Vitamin B12 deficiency as a cause of severe neurological symptoms in breast fed infant - a case report.

BACKGROUND: Vitamin B12 (cobalamin, cbl) deficiency in children is rare and may occurs in exclusively breast fed infants of mothers on vegetarian or vegan diet with lack of appropriate supplementation. The clinical manifestation of vitamin B12 deficiency include neurological disorders, megaloblastic anemia and failure to thrive. Routine and commonly used laboratory tests such as cell blood count (CBC) or serum vitamin B12 level are sufficient for appropriate diagnosis. Typical therapy is based on intramuscular cobalamin injections. Early diagnosis and early onset of treatment are crucial facto

Prevalence and etiology of acquired anemia in Medieval York, England.

This paper presents three distinct models for the development of acquired anemia: iron-deficiency anemia produced by the inadequate intake and/or absorption of iron, the anemia of chronic disease (ACD) caused by the body's natural iron-withholding defense against microbial invaders, and megaloblastic anemia caused by insufficient intake and/or absorption of vitamin B(12) or folic acid. These etiological models are used to interpret the distribution and etiology of anemia among adult individuals interred at the Medieval Gilbertine Priory of St. Andrew, Fishergate, York (n = 147). Reversal by cobalamin therapy of minimal defects in the deoxyuridine suppression test in patients without anemia: further evidence for a subtle metabolic cobalamin deficiency.

Subtle cobalamin deficiency states, where low serum cobalamin levels are not accompanied by megaloblastic anemia or malabsorption of free cobalamin, often display metabolic evidence of cellular depletion as shown by the deoxyuridine suppression test. However, the suppression test abnormalities are usually mild and are sometimes atypical; moreover, their response to cobalamin therapy has never been documented. Four patients with this subtle defect, at least three of whom had food-cobalamin malabsorption, were therefore tested before and after cobalamin treatment. Each patient had low serum coba

Could thiamine supplementation delay the progression of retinal dystrophy in Rogers syndrome?

Thiamine-responsive megaloblastic anemia (TRMA) syndrome is a rare defect of thiamine transport caused by biallelic SLC19A2 gene mutations. It is characterized by a triad of diabetes mellitus, megaloblastic anemia responsive to thiamine, and sensorineural hearing loss. The pigmentary retinopathy associated with this syndrome remains poorly investigated. Furthermore, while thiamine supplementation is known to reverse some of the systemic features of this syndrome, its therapeutic role in managing the pigmentary retinopathy remains unknown. We report a longitudinal characterization of TRMA syndr

TRMA syndrome with a severe phenotype, cerebral infarction, and novel compound heterozygous SLC19A2 mutation: a case report.

BACKGROUND: Thiamine-responsive megaloblastic anemia (TRMA) is a rare autosomal recessive inherited disease characterized by the clinical triad of megaloblastic anemia, sensorineural deafness, and diabetes mellitus. To date, only 100 cases of TRMA have been reported in the world. CASE PRESENTATION: Here, we describe a six-year-old boy with diabetes mellitus, anemia, and deafness. Additionally, he presented with thrombocytopenia, leukopenia, horizontal nystagmus, hepatomegaly, short stature, ventricular premature beat (VPB), and cerebral infarction. DNA sequencing revealed a novel compound het

Imerslund-Gräsbeck syndrome in a 15-year-old German girl caused by compound heterozygous mutations in CUBN.

Imerslund-Gräsbeck syndrome (IGS) is a recessive disorder of intestinal cobalamin (Cbl) absorption and renal tubular protein reabsorption sometimes accompanied by urinary tract malformation. Mutations in the cubilin (CUBN) and amnionless (AMN) genes have been described as causal defects. CUBN and AMN proteins form the cubam complex that functions as the receptor for the intrinsic factor-Cbl (IF-Cbl) complex in the ileum and for proteins found in the primary urine in the kidney. We report the case of a 15-year-old German girl who presented with megaloblastic anaemia and funicular myelosis due t

Hereditary partial transcobalamin II deficiency with neurologic, mental and hematologic abnormalities in children and adults.

BACKGROUND: Transcobalamin II is a serum transport protein for vitamin B12. Small variations in TC-II affinity were recently linked to a high homocysteine level and increased frequency of neural tube defects. Complete absence of TC-II or total functional abnormality causes tissue vitamin B12 deficiency resulting in a severe disease with megaloblastic anemia and immunologic and intestinal abnormalities in the first months of life. This condition was described in hereditary autosomal-recessive form. Low serum TC-II without any symptoms or clinical significance was noted in relatives of affected

Vitamin B12 deficiency from the perspective of a practicing hematologist.

