Psychiatric history and related exposures as risk factors for Alzheimer's disease: a collaborative re-analysis of case-control studies. EURODEM Risk Factors Research Group.

Data from case-control studies of Alzheimer's disease (AD) were pooled to examine the possible roles of history of depression, anti-depressant treatment and adverse life events as risk factors. History of depression was found to be associated with AD, although the effect was confined to late onset cases. The association held for episodes of depression more than 10 years before AD onset, as well as for episodes occurring within a decade of onset. No association was found with anti-depressant treatment. However, data were only available from two studies, limiting the power of the analysis. Also,

Diagnosing depression--looking backward into the future.

Before the advent of antidepressants, no generally accepted and operationalized classification of depression existed. In the late fifties and sixties we developed a multi-axial and operationalized depression taxonomy in order to be able to study systematically the clinical and biochemical action of antidepressants and biological characteristics of depressives responsive to antidepressants. The keypoints of that system and the data on which it was based are discussed, after which it is compared with the depression classification proposed by the DSM III and DSM III-R. Though it is recognized tha

Correlates of psychological distress in involuntary job loss.

This study estimated correlates of psychological distress with a heterogeneous sample of American subjects (N = 122; M age = 33.5 yr.) who were involuntarily displaced from work. Expectation of finding a new job is a significant source of psychological distress in job loss after taking into account length of unemployment and economic dependence. Psychological distress also showed significant negative association with self-esteem and significant positive relationship with job-seeking effort. Other important findings are discussed.

[Personality and coping with stress in patients with functional dysphonia].

The study was done to prove the etiopathogenetical power of some personality traits, proceeding life events, and strain effects when exposed to an experimental work-stress. 20 functional dysphonics, 14 individuals suffering from organic voice disorders and a group of 20 control persons were examined. The results show that the group of functional dysphonics produced a significant more positive self-evaluation regarding their personality with a clear tendency to restraint. Compared to the control group they showed a higher level of state anxiety in situations causing stress. Furthermore the infl

Lateralization of emotional expression under a neuroleptic drug.

Sixteen student volunteers were administered placebo and 50 mg of chlorpromazine (CPZ) in tablet form, on separate occasions, two hours before testing. Whilst volunteers related pleasant and unpleasant personal life events, the frequency and intensity of expressions on the left and right sides of the face were rated by independent observers. Subsequently the valence of the narration was also rated. Similar to a previous study facial expressions during negative emotions were expressed more often and intensely in the left face but positive emotions were bilaterally represented. CPZ neutralised t

Depression among adolescent mothers: a pilot study.

Eighteen women less than 22 years old, who had their first child during adolescence, were assessed with a standard in-depth diagnostic interview for the presence of and/or history of depression and other disorders. Eight of the 18 women (44%) had experienced at least one episode of major depressive disorder at some point in their lives. A majority of episodes had their onset during pregnancy. Moreover, six women (33%) were experiencing a diagnosable form of depression at the time of the interview. Most of the women who had been depressed experienced at least part of their depression while they

Cortisol nonsuppression in depression: relationship to clinical variables.

The dexamethasone suppression test was performed in 63 depressed patients and 43 normal controls. Cortisol nonsuppression was found in 47.6% of the depressed patients and 4.6% of the controls. Among depressed patients both age and reported weight loss, but not severity of depression, were significantly related to postdexamethasone plasma cortisol levels. Patients who were cortisol suppressors had experienced significantly more life events before the onset of depression and had significantly higher hostility scores on a personality questionnaire than cortisol nonsuppressors.

Differences between experienced and anticipatory distress.

Differences between distress ratings of anticipated and experienced life events were examined (N = 168). Results showed significant differences between perceived and experienced aversion across events predicting symptoms of stress, depression, anxiety, and somatic discomfort in occupational, social, and familial situations. Gender differences were also found. Women reported significantly greater ratings of distress than men. Results reinforce the use of experienced events in overall distress assessment for individual clients, bring to question the existence of anticipated or perceived stress,

Does depressive illness in the elderly have a poor prognosis?

The few available longitudinal studies of depressive illness in the elderly indicate a poor prognosis. Various risk factors are discussed. Both severe and chronic physical illness, and severity of the depressive disorder are the two most important prognostic factors. The possibility of interactional effects between these risk factors should be studied. Most studies to date have been of severely depressed patients and this may give an unduly pessimistic picture. A case is made for the desirability of well-designed prognostic longitudinal studies with subjects being drawn from a wide spectrum of

Life events assessment of depressed patients: a comparison of self-report and interview formats.

