Capturing incarceration in the medical record: we cannot manage what we cannot measure.

Nearly 2 million people are incarcerated in US jails and prisons, where they experience disproportionate burdens of chronic disease, mental illness, substance use disorders, and infectious diseases. Although many receive specialty or acute care in community hospitals, incarceration status is rarely captured in clinical or administrative data, rendering this population largely invisible to quality-improvement efforts, research, and health equity initiatives. Existing identification strategies-including International Classification of Diseases (ICD) codes capturing social determinants of health

Databases and registries recording the usage of cardiovascular implantable medical devices: A rapid review of the literature.

BACKGROUND: Implantable Medical Devices (IMDs) are essential for ensuring the quality of life in patients. They also play a crucial role in terms of the costs associated with surgical procedures, as well as incurring potential risks requiring continuous monitoring following implantation. OBJECTIVE: The aim of this systematic review of public health intervention was to determine which countries collect data on the utilization of implantable cardiovascular medical devices (IMD-Cs), and how countries record the captured data. We identified parameters for comparing the use of IMD-Cs among countri

Reducing Low-Value Delirium Care Using Behavioral Design in the Electronic Medical Record.

Delirium is associated with increased morbidity, mortality, and healthcare costs. Despite best practice guidelines recommending non-pharmacological treatment strategies, delirium is frequently sub-optimally managed with reliance on antipsychotics or benzodiazepines. To optimize delirium management at our institution, we implemented a clinical decision support tool embedded within our Epic electronic medical record (EMR). Our team developed an Our Practice Advisory (OPA) triggered by a positive delirium screening result to alert hospitalists to patients at high risk for delirium. An accompanyin

The Estimated Prevalence of Multiple Record Encounters in the National Emergency Medical Services Information System.

OBJECTIVES: The National Emergency Medical Services Information System (NEMSIS) is a large repository of de-identified Emergency Medical Services (EMS) data supported by the National Highway Traffic Safety Administration and is a cornerstone of large database EMS research. A known concern with large EMS datasets like NEMSIS is that multiple records may exist for a single episode of patient care. However, the extent of potentially related records is not well described. We evaluated potential EMS episodes of care contained within NEMSIS. METHODS: We used an observational study design and conduc

Adding employment status in the medical record demonstrates its importance as a social determinant of health.

OBJECTIVE: To share findings of a quality improvement initiative to capture employment status in the electronic medical record (EMR) to mitigate the potential impact of work loss on the health of patients by utilizing the results to identify eligible individuals for resources to assist return-to-work efforts. MATERIALS AND METHODS: Patients self-identified employment status through a structured new social determinants of health (SDOH) question within the EMR. An electronic outreach campaign was developed to provide information via the patient portal detailing services within healthcare and th

Dementia prevalence and risk factors in people with and without HIV in Malawi: A medical record review.

BACKGROUND: Sub-Saharan Africa (SSA) is experiencing a rapid increase in its aging population, including people living with human immunodeficiency virus (HIV) (PLHIV). The purpose of this study was to determine the prevalence of dementia among PLHIV and people without HIV (POHIV) in Malawi. METHODS: We conducted a retrospective medical record review of 400 consecutive patients from a single tertiary health center (200 PLHIV from an HIV clinic and 200 POHIV from an outpatient clinic) in Lilongwe, Malawi. RESULTS: The overall rate of dementia was higher in PLHIV than that in POHIV (22% vs 10%;

COVID-19 waves in an urban setting 2020-2022: an electronic medical record analysis.

BACKGROUND: Global and national surveillance efforts have tracked COVID-19 incidence and clinical outcomes, but few studies have compared comorbid conditions and clinical outcomes across each wave of the pandemic. We analyzed data from the COVID-19 registry of a large urban healthcare system to determine the associations between presenting comorbidities and clinical outcomes during the pandemic. METHODS: We analyzed registry data for all inpatients and outpatients with COVID-19 from March 2020 through September 2022 (N = 44,499). Clinical outcomes were death, hospitalization, and intensive ca

Use of the electronic medical record to screen for high-risk geriatric patients in the emergency department.

