A Case of Fungus Ball-Type Maxillary Sinusitis Due to Penicillium Roqueforti.

The Penicillium genera, encompassing about 225 different species of fungi, are naturally present in the environment. These genera are poorly linked to human disease, except for Penicillium marneffei causing septicemia in immunocompromised hosts. Thus, Penicillium species recovered from respiratory tract samples are often considered as inhaled contaminants in the clinical laboratory. However, we report here a case of fungal maxillary sinusitis due to Penicillium roqueforti diagnosed in a 40-year-old female, a teacher, complaining of moderate pain for months in the maxillary sinus and chronic po…

Acute maxillary sinusitis associated with internal sinus lifting: report of a case.

Sinus floor augmentation (SFA) is one of the techniques that has been proposed for improving the long-term retention of dental implants. The procedure involves the creation of a submucoperiosteal pocket in the floor of the maxillary sinus for placement of a graft consisting of autogenous, allogenic, or alloplastic material. Complications of the SFA predominantly consist of disturbed wound healing, hematoma, sequestration of bone, and transient maxillary sinusitis. In this report, we presented an acute maxillary sinusitis complication following internal sinus lifting in a patient with chronic m…

Massive enlargement of the nasolacrimal canal causing epiphora and chronic maxillary sinusitis.

OBJECTIVES: Enlargement of the nasolacrimal canal (NLC) is a rare anatomic variant. We present the first report of a massively dilated NLC causing epiphora and chronic sinusitis. METHODS: The authors conducted a literature review and case report. RESULTS: A 65-year-old man with refractory sinonasal symptoms and a remote history of a dacryocystorhinostomy was found to have a massively dilated, air-filled NLC. Using a combined endoscopic transnasal and endoscope-assisted Caldwell-Luc approach, the posterolateral wall of the NLC was removed. The patient's symptoms were improved 10 months postop

Leukotriene B4 levels in rabbit maxillary sinusitis: limitations of the current model.

Since the late 1980s, the rabbit model for sinusitis has been widely used for experimental studies on sinusitis; however, the clinical relevance of these experimental data has been questioned. To elucidate the role of leukotrienes in the pathogenesis of sinusitis, leukotriene B4 (LTB4) levels were determined in acute Streptococcus pneumoniae sinusitis in this model. The rabbit model for acute maxillary sinusitis was utilized. Briefly, the right maxillary ostium of each New Zealand white rabbit was occluded with cyanoacrylate under general anesthesia. Twenty-four hours after occlusion, the occl…

Computed tomography evidence of dental restoration as aetiological factor for maxillary sinusitis.

Maxillary sinusitis due to dental causes is usually secondary to periodontal disease or periapical infection and is commonly associated with mucosal thickening of the floor of the maxillary antrum. Computed tomography (CT) is currently the modality of choice for evaluating the extent of disease and any predisposing factors in patients with symptoms of chronic maxillary sinusitis, but it is unable to diagnose dental disease reliably. The presence of restorative dentistry is, however, easily seen at CT and is associated with both periapical and periodontal disease. We aimed to determine whether …

Do simple laboratory tests help in etiologic diagnosis in acute maxillary sinusitis?

The aim of the present study was to evaluate whether the results of such simple hematologic tests as erythrocyte sedimentation rate (ESR), white blood cell count (WBC) and C-reactive protein (CRP) could give any useful information about the causative agents in 176 patients with acute maxillary sinusitis (AMS). The great majority of tests (82%) showed values which were within normal limits. This may be due to the fact that 22% of AMS cases were culture negative for bacteria and about 60% of culture positive cases had Haemophilus influenzae as the etiologic agent. Significantly raised test value…

Serological evidence of viral or Mycoplasma pneumoniae infection in acute maxillary sinusitis.

Evidence for the involvement of viruses, Mycoplasma pneumoniae, and Chlamydia spp. was studied by the complement fixation test in paired sera from 310 young adults (297 men and 13 women) with acute maxillary sinusitis. The diagnosis of acute sinusitis was confirmed by radiography and sinus puncture. Elevated antibody titres were found in 102 patients (33%). A four fold or greater titre rise was detected in 21.5%, and a high stable titre suggestive of recent viral infection was present in a further 11.5%. Adenovirus, influenza A and B viruses, and Mycoplasma pneumoniae accounted for most of the…

Relevance of the conventional Waters' view in evaluating chronic bacterial maxillary sinusitis.

Considerable controversy exists as to whether X-ray examination of the sinuses is a reliable diagnostic guide or not, in cases of chronic bacterial maxillary sinusitis, as many factors apart from infection in the sinuses may produce radiographic signs on the X-ray film. With those facts in mind, it was decided to investigate the diagnostic reliability of Waters' view in these cases. The present work showed that radiographic signs of chronic maxillary sinusitis have different diagnostic and predictive reliabilities ranging from the almost complete inaccuracy of reduced translucency or 'veiling'…

Treatment of acute maxillary sinusitis. Erythromycin base and phenoxymethyl-penicillin (penicillin V).

One hundred patients with acute maxillary sinusitis have been studied. The diagnosis was made clinically and confirmed radiologically, using a roentgenological 6-point score for mucous membrane thickening and secretion. Fifty patients in each group were treated with either erythromycin base or phenoxymethyl-penicillin (penicillin V). The roentgenological state of each sinus was classified on the 1st, 5th, 10th, and 15th day, giving an objective evaluation of the treatment effect. There was no significant difference between the two treatment modes as to therapeutic outcome, and the results are …

Studies on Branhamella Catarrhalis (Neisseria catarrhalis) with special reference to maxillary sinusitis.

Paired sera from 97 patients with acute maxillary sinusitis were examined regarding antibodies to Branhamella catarrhalis. Precipitating antibodies were demonstrated in almost all sera both from patients and from healthy blood donors. Complement-fixing (CF) antibodies to B. catarrhalis were present in sera from 25 of the 97 patients and in one of 20 healthy blood donors. The titres were low and the titre changes when present were of a small magnitude. CF antibodies were most commonly demonstrated in the younger age groups. The patients with demonstrable CF antibodies to B. catarrhalis did not …
