Interstitial magnetic resonance lymphography: is it a new method for the diagnosis of lymphedema?

AIM: The aim of this study was to evaluate the method of interstitial magnetic resonance lymphography (MRL) as an examination for the depiction of the lymphatic system in humans in comparison with the method of direct X-ray lymphography. METHODS: We studied 6 persons, 2 volunteers and 4 patients with clinical suspicion of lymphedema in lower extremities. We administered subcutaneous gadobutrol for the MRL with a volume of 5 mL composed of 4.5 mL of Gadobutrol mixed with 0.5 mL of lidocaine hydrochloride and after 7 days lipiodol in the lymph vessel for the X-ray direct lymphography (in 3 pati

Assessment of leg oedema by dynamic lymphoscintigraphy with intradermal injection of technetium-99m human serum albumin and load produced by standing.

This study was a preliminary evaluation of the utility of dynamic lymphoscintigraphy with technetium-99m human serum albumin (HSA) and a load produced by standing in the assessment of lymphatic dysfunction in patients with leg oedema. The 71 subjects investigated included 53 patients with lymphoedema, six with venous occlusion alone and five with lymphovenous occlusion, as well as seven normal subjects. After intradermal injection of 99mTc-HSA into an interdigital space in each foot, dynamic scintigrams were recorded with the patient supine for 15 min. The subjects then stood in place and imag

Wrist arthrography.

Wrist arthrography can be helpful in the evaluation of the chronically painful wrist and, more specifically, in visualization of the integrity of the triangular fibrocartilage and interosseous ligaments. To be meaningful, arthrographic findings, i.e., compartment communication, synovial irregularity, tendon sheath and lymphatic visualization, loose bodies, and cartilage abnormalities, must be correlated with clinical history and physical examination.

Lower Extremity Lymphatic Function in Nonambulatory Patients with Neuromuscular Disease.

Background: Lymphedema results from inadequate lymphatic function causing swelling in subcutaneous tissues. Lymph is transported proximally through valved lymphatic channels and muscle contraction. The purpose of this study was to determine lymphatic function in nonambulatory patients with lower extremity neuromuscular disease. Methods and Results: Our Lymphedema Program database of 700 patients was reviewed for nonambulatory patients with lower extremity neuromuscular disease. Patient age, gender, disease, body mass index (BMI), and lymphoscintigram result were recorded. Eight patients were i

Lymphangiomatosis diagnosed using transbronchial lung cryobiopsy: A case report.

Lymphangiomatosis, a rare systemic condition characterized by abnormally dilated lymphatic structures affecting multiple organs, is typically diagnosed using surgical lung biopsy or lymphangiography, both of which present significant technical challenges. Here, we report the first case of lymphangiomatosis diagnosed using transbronchial lung cryobiopsy (TBLC) in a 20-year-old asymptomatic man. The medical history of the patient was unremarkable, and chest computed tomography revealed diffuse interstitial thickening, ground-glass opacities, and pericardial effusion. Abdominal imaging indicated

Internal mammary lymphoscintigraphy in the conservative management of breast carcinoma: an update and recommendations for a new TNM staging.

The results of internal mammary lymphoscintigraphy (IML) in 524 patients with breast carcinoma who underwent partial mastectomy, with or without axillary dissection, demonstrate the predictive value of an abnormal lymphoscintigram. There was a significant difference (P less than 0.0005) in actuarial survival between patients with normal and abnormal IML. In view of the increased mortality associated with metastases to both internal mammary and axillary lymphatics compared with involvement of either site alone, and the most favourable outcome in patients with involvement of neither site, it is

Clinical value of MRI in central lymphatic vessel evaluation and grading in primary lower-extremity lymphedema.

Introduction: The manifestations of the central lymphatic vessels in primary lower-extremity lymphedema (LEL) remain incompletely understood. Advanced magnetic resonance imaging (MRI) techniques offer the potential to noninvasively evaluate central lymphatic vessels and their relationship with disease severity. Our objective was to investigate the differences in imaging performance of central lymphatics and inguinal lymph nodes in primary LEL of different clinical stages, and to assess the ability of MRI to grade primary LEL. Methods: Patients were staged according to the International Society

Lymphatic Function in the Arms of Breast Cancer Patients-A Prospective Cohort Study.

BACKGROUND: Lymphedema is a highly feared complication of breast cancer treatment, but the underlying complex mechanisms are still unknown. Thus, we investigated the lymphatic morphology and contractility in the lymphatic vessels of arms of high-risk breast cancer patients treated for node-positive early breast cancer. METHODS: In this prospective cohort study 32 women treated for unilateral node-positive breast cancer were enrolled and studied 36 ± 23 days after loco-regional radiotherapy. Near-infrared fluorescence imaging was used to assess morphology and function of the superficial lympha

Receptor Activity Modifying Protein RAMP Sub-Isoforms and Their Functional Differentiation, Which Regulates Functional Diversity of Adrenomedullin.

AM knockout (AM-/-) and RAMP2 knockout (RAMP2-/-) proved lethal for mice due to impaired embryonic vascular development. Although most vascular endothelial cell-specific RAMP2 knockout (E-RAMP2-/-) mice also died during the perinatal period, a few E-RAMP2-/- mice reached adulthood. Adult E-RAMP2-/- mice developed spontaneous organ damage associated with vascular injury. In contrast, adult RAMP3 knockout (RAMP3-/-) mice showed exacerbated postoperative lymphedema with abnormal lymphatic drainage. Thus, RAMP2 is essential for vascular development and homeostasis and RAMP3 is essential for lympha

Fatal systemic angiomatosis with widespread sclerotic skeletal changes, diagnosed with the aid of a bone marrow biopsy: the lymphatics enter the bone marrow.

