosis), sudden hearing loss and tinnitus. For infections, the most widely used anti-microbial agents are the penicillins and cephalosporins. There are no antiviral agents that have been proven useful for inner ear viral infections. However, steroids have been of some value for controlling some of the se

Closed versus open technique in the management of labyrinthine fistulae.

A labyrinthine fistula is the most common complication of cholesteatomatous chronic ear disease. Its treatment remains a controversial subject. The present paper reports our approach to the management of this complication. Operations were performed on 1,226 cases of chronic otitis media with cholesteatoma between January 1971 and December 1985. A labyrinthine fistula was detected in 158 cases. We favor intact canal wall tympanoplasty even in the presence of medium or large fistulas: in the latter case, the matrix is not removed but is trimmed to cover only the bony defect and it is left in pla