Older adults with multimorbidities have the highest rate of emergency department (ED) usage. These patients are typically on numerous medications, may have underlying dementia, and often present with falls and delirium. Identifying these high-risk older adults for possible intervention is challenging in the ED setting since available screening methods are manual and resource-intensive. The objective is to study the electronic medical record (EMR) use for identifying high-risk older adults in ED. This feasibility study is conducted in an academic ED with 67 000 total and 24% geriatric (age ≥ 65

The most common medications dispensed to lactating persons: An electronic health record-based approach.

PURPOSE: Using a novel, electronic health record (EHR)-based approach, to estimate the prevalence of prescription medication use at 2, 4, and 6 months postpartum among lactating individuals. METHODS: We utilized automated EHR data from a US health system that records infant feeding information at well-child visits. We linked mothers who received prenatal care to their infants born May 2018-June 2019, and we required infants to have ≥1 well-child visit between 31 and 90 days of life (i.e., 2-month well-child visit with a ±1 month window). Mothers were classified as lactating at the 2-month wel

A Retrospective Medical Record Review of Adults with Non-Cancer Diagnoses Prescribed Medicinal Cannabis.

Research describing patients using medicinal cannabis and its effectiveness is lacking. We aimed to describe adults with non-cancer diagnoses who are prescribed medicinal cannabis via a retrospective medical record review and assess its effectiveness and safety. From 157 Australian records, most were female (63.7%; mean age 63.0 years). Most patients had neurological (58.0%) or musculoskeletal (24.8%) conditions. Medicinal cannabis was perceived beneficial by 53.5% of patients. Mixed-effects modelling and post hoc multiple comparisons analysis showed significant changes overtime for pain, bowe

Electronic medical record based tools: Not a panacea in the diagnosis of coin-shaped foreign bodies.

INTRODUCTION: Electronic medical record-based tools have been demonstrated to improve timeliness of x-ray order placement in patients presenting to the emergency department (ED) with coin-shaped foreign body ingestion. Similar efforts directed towards downstream processes are necessary to expedite diagnosis of an esophageal button battery. We predicted that improvement tools such as electronic medical record-based alerts and process standardization could be utilized to expedite x-ray completion. METHODS: Using Plan, Do, Study, Act methodology, iterative interventions were implemented. In July

Harnessing the epic electronic medical record to track indwelling ureteral stents in a pediatric population.

INTRODUCTION: Retention of indwelling ureteral stents due to loss to follow-up can result in significant harm to patients, often requiring multiple trips to the operating room. Despite widespread use of electronic medical records, there are few standardized options for urologists to track ureteral stents and no data on the rate of retained stents in a pediatric population. OBJECTIVE: This pilot quality improvement project aims to: 1) develop a simple process to track indwelling ureteral stents using the Epic electronic medical record and 2) determine the incidence of forgotten stents in a ped…

The relationship between stress and vitiligo: Evaluating perceived stress and electronic medical record data.

Vitiligo is a T-cell mediated skin disorder characterized by progressive loss of skin color. In individuals genetically predisposed to the disease, various triggers contribute to the initiation of vitiligo. Precipitating factors can stress the skin, leading to T-cell activation and recruitment. Though hereditary factors are implicated in the pathogenesis of vitiligo, it is unknown whether precipitating, stressful events play a role in vitiligo. To understand this, we utilized a validated perceived stress scale (PSS) to measure this parameter in vitiligo patients compared to persons without vit

Creating a Synthetic Clinical Trial: Comparative Effectiveness Analyses Using an Electronic Medical Record.

PURPOSE: Electronic medical records (EMRs) are a vast resource of potentially mineable data that can be used to complement and extend clinical trials. Extracting and analyzing EMR data are impeded by technical complexities associated with large, multiformat databases. We sought to develop and validate a framework that would overcome the difficulties associated with EMR data and create a simple, portable, and expandable system to better use this resource. MATERIALS AND METHODS: An Internet-accessible program was developed in Python that applied user-defined criteria to identify and extract pat…
