Red cell hypoplasia and monoclonal gammopathy in a patient with lymphoproliferative disorder.

A 69-year-old male was observed to have red cell hypoplasia. Two years later monoclonal gammopathy IgG-I, K, Gma+ and InV (1-) was documented in this patient. Persistent lymphocytosis, abnormal response to phytohemagglutinin, and at autopsy multiple lymphoid nodules occurring in the bone marrow, suggestive of lymphoproliferative disorder, were observed. A review of the literature indicates that this clinical presentation is rare. The possibility that red cell aplasia may be associated with lymphoproliferative disorder in some instances must be considered inasmuch as this may have an important

Progressive multifocal leukoencephalopathy as the first presentation of an underlying lymphoproliferative disorder.

Progressive multifocal leukoencephalopathy (PML) is a demyelinating central nervous system disease caused by John Cunningham virus (JCV) reactivation in immunosuppressed patients. Although classically associated with advanced HIV infection, PML also occurs in patients with hematological malignancies, organ transplants, or on immunomodulatory therapies. The mortality rate can be high, ranging from 20 to 90%, depending on the underlying condition. Clinical presentation is variable, depending on the site of brain involvement, and diagnosis requires integration of clinical, radiological, pathologi

Early Posttransplant Lymphoproliferative Disorder and Cryptosporidiosis After Liver Transplantation.

Posttransplant lymphoproliferative disorder (PTLD) is a rare yet potentially fatal complication following liver transplantation. This condition is usually associated with Epstein-Barr virus (EBV) infection and profound immunosuppression. In this report, a young woman is introduced who had undergone liver transplantation due to autoimmune hepatitis and developed persistent diarrhea 3 months after the transplantation. Simultaneously, Cryptosporidium infection was diagnosed as the cause of the diarrhea. However, due to the persistence of symptoms and lack of full recovery with initial treatment,

Clinical Analysis of Posttransplant Lymphoproliferative Disorder After Liver Transplant.

OBJECTIVES: Posttransplant lymphoproliferative disorder is a severe and potentially fatal complication after liver transplant, resulting from immunosuppression-driven uncontrolled lymphoid proliferation. In this study, we aimed to analyze the incidence, clinico-pathological characteristics, management, and outco-mes of posttransplant lymphoproliferative disorder in liver transplant recipients. MATERIALS AND METHODS: We conducted a retrospective analysis of 1288 patients who underwent liver transplant between January 2015 and December 2024. Among them, 8 recipients (0.62%) were diagnosed with

From Rhabdomyolysis to a Lymphoproliferative Disorder: A Long Diagnostic Work-Up.

Rhabdomyolysis is characterized by myocyte necrosis with destruction of the skeletal muscle that can lead to severe complications if left untreated. Due to its multiple possible etiologies, identifying the underlying cause can be challenging, especially when the clinical presentation is atypical or evolves. We present the case of a 73-year-old man who developed progressive proximal muscle weakness shortly after COVID-19 vaccination. Laboratory tests were consistent with rhabdomyolysis. Despite supportive treatment and clinical response to corticosteroids, the underlying cause remained initiall

Risk factors for tonsillar hypertrophy and post-transplant lymphoproliferative disorder.

INTRODUCTION: Post-transplant lymphoproliferative disorder (PTLD) is a rare complication of immunosuppression in solid organ transplant patients. Tonsillar hypertrophy commonly results from PTLD, and tonsillectomy is usually performed when PTLD is the suspected cause. Risk factors for PTLD-mediated tonsillar hypertrophy remain poorly defined. This study aimed to identify clinical and therapeutic factors associated with tonsillar hypertrophy following PTLD diagnosis. METHODS: We conducted a retrospective cohort study using the TriNetX Database. Pediatric solid organ transplant recipients were

Oncogenic alterations in KIR3DL1 in cutaneous acral CD8+ lymphoproliferative disorder.

BACKGROUND: Primary cutaneous acral CD8+ T-cell lymphoproliferative disorder (TLPD) is a rare and indolent lymphoma entity. Although TLPD was first identified many years ago, the molecular pathogenesis is still not fully understood. OBJECTIVES: In order to better understand the molecular pathogenesis of cutaneous acral CD8+ TLPD and to identify further discriminatory markers to differentiate this lymphoma subtype from other CD8+ cutaneous lymphomas, we analysed five cases of cutaneous acral CD8+ TLPD for putative molecular alterations. METHODS: Somatic alterations were assessed using whole-e

Lymphoproliferative disorder risk after methotrexate treatment for rheumatoid arthritis.

