The Role of Imaging in Mandibular Reconstruction with Microvascular Surgery.

Imaging plays a critical role in the diagnosis, staging, and management of segmental mandibular defects. Imaging allows mandibular defects to be classified which aids in microvascular free flap reconstruction. This review serves to complement the surgeon's clinical experience with image-based examples of mandibular pathology, defect classification systems, reconstruction options, treatment complications, and Virtual Surgical Planning.

Long-Term Update: Free Fibula Flap Growth After Pediatric Mandibular Reconstruction.

A free fibular flap is commonly used in adult mandibular reconstruction; however, its use in the pediatric population is not strongly supported. The authors are reporting the long-term update of a case of a pediatric patient who underwent a mandibular reconstruction using a free fibular flap after a resection of mandibular desmoid fibromatosis. Greatest growth was objectively measured and demonstrated at the condyle using a 3-dimensional model generated from Materialise software. This is 1 case and subsequent studies should be observed to further elucidate the full growth potential of the mand

An osteochondroma of the mandibular condyle.

OBJECTIVE: Osteochondroma is rarely seen in the facial region, especially around the condyle. Here, we report a case of condylar osteochondroma, aiming to assist the diagnosis and treatment of the tumor. METHODS: A case of osteochondroma of the left mandibular condyle in a 49-year-old man was presented. Medical records with x-ray, computed tomographic scan, and bone scan of histologically proven osteochondroma of mandibular condyle were obtained. RESULTS: The patient underwent a surgical resection and had fewer functional changes as well as less dysfunction of the temporomandibular joint. C

Silicone implant for chin augmentation mimicking a low-grade liposarcoma.

The silicone implant has been widely used in facial aesthetic surgery for chin augmentation. A case of a 71-year-old woman with a lesion in the anterior mandibular region detected by radiographies and suggestive of benign soft tissue tumor is presented. The patient denied previous surgical intervention in the mental area. A biopsy was performed and initially interpreted as low-grade liposarcoma. After an immunohistochemistry study showing CD-68-positive cells and complementary information regarding a silicone implant for chin augmentation, the definitive diagnosis was silicone granuloma. This

Mandibular African histoplasmosis: imitation of neoplasia or giant-cell granuloma?

The lesion reported in this article destroyed much of the right side of the mandible in a 14-year-old boy. Clinically, it resembled Burkitt's lymphoma and was initially treated as such. After correct microscopic diagnosis, the mass regressed with the use of intravenous amphotericin B and surgical reduction. The histologic comparison with giant-cell granuloma is considered.

Intraosseous benign peripheral nerve sheath tumor of the jaws: report of 4 new cases and a comprehensive literature review.

Benign peripheral nerve sheath tumors (BPNSTs) are a vast, common, heterogeneous group of lesions that are often diagnostically challenging. The head and neck region is recognized as the second most common location for these lesions, especially the tongue. However, BPNSTs occurring centrally within the jaw bones are exceedingly rare. To date, approximately 161 cases of intraosseous BPNSTs have been reported in the jaws. This group mainly includes the relatively more common neurofibromas and schwannomas, and the less common perineuriomas and hybrid forms. Unlike soft tissues, schwannomas are th

Computed tomography evaluation of alterations in the masticator space due to invasion by malignant head and neck neoplasms.

OBJECTIVE: To evaluate alterations in the masticator space due to the dissemination of malignant neoplasms originating from the tonsillar fossa, retromolar trigone, maxillary sinus, or nasopharynx, using computed tomography (CT), as well as to correlate the presence of trismus with the CT findings and the dimensions of the tumor. MATERIALS AND METHODS: We evaluated the medical records of 65 patients with malignant tumors in the regions described. The images were analyzed by two physician examiners, working independently, who were blinded to the clinical data. In the evaluation of the masticat

Serendipitous diagnosis of protein S deficiency.

