Medico-legal implications of traumatic cataract.

Traumatic cataract is due to lens damage when mechanical, irradiative, electrical or chemical agents injury the globe. The appearance of a traumatic cataract is typically short and unilateral with rare spontaneous resolution and often involves other ocular anatomical areas. Medico-legal evaluation of the appearance and the consequences of a traumatic cataract requires a correct methodological approach with the support of qualified ophthalmological competences.

Retained nuclear fragment found during Descemet-stripping automated endothelial keratoplasty.

An 82-year-old man with a 2- to 3-month history of progressive visual loss due to corneal edema was referred to our center. The ocular history was significant for uneventful cataract surgery approximately 3 years earlier. Pseudophakic bullous keratopathy was diagnosed and Descemet-stripping automated endothelial keratoplasty (DSAEK) performed. During surgery, a retained nuclear fragment was discovered in the anterior chamber. The fragment was removed and the DSAEK completed successfully. At the 4-month follow-up, the symptoms had completely resolved and the uncorrected distance visual acuity i

[Phacoemulsification of the dislocated lens nucleus in the vitreous body].

BACKGROUND: one of the most intriguing complications in phacoemulsification is the rupturing of the posterior capsule with los of the nucleus into the vitreous. As severe inflammation will follow, the removal of the nucleus is mandatory. Several methods have been described. Our approach has the advantage to minimize some of the imminent risks. PATIENTS AND METHOD: On 15 eyes phacoemulsification in the vitreous cavity following vitrectomy has been carried out. Supporting liquids like viscoelastic substances or fluordecalin were not necessary. RESULTS: There was no indication of retinal damage

Posterior chamber intraocular lenses in a series of 75 autopsy eyes. Part III: Correlation of positioning holes and optic edges with the pupillary aperture and visual axis.

The possibility that undesirable visual complications such as glare, halo, monocular diplopia, or other visual aberrations can result from the presence of posterior chamber lens optic edges or such lens elements as positioning holes or loop-optic junctions within the pupillary aperture has received little attention. There is recent clinical evidence that these phenomena may be clinically significant. In a series of 75 autopsy eyes with posterior chamber intraocular lenses, we have observed that in 71% of cases an optic edge, or element of the optic such as a positioning hole, was situated eith

Aetiopathological aspects of inherited lens dislocation in the Tibetan Terrier.

Four litters of Tibetan Terriers were bred in order to study the ocular pathology of primary lens luxation, which in this breed is due to an autosomal recessive gene. Scanning and transmission electron microscopy were carried out on ocular tissues of 5 dogs from 2 homozygous litters killed at various ages up to the time of onset of clinical subluxation. All eyes showed zonular abnormality, in particular a bizarre arrangement of fibrillar material associated with the ciliary processes and relating to the system of zonular fibres inserting posteriorly at the lens equator. A further litter, the r

18-month results of double-fanged 5-0 polypropylene suture transscleral bag fixation in subluxated cataracts.

PURPOSE: To evaluate the stability and efficacy of the double-flanged 5-0 polypropylene suture to fixate subluxated cataracts at 18 months and the possible complications of this new technique. METHODS: This technique uses a 5-0 polypropylene monofilament to create two flanges with a thermocautery, for fixation of a capsular tension segment to the sclera to fix the subluxated capsular bag. This technique was implemented in 17 eyes requiring intraocular lens implantation in a setting of zonular dialysis due to trauma, Marfan syndrome, microspherophakia, idiopathic disease, and post-phacoemulsif

[Angiomatous choroidal and orbital lesions in a patient with Sturge Weber syndrome].

CASE REPORT: We present the case of a 34-year-old man with Sturge-Weber syndrome, who presented to the emergency room with left ocular pain and left chronic exophthalmos. He suffered an acute glaucoma secondary to pupillary block consequent upon an anterior lens subluxation. Orbital contrast Magnetic Resonance Imaging (MRI) was performed and he underwent intracapsular lens extraction. DISCUSSION: The MRI showed T2 enhanced lesions in the left meninges, choroid, and orbit, compatible with cavernous hemangiomas, as well as a dilated superior ophthalmic vein. Intraocular pressure after cataract …

Post traumatic subconjunctival dislocation of lens in Ehlers-Danlos syndrome.

Ehlers-Danlos Syndromes are a rare group of inherited connective tissue disorders of defective collagen synthesis. They predominantly involve the skin, joints and vessels. Ocular involvement in the form of blue sclera, microcornea and susceptibility to trauma is a common feature of EDS type VI. A minor ocular trauma in these patients can cause globe rupture. We herein report simple and successful management of a case of traumatic subconjunctival dislocation of lens in a patient of EDS type VI.
