UNLABELLED: There are many well-established methods for the reconstruction of the lower lip. The selection of a particular method generally relies on the amount of lip resected and the amount of lip remaining. In cases of large defects (>50% of the lip length) where direct closure and lip-switch techniques are inadequate, a perioral flap is used. All techniques for perioral flaps described until now result in an unwanted decrease in circumoral opening. Lip augmentation procedures can restore volume and shape to the aging, thin upper lip, but some patients may develop problematic lip tightness. This stiff upper lip is manifested by a restricted smile and an adynamic central upper lip. We have had success in treating postreconstruction and postaugmentation stiff upper lip with a therapeutic device and treatment regimen. This therapy alleviated tightness and inability to smile. Also, the change in lip commissure-to-commissure distance in repose and when smiling improved after treatment.

The author describes a modified mucous-membrane advancement flap for the repair of defects following the removal of premalignant actinic cheilitis or squamous cell carcinoma. The advantages of this modification over other techniques include a more broadly resurfaced lip bilaterally, and prevention of punctate scarring at the lateral oral commissures by use of a continuous mucous membrane in and around the commissures themselves.

Lipoma is very common benign tumor of adipose tissue, but its presence in the oral cavity is rare. Fibrolipoma is a histological variant of lipoma that affects the buccal mucosa and nasal septum in the orofacial region and causes functional and cosmetic issues. However, this lesion has not been reported on the upper lip and may be confused with other soft tissue lesions of children. Hence, accurate histopathological examination of lipomas is important for a correct treatment plan. The purpose of this report is to describe the case of an 11-year-old boy with fibrolipoma of the upper lip.

The upper lip was totally reconstructed with a radial forearm sensory flap and vermilionplasty using medical tattooing after resection of a malignant melanoma.
