Cholesteatomas are keratinizing cystic lesions of the temporal bone, most commonly involving the middle ear and mastoid. Intracranial extension is rare and usually associated with acquired disease, while giant supratentorial presentations are exceptional. We report a well-documented and neuroradiologically illustrated case of an exceptionally large congenital mastoid cholesteatoma with supratentorial intracranial extension. A 49-year-old woman presented with left parietal bone softening and recent ipsilateral facial sensory disturbance. MRI revealed a giant left temporoparietooccipital extra-a…

BACKGROUND: Mastoid pneumatization reflects middle ear aeration and may be disrupted in chronic inflammatory conditions such as Antrochoanal polyps (AP), potentially predisposing children to otologic complications.
OBJECTIVE: We aimed to investigate the developmental relationship between AP volume and total mastoid air cell volume (TMCV).
METHODS: The correlation between the volume of nasal polyps and TMCV was assessed in 21 pediatric patients with AP. TMCV on the side not affected by AP served as the control within each patient. Polyp volume and TMCV were quantified using computed tomograph…

Pulsatile tinnitus is a rhythmic type of tinnitus. Most cases are caused by vascular disorders and present as synchronous with the patient's heartbeat. A dilated mastoid emissary vein may cause pulsatile tinnitus. Here, we report the first case of bilateral pulsatile tinnitus caused by dilated bilateral mastoid emissary vein. The patient suffered from bilateral pulsatile tinnitus for 2 years. His bilateral tinnitus disappeared temporarily when the bilateral mastoid processes were compressed. Temporal bone computed tomographic (CT) showed the bilateral dilated mastoid emissary canals. Cranial M…

We describe the use of calcium sulfate beads as antibiotic carrier in a patient, who suffered from chronic mastoiditis with consecutive otogenic meningitis due to Burkholderia cenocepacia. Our findings suggest a possible role of calcium sulfate matrix as a local antibiotic carrier in the mastoid in complicated mastoiditis cases.

We describe the case of a child with an isolated penetrating trauma to the mastoid tip. Nasal blowing consequently induced air bubbles coming through the mastoid cutaneous fistula and causing extensive subcutaneous neck emphysema. A computed tomography (CT) demonstrated a right mastoid tip bone fracture with extensive cervical subcutaneous emphysema. The patient was treated conservatively with antibiotics and did not require operative intervention. His subsequent course was uncomplicated. This case emphasizes the importance of taking seriously even what seems to be a minor skin laceration.

INTRODUCTION: Free anterolateral thigh (ALT) flap constitutes an alternative to latissimus dorsi and radial forearm flaps for head and neck reconstruction. The authors report a case of mastoid reconstruction in a patient with multi-operated cholesteatoma associated with tubal dysfunction.
CASE REPORT: This patient presented a fistula of the lateral semicircular canal and invagination of retroauricular soft tissues responsible for non-compensated almost complete areflexia with anacusis. She did not present any signs of cholesteatoma recurrence, but chronic inflammation of the mastoidectomy cav…

Epidermoid cysts of the temporal bone are extremely rare and such lesions arising in isolation within the mastoid bone have never been reported in literature. We report and describe the first two unique cases of primary epidermoid cysts arising in the mastoid bone. Of the two cases, one presented with progressive headache and imbalance and the other with unilateral hearing loss and tinnitus. Both cases needed CT and MRI scans and needed surgical management. We review the clinical presentations, histology, pathogenesis, radiological findings and management of these challenging cases. The diagno…

Histiocytosis X (Langerhans cells) is a rare disease with different forms of manifestation in the head and neck, which mostly occurs in children. We report the case of a 52-year-old male patient from Saudi Arabia who presented in the out-patient department of the University Hospital Charité Berlin with therapy-resistant otitis externa. ENT examination additionally showed nasal polyps. A paranasal computed tomography scan revealed as an incidental finding a mass forming tumor in both mastoids with osseous destruction of the posterior cranial fossa. Histological evaluation of the suspected tissu…

BACKGROUND: Vertigo during suction-cleaning of open mastoid cavities is caused by cold stimulation of the vestibular organ. In clinical and model-experimental measurements we tried to estimate the physical background and the dimension of this temperature drop.
METHODS: Using different techniques like thermo-probes, thermovision, or additional model experiments temperature changes during suction were measured in 5 volunteers with open mastoid cavities. These registrations were carried out with special regard to the moisture of the cavity-walls.
RESULTS: Distinct temperature changes during suc…

Five cases of schneiderian-type mucosal papillomas arising in the middle ear space are reported. The patients were all women, ranging in age from 19 to 57 years (median, 31 years). Clinical complaints--unilateral conductive hearing loss, pain, or otorrhea--ranged from those lasting several months to recurrent problems spanning 20 years. All of the patients had a history of chronic otitis media predating the development of the papillomas; none of the patients had a history of sinonasal or nasopharyngeal schneiderian-type papillomas. Clinically, three patients had intact tympanic membranes, whil

The early stages of Meniere's disease may consist only of vestibular symptoms. These patients with vertigo alone may be incapacitated by this condition. A great many of these patients are unresponsive to medical therapy and tend to progress to cochleovestibular disease. Early surgery on such individuals may decrease vertigo and give the added benefit of preserving hearing. This paper reports the results of endolymphatic mastoid shunts performed on 24 patients with normal hearing who had medically uncontrolled vestibular Meniere's disease. The patients underwent surgery between 1978 and 1981, a…

Hearing results are presented for 627 ears with chronic otitis media that were operated on radically and obliterated (Palva flap) and in which a tympanoplasty was performed. The ears were examined annually for five to 14 years (mean, 8.8 years). The long-term improvement (five to 14 years after the surgery) was the greatest in ears with an intact ossicular chain and in ears with ossicular reconstruction using autograft or homograft ossicles or autogenous cortical bone columellae. As a whole, the early (one year after the operation) improvement in the air-bone gap was 8.0 dB when compared with

Congenital cholesteatomas are defined as a collection of epithelium in the absence of prior surgery or pathologic retraction. They are most commonly found in the middle ear and are thought to arise from a residual epithelial rest present since birth; however, a small number of cases present with disease isolated to the mastoid bone. One such case and a review of prior reports are presented. A 29-year-old male with a 2-year history of headaches and no otologic surgery was found to have a destructive mass centered in the right mastoid bone, invading the jugular foramen and posterior fossa withou…