Both self-report questionnaires and semi-structured interviews have been employed in past research on the association between life events and psychiatric disorder. We examined the comparability of these two approaches by giving both a self-report life events scale and semi-structured life events interview to 38 depressed patients. About 40% of the items noted on the interview were missed when using the questionnaire. Approximately 15% of the items noted on the questionnaire were errors since the events did not meet the definition or time criteria specified in the questionnaire. Twenty percent

A model for evaluating the stressed patient in the family practice setting.

Family physicians frequently encounter patients with stress-induced medical and psychological problems. When the symptoms are severe, or the mechanisms are vague, it is often useful to perform a detailed medical, behavioral, and psychological evaluation in order to initiate effective therapy. This paper illustrates a model for evaluating stress-related and other psychosomatic problems in a family practice.

Stressful life events and depressive symptomatology in obstetric patients: a pilot study in a family practice setting.

Several studies have found the presence of stressful life events to be associated with the appearance of postpartum depression. However, many of these previous studies have been conducted in obstetric clinics; relatively little research in this area has been done in a family practice setting. The first objective of this study was to note the prevalence of depressive symptomatology in a family practice clinic's pregnant patients. The second purpose was to evaluate the relationship between stressful life events and depressive symptomatology during pregnancy and the postpartum period. Each subjec

Life events occurring before and after onset of depression in a Kenyan setting--any significance?

In a controlled study of life events in depressed patients in Kenya it was found that the rate of life events was higher in the depressed group in the 27 weeks preceding the onset of depression but with a sharp increase in the last 6-9 weeks before onset. It was also found that the depressed patients continued to experience independent life events after the onset of the depression. It is hypothesized that the continued occurrence of life events after the onset of depression is an incentive to seek help and, eventually, a psychiatric referral.

[Clinical aspects of geriatric psychiatry].

Cause and development of psychic disorders during old age today cannot be explained merely on the basis of a degenerative organic brain process alone but have to be viewed under the aspects of multimorbidity with biologic-somatic factors, biographic and social determined situations as well. The disease of old age does develop in the sense of a multifactorial genesis. Besides that exogenous bodily-factors, e.g. inflammatory disease, cardio-vascular diseases do play an important role. In judging the psychopathology one always has to consider the possibility that the behavior of the sick and old

Climacteric symptoms: a study in the Indian context.

The climacteric is a universal phenomenon which has received relatively little attention from psychiatrists, psychologists, sociologists, anthropologists and social workers all over the world, but almost no research on this subject has been carried out in the Third-World countries. This study, carried out in India, has been conducted for the purpose of unravelling the difficulties that Indian women have to face during the climacteric. 405 married women between 40 and 55 yr of age from the general population were contacted and interviewed. The results, obtained with the menopausal symptom check

Psychosomatic factors in patients with hyperprolactinemia and/or galactorrhea.

A biographic and clinical investigation of 101 patients with hyperprolactinemia and/or galactorrhea is reported. Fifty-one patients were reared without their fathers and 18 with an alcoholic, violent one. These situations were uncommon in the control population, and the differences were statistically significant. There was a high frequency of complaints of obesity, headaches, frigidity, lightheadedness, and fullness of the abdomen, limbs, or face. There was a significant temporal correlation of external events in the natural history with onset or worsening of the symptoms. It is concluded that

The value of interviewing family and friends in assessing life stressors.

A reliability study of a life events questionnaire administered to 117 pairs of respondents indicated that a "significant other" (family member or friend) added approximately 29% new information to that gathered from the patient alone. A validity check of this information with a "knowledgeable" third party confirmed approximately 80% of the events reported by the subjects and significant others. The findings suggest that studies designed to collect information about the occurrence of specific life stressors would obtain more reliable and no less valid data from separate interviews of patients

Reduce negative life events to Increase satisfaction: A daily diary study on the relationship between negative life events and life satisfaction.

Life satisfaction refers to an individual' s cognitive evaluation of the overall quality of their life considering the various aspects therein. Although the existing research has demonstrated the between-person relationship between negative life events and life satisfaction based on retrospective measures, less is known about this relationship at the within-person level. A daily diary method could examine this within-person relationship and decrease systematic recall biases. Therefore, this study investigated the link between daily negative life events and daily life satisfaction, as well as t
