The results of endoscopic percutaneous transhepatic cholelithotripsy in seven patients with stones in the deep biliary passages which could not be treated by endoscopic papillotomy (EST) or extracorporeal shock wave lithotripsy (ESWL) are presented. A new dilator technique combined with mechanical or laser-lithotripsy was employed. All of the patients were relieved of the stones after uncomplicated course. It is concluded that endoscopic percutaneous cholelithotripsy which can be carried out under local anaesthesia, should be offered to patients with stones in the deep biliary passages which a

Endoscopy is widely accepted as the first treatment option in the management of bile duct stones. In this review we focus on the alternative endoscopic modalities for the management of difficult common bile duct stones. Most biliary stones can be removed with an extraction balloon, extraction basket or mechanical lithotripsy after endoscopic sphincterotomy. Endoscopic papillary balloon dilation with or without endoscopic sphincterotomy or mechanical lithotripsy has been shown to be effective for management of difficult to remove bile duct stones in selected patients. Ductal clearance can be sa

Sialolithiasis is a common salivary gland disease. It may cause swelling of the gland, infection of the gland, or even death of glandular parenchymal cells. Various treatment modalities are available for management of sialolithiasis such as extracorporeal shock wave lithotripsy, sialoendoscopy, laser intracorporeal lithotripsy, interventional radiology, the video-assisted conservative surgical removal of the parotid, and submandibular calculi. These sophisticated procedures may not be available in all health-care centers. In this article, a new simple yet precise technique for management of du

INTRODUCTION: Horseshoe kidney is often associated with ureteropelvic junction obstruction and nephrolithiasis. Retrograde intrarenal surgery (RIRS) is becoming one of the main treatment options for large stones in such patients.
AIM: To study the results of RIRS in patients with horseshoe kidney.
MATERIALS AND METHODS: Between November 2016 and April 2021, 12 patients with stones in horseshoe kidney underwent RIRS in our clinic. There were 9 men and 3 women. The average age of the patients was 44.5+/-12.0 years, the size of the stone was 1.6 cm. In 9 patients, a solitary pelvic stone with a

Twenty-nine patients with salivary stones were treated with the endoscopically-controlled electrohydraulic shock wave lithotripsy (EISL). This new minimally invasive treatment of sialolithiasis is performed under local anesthesia on an outpatient basis with little inconvenience to the patient. For endoscopy, a flexible fibroscope with an additional probe to generate shock waves is placed into the submandibular duct and advanced until the stone is identified. For shock wave-induced stone disintegration, the probe electrode must be placed 1 mm in front of the concrement. The shock waves are gene

This study aims to evaluate the discriminative ability of the S.T.O.N.E. score for residual fragments following ureteroscopy and laser lithotripsy in patients with surgically confirmed impacted ureteral stones. We retrospectively analyzed 272 adult patients who underwent laser lithotripsy via semirigid ureteroscopy for impacted ureteral stones between January 2019 and January 2025. Preoperative computed tomography imaging was used to calculate the S.T.O.N.E. score, incorporating five parameters: stone size, location, hydronephrosis, number, and Hounsfield Unit value. Statistical analyses inclu

Introduction Ureteric stone management has undergone a transformation since the advent of different types of lithotripters. Although holmium: yttrium-aluminum-garnet laser lithotripsy has been considered the gold standard of choice for several centers, it is limited in availability in underdeveloped/low-resource areas. Pneumatic lithotripsy (PL) is still a relatively inexpensive and widely available modality. The purpose of this study is to evaluate the effectiveness and safety of PL as an alternative to laser lithotripsy in low-income settings. Methods Retrospective data from 215 patients tre

The main indications for laser therapy in the gastrointestinal tract are inoperable tumors of the esophagus, cardia and the recto-sigmoid. Exophytic tumors with a maximum length of the tumor stenoses between 5 and 7 cm are the best candidates for laser treatment. In gastroenterological stone centers, the laser-induced intracorporeal shock wave lithotripsy is now established. The sophisticated principle of the so-called photodynamic therapy is still in an experimental stage. Photodynamic therapy offers the option of curative therapy in small malignant or premalignant (Barrett esophagus with sev

Endoscopic sphincterotomy is one of the more effective therapeutic procedures for the management of some biliary tree abnormalities. In choledocolitiasis, a 90% succesfull rate has been obtained. Complications include bleeding, perforation, cholangitis, and pancreatitis. Mortality rates between 1.0 to 1.3% are informed. Contraindications are the same as for panendoscopy as well as the presence of stones greater than 2.5 cms. In giant stones, some other endoscopic approaches can be used, including mechanical lithotripsy, chemical treatment, electrohydraulic shockwaves, laser and biliary stent a