Methotrexate (MTX)-associated lymphoproliferative disorder (MTX-LPD) is a troublesome problem in patients receiving MTX for rheumatoid arthritis (RA). However, its incidence, prognosis, and risk factors remain unclear. In this retrospective study, we evaluated the actual incidence, prognostic impact, and risk factors of MTX-LPD. Of the 986 patients with RA treated with MTX, 90 patients experienced 95 new malignancies (NMs), with LPD as the most frequent in 26 patients. The cumulative LPD incidences were 1.3% and 4.7% at 5 and 10 years after MTX initiation, respectively. Among the 24 patients w

Case Report of a Novel NFkB Mutation in a Lymphoproliferative Disorder Patient.

BACKGROUND: Lymphoproliferative disorders include a heterogeneous list of conditions that commonly involve dysregulation of lymphocyte proliferation resulting in lymphadenopathy and bone marrow infiltration. These disorders have various presentations, most notably autoimmune manifestations, organomegaly, lymphadenopathy, dysgammaglobulinemia, and increased risk of chronic infections. CASE PRESENTATION: A young boy presented with symptoms overlapping different lymphoproliferative disorders, including episodes of chronic respiratory tract infections, dysgammaglobulinemia, lymphadenopathy-associ…

Post-Liver Transplant Lymphoproliferative Disorder Presenting as Biliary Stricture.

A 63-year-old man with nonalcoholic steatohepatitis cirrhosis who underwent orthotopic liver transplant presented 1 year later with obstructive jaundice because of a biliary stricture. This anastomotic stricture was initially believed to be ischemic, but further investigation revealed malignant biliary obstruction because of encasement of the bile duct by a mass arising from liver segment VII, later determined to be post-transplant lymphoproliferative disorder with widespread metastasis. After reduction of immunosuppression and systemic chemotherapy, he experienced complete remission. This cas

Transbronchial cryobiopsy: an effective tool in the diagnosis of lymphoproliferative disorders of the lung.

INTRODUCTION: Malignant lymphoproliferative disorders are rarely observed in the lung and, considering their clinical and radiological heterogeneity, diagnosis is often difficult and may require invasive methods. Transbronchial cryobiopsy has been confirmed as a new tool in the diagnosis of interstitial lung diseases, given its fewer risks and costs compared to surgical approach. This study is aimed at assessing the effectiveness of cryobiopsy in the diagnosis of lymphoproliferative disorders. MATERIALS AND METHODS: Among 970 consecutive cryobiopsies, performed between January 2011 and June 2…

A case of esophageal cancer complicated with methotrexate-related lymphoproliferative disorder.

A 69-year-old woman had been undergoing treatment with methotrexate for rheumatoid arthritis for 9 years. Because fever and right hypochondriac pain continued, she visited the nearby hospital. The enlargement of intraabdominal multiple lymph nodes was detected through abdominal computed tomography. Esophagogastroduodenoscopy showed a depressed lesion in the lower intrathoracic esophagus, and squamous cell carcinoma was diagnosed by a biopsy. Positron emission tomography-computed tomography showed a highly abnormal accumulation of lymph nodes, mainly in the upper abdomen, and some lymph nodes a

Progressive multifocal leukoencephalopathy associated with a lymphoproliferative disorder treated with pembrolizumab.

Progressive multifocal leukoencephalopathy (PML) is a demyelinating disease affecting the central nervous system as a result of reactivation of the John Cunningham (JC) polyomavirus and occurs almost exclusively in immunosuppressed individuals. The disease course of PML is variable but usually progressive and often fatal. Treatment is predominantly focused on immune restoration, although this is difficult to do outside of human immunodeficiency virus-associated PML. A recent case series demonstrated a potential role for programmed cell death protein 1 (PD-1) inhibitors, such as pembrolizumab,

Methotrexate-associated lymphoproliferative disorder in the stomach and duodenum: a case report.

BACKGROUND: Methotrexate-associated lymphoproliferative disorder (MTX-LPD) can present as a benign lymphoid proliferation or a malignant lymphoma in patients taking MTX. Almost 50% of MTX-LPD cases show spontaneous remission after withdrawal of MTX treatment. Studies have suggested that the hyper-immune state of rheumatoid arthritis, the immunosuppressive state associated with MTX, and the carcinogenicity of the Epstein-Barr virus might contribute to MTX-LPD development. Although most cases of MTX-LPD occur at extranodal sites, few cases of MTX-LPD affecting the stomach and duodenum have been