B12 deficiency is the leading cause of megaloblastic anemia, and although more common in the elderly, can occur at any age. Clinical disease caused by B12 deficiency usually connotes severe deficiency, resulting from a failure of the gastric or ileal phase of physiological B12 absorption, best exemplified by the autoimmune disease pernicious anemia. There are many other causes of B12 deficiency, which range from severe to mild. Mild deficiency usually results from failure to render food B12 bioavailable or from dietary inadequacy. Although rarely resulting in megaloblastic anemia, mild deficie

[Extra-intestinal complications of ulcerative colitis: hematologic complication].

Iron deficiency anemia, autoimmune hemolytic anemia, folic acid deficiency megaloblastic anemia, granulocytopenia, acute or chronic leukemia, idiopathic thrombocytopenic purpura have been reported as blood diseases among the extra-intestinal complications with ulcerative colitis until now. Iron deficiency anemia is most frequently seen, and it often derives from apparent or inapparent continuous gastrointestinal bleeding. Autoimmune hemolytic anemia produces antierythrocyte membrane autoantibody while idiopathic thrombocytopenic purpura produces antithrombocyte autoantibody leading to anemia o

Granulocyte Fc-IgG and C3b receptor expression in the primary myelodysplastic syndromes (MDS): relationship with dysgranulopoiesis and evidence for heterogeneity of morphological subgroups.

The expression of Fc-IgG and C3b membrane receptors by granulocytes and their precursors was examined in 78 patients with primary myeloid dysplasia (MDS). The marrows were categorized into five morphological groups, broadly corresponding to those described by the FAB group, and further graded into mild and severe according to the severity of dysgranulopoiesis. Fractionated bone marrow and peripheral blood granulocyte receptors were assessed by rosette formation with optimally sensitized ox erythrocytes and the results compared with those found in 17 normal marrows and with those previously rep

VIT. B12 DEFICIENCY IN CHILDREN (IMERSLUND-GRÄSBECK SYNDROME IN TWO PAIRS OF SIBLINGS).

UNLABELLED: Improvement in the quality of life in Europe and North America in last decades caused that economical and social aspects of living conditions of the population have less effect and genetic defects of malabsorption of vitamin B12 became the main reason for cobalamin deficiency in children. Imerslund-Grasbeck syndrome (IGS) is characterized by vitamin B12 deficiency that leads usually to megaloblastic anemia and mild proteinuria. We described two pairs of siblings in two families with IGS. The diagnosis in first family (two brothers) was established at 33 and 22 months of age. The re

Vitamin B12 Deficiency-Induced Massive Schistocytosis and Hemolytic Anemia Due to Pseudo-Thrombotic Microangiopathy: A Case Report.

A 37-year-old male presented with complaints of fatigue, weight loss, and decreased appetite. Laboratory examinations revealed hemolytic anemia, an elevated lactate dehydrogenase (LDH) level, and marked schistocytosis. His other laboratory hematological findings included nucleated red blood cells with abnormal morphologies in the peripheral blood, as well as a small peak that was indicative of schistocytes on a red blood cell histogram. A bone marrow examination demonstrated megaloblastic changes; therefore, a preliminary diagnosis of vitamin B12 deficiency was made. Intramuscular vitamin B12

Diagnosis and classification of pernicious anemia.

Pernicious anemia (PA) is a complex disorder consisting of hematological, gastric and immunological alterations. Diagnosis of PA relies on histologically proven atrophic body gastritis, peripheral blood examination showing megaloblastic anemia with hypersegmented neutrophils, cobalamin deficiency and antibodies to intrinsic factor and to gastric parietal cells. Anti-parietal cell antibodies are found in 90% of patients with PA, but have low specificity and are seen in atrophic gastritis without megaloblastic anemia as well as in various autoimmune disorders. Anti-intrinsic factor antibodies ar

Intestinal absorption, liver uptake, and excretion of 3H-folic acid in folic acid-deficient, alcohol-consuming nonhuman primates.

Nonhuman primates fed folic acid-deficient diets +/- 30% kcal ethanol were used to determine alcohol effects on megaloblastic anemia development and folate bioavailability. Lower hemoglobin (Hb) and red blood cell (RBC) counts and higher mean corpuscular volume (MCV) occurred after 13 wk in alcohol-fed monkeys, later in controls. Plasma, RBC, and liver folate declined and urinary formiminoglutamic acid (FIGLU) was elevated in both groups with FIGLU increasing more among alcohol-fed monkeys at 38 wk. After 40 wk, the bioavailability of oral 3H-folic acid was investigated and showed increased fe
