Reconstruction of mandibular defects following tumor ablation by vertical distraction osteogenesis using intraosseous distraction devices.

OBJECTIVES: Distraction osteogenesis has recently evolved a challenging technique to overcome major drawbacks of conventional augmentation procedures. We, therefore, report the application of miniaturized intraosseous distraction devices for the rehabilitation of mandibular defects due to ablative tumor surgery. METHODS: In 10 patients who had undergone box-shaped or segmental resections, augmentation of the residual mandibular bone or of full thickness iliac crest grafts was performed by intraosseous implant-shaped distractors. Distraction and reconsolidation was monitored by ultrasonography

Reconstruction of residual alveolar cleft defects with one-stage mandibular bone grafts and osseointegrated implants.

PURPOSE: This study evaluates a treatment regimen for reconstruction of residual maxillary alveolar cleft defects consisting of mandibular bone grafting and immediate implant installation. PATIENTS AND METHODS: Sixteen cleft patients (five female and 11 male) had residual cleft defects of the alveolar ridge reconstructed with bone grafts from the mandibular symphyseal region. The bone graft was pretapped at the donor site before fixation in the residual ridge with Brånemark implants. Twenty implants were installed according to this concept. The period of observation ranged from 36 to 69 month

Combined V figure-shaped scapular osteocutaneous and latissimus dorsi myocutaneous flap for composite mandibular reconstruction.

BACKGROUND: As early as 1981, the scapular crest was reported as a versatile donor site of vascularized bone flap. In our institute, much attention has been paid to applying a compound flap based on the subscapular vascular system for composite mandibular reconstruction. METHODS: The combined V figure-shaped scapular osteocutaneous and latissimus dorsi myocutaneous flap has been used for primary or secondary reconstruction of the mandible, intraoral mucosa and/or external skin following major ablation of the malignant tumors in seven patients. RESULTS: There was one total flap failure. Satis…

Mandibular Reconstruction after Resection of Ameloblastoma by Custom-Made CAD/CAM Mandibular Titanium Prosthesis: Two Case Reports, Finite Element Analysis and Discussion of the Technique.

Virtual surgical planning for CAD/CAM mandibular reconstruction by titanium prosthesis was recently reported for resected cases. Even if some advantages are evident, difficulties that may arise for TMJ function after reconstruction originate from prosthesis contamination through oral mucosa dehiscence. In these two cases reported of mandibular reconstruction after resection of ameloblastoma by custom-made CAD/CAM titanium prosthesis, the procedures were aimed to preserve the TMJ glenoid cavity and articular disc avoiding functional problems for hemi-mandibular resections that included the cond…

[Secondary Maxillofacial Revision Reconstruction Using Custom-Made Artificial Bone Following Resection of Primary Intraosseous Mandibular Squamous Cell Carcinoma and Microvascularized Fibula Flap Reconstruction].

The basic treatment for oral cancer is radical tumor resection and reconstruction, which alters the maxillofacial morphology and causes dysfunction. Reconstructive surgery can be performed with bone and soft tissue transplantation, but it is invasive and good morphological repair is difficult. Custom-made artificial bone(CT-BoneTM)consisting of calcium-deficient hydroxyapatite was newly authorized for clinical use in April 2018. It enables precise, minimally invasive maxillofacial bone reconstruction. Here, we report favorable facial morphological improvement using CT-BoneTM in a patient with

Evaluation of Postoperative Facial Asymmetry Index Following Mandibular Reconstruction Using Free Fibular Flap With Condylar Involvement.

BACKGROUND: Loss of the temporomandibular joint (TMJ) following mandibulectomy disrupts maxillomandibular continuity and occlusion, potentially resulting in facial asymmetry with functional, structural, and aesthetic consequences. The free fibular flap remains the gold standard for mandibular reconstruction. However, the impact of condylar involvement on postoperative facial asymmetry remains poorly studied. METHODS: A retrospective cohort study was conducted on patients undergoing free fibular flap mandibular reconstruction at Cipto Mangunkusumo Hospital (RSCM) between 2023 and 2025. Subject…

A Novel 3D-Printed Titanium Prosthesis with Osseointegrated Dental Implants for Reconstruction of Segmental Mandibular Defects.

INTRODUCTION: Segmental mandibular defects present complex reconstructive challenges and are associated with significant functional and aesthetic morbidity. While vascularized bone flaps remain the standard for reconstruction, they are associated with donor-site morbidity, prolonged operative times, and limitations in predictable dental rehabilitation. This proof-of-concept study evaluated the mechanical feasibility, in a synthetic bench-top model, of a patient-specific, 3D-printed titanium mandibular prosthesis incorporating conventional dental implants for both dental rehabilitation and biol

Long-term Evaluation of Mandibular Reconstruction with Fibular Flap in SAPHO Syndrome: 18 Years Post-onset.

Synovitis, acne, pustulosis, hyperostosis, and osteitis (SAPHO) syndrome is a systemic inflammatory disease that involves mandibular osteomyelitis with diffuse sclerosis in 10% of cases. Drug therapy is the primary treatment choice; however, severe deformity resulting in restricted mouth opening requires mandibular reconstruction. We report a rare case of mandible reconstruction in a patient with SAPHO syndrome, using a vascularized fibular flap with long-term follow-up. One year after surgery, a fistula developed in the chin region following a rash on both lower extremities as a skin manifest…

Implementation of Medicalholodeck® for Augmented Reality Surgical Navigation in Microsurgical Mandibular Reconstruction: Enhanced Vessel Identification.