A 56-year-old female presented with popliteal venous thrombosis, splenomegaly, and sclerotic bone lesions. Bone marrow biopsy showed fibrosis, proliferation of abnormal blood vessels and lymphatics, bone remodelling, and no significant changes in haematopoietic elements. Following a relatively indolent initial clinical course, one year later she rapidly deteriorated and died of respiratory failure associated with widespread disease. Ingrowth of podoplanin+ lymphatics mixed with CD34+/podoplanin- blood vessels into the bone marrow is a new finding and may be a unique feature of this disease.

Massive hemangioma or lymphedema? A case with a diagnostic dilemma.

The authors present an unusual case of massive enlargement of the right lower extremity that was treated as lymphedema secondary to filarial infection. A multiple-method approach beginning with scintigraphy led to the diagnosis of massive hemangioma with probable associated lymphatic proliferation. Abnormal local proliferation of vascular and lymphatic tissue is a developmental anomaly that is difficult to diagnose or characterize, even with histopathologic data. Individual imaging methods may also yield nonspecific findings. Blood-pool scintigraphy, therefore, has a definitive role to play in

Scintilymphangiography with 99 mTc-antimony sulfide colloid in hereditary lymphedema (Nonne-Milroy diseases).

A family of four patients suffering from Nonne-Milroy disease has been examined by pedal scintilymphangiography. Lower limb edema was present in three patients and was bilateral in two instances. Scintilymphangiographic abnormality was invariably present, ranging from almost total obstruction of lymphatic flow to lymphatic pooling. The inguinal, iliac and para-aortic nodes on the affected sides were also noted to be small and few in number. The examination is safe and simple and may be used to distinguish Nonne-Milroy edema from edema due to other causes.

Lymphangiogenesis and Lymphatic Zippering in Skin Associated with the Progression of Lymphedema.

Secondary lymphedema often develops after lymph node dissection or radiation therapy for cancer treatment, resulting in marked skin fibrosis and increased stiffness owing to insufficiency of the lymphatic system caused by abnormal structure and compromised function. However, little is known about the associated changes of the dermal lymphatic vessels. In this study, using the lower limb skin samples of patients with secondary lymphedema, classified as types 1-4 by lymphoscintigraphy, we first confirmed the presence of epidermal thickening and collagen accumulation in the dermis, closely associ

Profound but dysfunctional lymphangiogenesis via vascular endothelial growth factor ligands from CD11b+ macrophages in advanced ovarian cancer.

Severe ascites is a hallmark of advanced ovarian cancer (OVCA), yet the underlying mechanism that creates an imbalance between peritoneal vascular leakage and lymphatic drainage is unknown. Here, we identified and characterized peritoneal lymphatic vessels in OVCA mice, a model generated by implantation of human OVCA cells into athymic nude mice. The OVCA mice displayed substantial lymphangiogenesis and lymphatic remodeling, massive infiltration of CD11b(+)/LYVE-1(+) macrophages and disseminated carcinomatosis in the mesentery and diaphragm, and progressive chylous ascites formation. Functiona

Topical sirolimus for the treatment of cutaneous manifestations of vascular anomalies: A case series.

BACKGROUND: Vascular anomalies (VA), characterized by the abnormal development or growth of blood and/or lymphatic vessels, encompasses a spectrum of conditions with a range of symptoms and complications. VA are frequently associated with cutaneous complications that can cause significant morbidity. Systemic sirolimus has previously been shown to be effective in the treatment of complicated VA. There are limited studies to date on the use of topical sirolimus for the treatment of cutaneous manifestations of VA. METHODS: Retrospective review of medical records of pediatric patients with VA tre

[Yellow nail syndrome. Apropos of 2 cases. Review of the literature].

The yellow nail syndrome is a clinical syndrome without specific biological parameters. It is characterized by the following threesome of symptoms: nail abnormalities, primary lymphedema, and pleuropulmonary manifestations. In most cases lymphography shows abnormalities of the lymphatic vessels and spirometry shows a restrictive defect and rarely obstructive disorders. Pathogenesis of this syndrome is still unknown. It is, however, remarkable that the yellow nail syndrome may occur in association with other diseases, the most frequent of which are cancer affections and immune disorders. We rep

Kaposiform lymphangiomatosis-the effects of long-term treatment with sirolimus: case series study and review of the literature.

INTRODUCTION: Kaposiform lymphangiomatosis (KLA) is an ultrarare disease characterized by abnormal lymphatic vessel proliferation due to hyperactivation of the Rat sarcoma virus (RAS) signaling pathway, resulting in multifocal lymphatic malformations (LMs) that can potentially involve multiple organ systems. Its distinctive histologic features include the presence of clusters of spindle cells expressing clusters of differentiation 31 (CD31), cluster of differentiation 34 (CD34), podoplanin (D2-40), prospero homeobox protein 1 (PROX1), and lymphatic vessel endothelial hyaluronan receptor 1 (LYV

Clearance mechanisms of the glymphatic/lymphatic system in the brain: new therapeutic perspectives for cognitive impairment.

The glymphatic/lymphatic system of the brain has been an important discovery in the field of neuroscience in recent years. As the "waste clearance network" of the central nervous system, it clears metabolic products and neurotoxic substances through the cerebrospinal fluid-interstitial fluid circulation, which is crucial for maintaining the homeostasis of the intracerebral environment and plays important roles in learning, memory and other advanced cognitive functions. The glymphatic/lymphatic system is crucial for the clearance of beta-amyloid and tau proteins, and thus the abnormal function