A 46-year-old male sought periodontal care for a swelling on his right mandibular gingiva. An excisional biopsy revealed a well-differentiated squamous cell carcinoma. Surgical treatment consisted of a right segmental mandibulectomy with ipsilateral right neck dissection and fibular free flap reconstruction. Two days after the surgical procedure, a weakened Doppler signal suggested vascular compromise of the graft. The patient was returned to the operating room where complete thrombosis of the internal jugular vein (recipient vessel) was observed. This event prompted a complete hematological e

A Cost-Effective Strategy for Mandibular Reconstruction: Fibula Flap With Existing Titanium Plates.

With the rapid advancement of digital and microsurgical techniques, immediate autologous bone grafting following resection of benign and malignant mandibular tumors has become a standard treatment. However, some patients are unable to undergo immediate autologous bone grafting due to various reasons. In such cases, titanium plate implantation (or combined with soft tissue free flap such as anterolateral thigh flap) serves as an alternative to prevent airway compression and maintain facial shape and function. This article reports a case of a patient, referred to as patient 1, who experienced a

Metastatic Intestinal Adenocarcinoma: A Rare Case Involving the Mandible.

BACKGROUND: Metastatic intestinal adenocarcinoma involving the mandible is rare, posing diagnostic challenges because of its unusual presentation. CASE PRESENTATION: A 55-year-old male presented with a rapidly growing mass in the right mandible, accompanied by facial asymmetry and vestibular obliteration. Histopathological examinations revealed features consistent with adenocarcinoma. Immunohistochemical analysis supported the diagnosis of intestinal adenocarcinoma, with subsequent metastasis confirmed by PET scan findings. DIAGNOSIS: The lesion was conclusively diagnosed as intestinal adeno

[Adenomatoid odontogenic tumor: about two cases].

Adenomatoid odontogenic tumor is a benign epithelial tumor which mainly affects young women. It usually occurs in the anterior portion of the upper jaw. Diagnosis can be clinically suspected in patients with cyst formation associated with retained tooth but histopathological confirmation is required. This study involved two patients aged 13 and 37 years, with no previous history, presenting after the occurrence of a swelling on the jaw and mandible. Anatomopathological examination of these lesions showed adenomatoid odontogenic tumor. This study highlights the anatomoclinical features, outcome

The use of radiation therapy in the definitive management of ameloblastic carcinoma: a case report.

Although rare, ameloblastic carcinoma (AC) is one of the most common malignant tumors occurring in the odontogenic epithelium. Evidenced-based medical guidelines for AC are not established because AC is a rare type of cancer. However, surgery for AC is the primary initial treatment modality. A few reports suggest that radiotherapy (RTx) is effective for AC; however, the tumor is generally considered to be radioresistant. Furthermore, if RTx is performed for AC, osteonecrosis may occur because most of the ACs arise from the jaw. Here, we report a case of an older patient with maxillary AC who h

Aggressive scapular chondroblastoma with secondary metastasis--a case report and review of literature.

Chondroblastoma is a benign bone tumor, accounting for approximately one percent of all benign bone tumors. It mostly occurs in typical locations such as long bones. Malignant transformation including metastasis has been described in only a few cases. Therefore, we report a unique case of chondroblastoma with tumor manifestation in the 7th decade of life, location of the tumor in the scapula and occurrence of metastasis in the soft tissue of the mandible branch. Due to aggravation of the clinical course, a scapula en bloc resection was performed. The differential diagnosis is discussed and the

Benign odontogenic tumors of the mandible and maxilla.

Benign odontogenic tumors are characterized by imaging findings of expansile growth and well-defined margins with smooth borders, and their appearance is very similar to that of odontogenic and nonodontogenic cysts. From the viewpoint of diagnostic imaging of odontogenic tumors, teeth are designed differently according to their origin from the apex or crown. Therefore, for differential diagnosis, it is necessary to select diagnostic methods that make it possible to evaluate these findings in detail. Intraoral and panoramic radiography are highly effective in examining the relationship between