INTRODUCTION: Postoperative infectious complications remain a clinically relevant concern after flexible ureteroscopy (FURS), including in patients with negative preoperative bladder urine cultures (UCs). Microbiological discordance between bladder and upper urinary tract samples may contribute to under-recognition of infectious risk. We assessed the clinical value of systematic intraoperative UCs obtained at predefined stages of FURS.
METHODS: We relied on a prospectively collected dataset of 238 consecutive FURS procedures for renal stone disease conducted by a single experienced surgeon pe

Biliary stones represent the most common indication for therapeutic endoscopic retrograde cholangiopancreatography. Many cases are successfully managed with biliary sphincterotomy and stone extraction with balloon or basket catheters. However, more complex conditions secondary to the specific features of stones, the biliary tract, or patient's needs could make the stone extraction with the standard techniques difficult. Traditionally, mechanical lithotripsy with baskets has been reported as a safe and effective technique to achieve stone clearance. More recently, the increasing use of endoscop

BACKGROUND & AIMS: Although single-operator cholangioscopy (SOC)-guided lithotripsy and large balloon sphincteroplasty (LBS)-based techniques are effective rescue measures, the ideal approach to management of difficult bile duct stones is unclear. We conducted a randomized trial to compare the effectiveness of SOC-guided lithotripsy and LBS-based approaches for endoscopic management of difficult bile duct stones.
METHODS: Patients with difficult bile duct stones who failed retrieval using balloon or basket were randomly assigned to groups that received SOC-guided laser lithotripsy (SOC-LL, n

PURPOSE: There is a trend toward miniaturization in endourological stone therapy. Good visibility, intrarenal pressures and temperature control should be ensured by ureteral sheaths. In the context of the present study, 10/12 Charr. sheaths and 12/14 Charr. sheaths for flexible ureterorenoscopy were investigated regarding stone-free rate, complication rate and efficacy for laser lithotripsy.
METHODS: From January 2020 to January 2022, 100 patients each with kidney stone up to 1.5 cm in diameter were included in the study. Use of a 12/14 Charr. vs. 10/12 Charr. ureteral sheath for flexible ure

Since the publication of the latest recommendations of the Lithiasis Comity of the French Association of Urology (CLAFU) on the management of ureteral and renal stones, practices have evolved. This text provides an update for the treatment of kidney stones. It is especially the important advances in the field of laser-ureterorenoscopy that changed practices. Percutaneous nephrolithotomy has been enriched by technical modifications and extracorporeal shockwave lithotripsy confirmed its predominant place in the first line of treatment. For stones less than 20mm, extracorporeal lithotripsy remain

Since endoscopic papillotomy and extraction of stones out of the bile duct have become established procedures, the therapeutic approach to choledochlithiasis has fundamentally changed. 80 to 90% of all stones can be extracted by this method. New techniques can be applied for larger stones or for anatomical variations of the common bile duct. Mechanical lithotripsy, a technique by which further 80 to 90% of the remaining stones can be extracted, has been perfected for clinical use. In addition, laser lithotripsy, guided by choledochoscopy, is an elegant alternative for treating large solitary c

INTRODUCTION: The aim of the study was to assess the efficacy and safety of ureterolithotripsy (URS) in treating ureteral calculi with holmium-YAG laser through adding retropulsion prevention and drainage function to ureteral catheter.
METHODS: An inner wire was fixed at the top of an Fr5 ureteral catheter and run through a tee joint. The proximal catheter was split into 4 strips. When the wire was pulled, the strips become arcuate, thus trapping the stone. The end of the tee branch was connected to the suction evacuation. Continuous irrigation and negative pressure suction were delivered aft

BACKGROUND: Peroral cholangioscopy (POCS) can be useful for difficult bile duct stone clearance. Large prospective multinational data on POCS-guided lithotripsy for clearing difficult bile duct stones in a single session of endoscopic retrograde cholangiopancreatography (ERCP) are missing.
METHODS: Patients with difficult bile duct stones (defined as one or more of: largest stone diameter ≥ 15 mm, failed prior attempt at stone clearance, impacted, multiple, hepatic duct location, or located above a stricture) were enrolled at 17 centers in 10 countries. The principal endpoint was stone cleara

Cholelithiasis is a common condition, and complications of gallstone disease include cholecystitis, cholangitis, pancreatitis, and, rarely, gallstone ileus or Bouveret syndrome. We present a case of a 79-year-old male with multiple comorbidities who was treated using endoscopic techniques to avoid the risks associated with surgery. The approach utilized holmium laser lithotripsy and electrohydraulic lithotripsy over multiple sessions to fragment and remove the large gallstone causing a duodenal obstruction. Conservative management of a subsequent duodenal perforation led to successful recovery