Mandibular reconstruction with the fibula free flap is the gold standard for large defects, with virtual surgical planning becoming integral to the process. The localization and dissection of critical vessels, such as the recipient vessels in the neck and the perforating vessels of the fibula flap, are demanding steps that directly impact surgical success. Augmented reality (AR) offers a solution by overlaying three-dimensional virtual models directly onto the surgeon's view of the operative field. We report the first case in Latin America utilizing a low-cost, commercially available holograph…

Model experiments on application of oral and maxillofacial surgical robot-assisted mandibular tumor resection and reconstruction.

BACKGROUND: Mandibular tumors significantly impact patient health and quality of life. Surgical resection is the primary treatment, often necessitating reconstruction to restore appearance and function. Traditional surgical methods rely heavily on surgeon experience, posing risks such as excessive blood loss and facial paralysis. The advent of digital and robotic-assisted surgical technologies offers improved precision and outcomes. METHODS: The study included five pairs of models with mandibular tumors, divided into a control group (traditional surgery) and an experimental group (robot-assis

Oral Rehabilitation With Clear Aligners and Implant-Supported Prosthetic Restorations After Mandibular Reconstruction Due to Osteosarcoma.

This case report presents the complex orthodontic and prosthetic rehabilitation process of a 30-year-old man who presented to our clinic seeking to restore chewing function and aesthetics following radical surgical resections for mandibular osteosarcoma. His oncological history included a tumor infiltrating the right mandibular ramus and corpus. Archived radiographic data indicated that this lesion was locally invasive and exhibited an aggressive bone destruction pattern, so the patient initially underwent a segmental mandibulectomy. However, due to positive surgical margins, subsequent extens

Data-Driven Design and Additive Manufacturing of Patient-Specific Lattice Titanium Scaffolds for Mandibular Bone Reconstruction.

The reconstruction of segmental bone defects requires patient-specific scaffolds that combine mechanical safety, biological functionality, and rapid manufacturing. We converted CT-derived mandibular geometry into a functionally graded Ti-6Al-4V lattice and optimised porosity, screw layout, and strut thickness through a cyber-physical loop that joins high-fidelity FEM, millisecond ANN, and a BN for uncertainty quantification. Fifteen candidate scaffolds were fabricated by direct metal laser sintering and hot isostatic pressing and were mechanically tested. FEM predicted stress and stiffness wit

Vascularized fascial flap for reconstruction of combined oral mucosa-mandibular defects: The multi-modal biological assessment.

OBJECTIVE: The reconstruction of composite defects in the oral and maxillofacial region using vascularized fascial flaps, such as the fibular, iliac, and temporal fascial flaps, has gained increasing attention among surgeons. However, there remains uncertainty regarding the suitability of fascial flaps as transplants, as well as their healing processes and outcomes, due to their non-mucosal nature. This study aims to comprehensively assess the biological aspects of vascularized fascial flaps at clinical, histological, and genetic levels, with the goal of providing essential biological referenc

Functional Outcomes After the Sacrifice of Mandibular Condyle Using Fibula Free Flap for Immediate Surgical Reconstruction.

Introduction Head and neck cancer with mandibular invasion often necessitates composite resection, leading to defects requiring reconstruction. Microvascular fibula free flap (FFF) surgery is a common approach for this purpose. In this study, we focus on our experience with condyle sacrifice, emphasizing treatment outcomes and functional results. Additionally, we highlight a contemporary perspective by discussing surgical techniques and radiographic outcomes based on a 3D analysis of neo-condyle placement on CT imaging. Methods We studied 23 patients who had undergone segmental mandibulectomy

Osteosynthesis for mandibular reconstruction with fibula free flap: Which type and why? A French national survey.

We explored the most suitable osteosynthesis type for mandibular reconstruction using fibula free flap (FFF) given the lack of robust data available. For this, an anonymous survey was e-mailed to 853 head-and-neck surgeons (France) asking about their practices and criteria guiding their osteosynthesis material and method choices. Questions assessed: (1) the most frequently used osteosynthesis type, (2) the explanatory variables related to surgeon career status/surgical specialty, and (3) justifications for osteosynthesis type (scientific, technical, "misuse"). Multivariate logistic regression …

Towards improved functionality of mandibular reconstruction plates enabled by additively manufactured triply periodic minimal surface structures.

Additive manufacturing for fabrication of patient-specific oral and maxillofacial implants enables optimal fitting, significantly reducing surgery time and subsequent costs. However, it is still common to encounter hardware- or biological-related complications, specifically when radiation treatment is involved. For mandibular reconstruction plates, irradiated patients often experience plate loosening and subsequent plate exposure due to a decrease in the vascularity of the irradiated tissues. We hypothesize that an acceleration of the bone ingrowth prior to radiation treatment can increase the…

Central Mandibular Reconstruction by Semiopen Wedge Osteotomy Double-barrel Fibula Flap for a Slim Aesthetic Appearance.

Mandibular reconstruction involving the central segment after malignant tumor resection requires the achievement of both functional and aesthetic quality. Three-dimensional reproduction based on the concept of a double arc composed of a marginal arc and an occlusal arc is important. Most reports of fibula flaps applied three-segmented closed wedge fibula osteotomy (bilateral and central segments); however, the aesthetic outcome sometimes became too stout for female patients because of the large central segment. We performed four-segmented fibula osteotomy for a 78-year-old woman using a semiop…
