Molecular pathology for cholangiocarcinoma: a review of actionable genetic targets and their relevance to adjuvant & neoadjuvant therapy, staging, follow-up, and determination of minimal residual disease.

Cholangiocarcinoma (CCA) represents a group of epithelial cell tumors classified based on their anatomic location along the biliary tree. This rare malignancy is often diagnosed at an advanced stage and deemed unresectable. Even for those patients who are surgical candidates, recurrence rates are high and survival rates low. The mainstay of therapy for advanced CCA remains cisplatin plus gemcitabine, with a median overall survival (mOS) under 12 months, although the TOPAZ-1 trial showed a survival benefit with the addition of programmed cell death ligand 1 (PD-L1) blockade. In recent years, mo

Cellular and molecular neurosurgery: pathways from concept to reality--part I: target disorders and concept approaches to gene therapy of the central nervous system.

Recent advances in cellular and molecular biology and better understanding of genetic and biochemical bases of different central nervous system (CNS) disorders have made gene therapy of the CNS a realistic goal. Concept approaches for gene therapy of CNS disorders are reviewed and include the following: 1) gene replacement with a single normal allele to correct the inherited global neurodegenerative disorders, such as enzyme deficiencies; 2) brain repair to restore the function of a particular subset of cells that were lost because of a neurodegenerative process; 3) gene therapy of brain tumor

Druggable targets in pediatric neurocutaneous melanocytosis: Molecular and drug sensitivity studies in xenograft and ex vivo tumor cell culture to identify agents for therapy.

BACKGROUND: Neurocutaneous melanocytosis (NCM) is a rare congenital disorder that presents with pigmented cell lesions of the brain or leptomeninges in children with large or multiple congenital melanocytic nevi. Although the exact pathological processes involved are currently unclear, NCM appears to arise from an abnormal development of melanoblasts or melanocyte precursors. Currently, it has an extremely poor prognosis due to rapid disease progression and lack of effective treatment modalities. METHODS: In this study, we report on an experimental approach to examining NCM cells by establish

Mechanisms of Resistance to Targeted Therapy in Epidermal Growth Factor Receptor (EGFR)- and Anaplastic Lymphoma Kinase (ALK)-Mutated Non-small Cell Lung Cancer: Molecular Basis, Emerging Biomarkers, and Therapeutic Strategies.

Currently, with the spectacular achievement of the tyrosine kinase inhibitors (TKIs), non-small cell lung cancer (NSCLC) carrying mutations of the gene epidermal growth factor receptor (EGFR) and rearrangements of the gene anaplastic lymphoma kinase (ALK) is becoming a paradigm of precision oncology. While there has been great progress in the field of progression-free survival (PFS) and longer-term clinical results have been achieved, resistance sets in, and the long-term impact of these therapies is limited. A summary of the molecular mechanisms of acquired resistance to EGFR- and ALK-targete

Identification of small-molecule inhibitors targeting HER2 for breast cancer therapy using structure-based virtual screening, ADMET profiling, and molecular dynamics simulations.

Human epidermal growth factor receptor 2 (HER2) is one of the key drivers of oncogenesis in HER2+ breast cancer. The overexpression resulted in uncontrollable cell division and additional tumor growth. Despite the current clinical therapeutics such as Tucatinib, Lapatinib, and Neratinib, the demand for novel inhibitors still exists. The research aimed to employ a multi-step computational framework for the discovery of highly effective small molecule inhibitors. The crystal structure of HER2 (PDB: 3PPO) was retrieved, and subsequently, virtual screening of small molecules was carried forward wi

Targeting of Nuclear Factor Erythroid 2-Related Factor 2 (NRF2) Signaling Pathway in Age-Related Eye Diseases: Molecular Mechanisms and Opportunities for therapy.

Age-related eye diseases (AREDs), such as diabetic retinopathy (DR), cataract, glaucoma, age-related macular degeneration (AMD), presbyopia, and represent the primary sources of vision impairment globally. Numerous investigations have indicated that the onset and progression of these conditions are significantly linked to oxidative stress (OS) affecting the eye. The Keap1-Nrf2-ARE signaling pathway is a well-established mechanism that defends the body against OS and inflammation. This pathway is also implicated in the advancement of AREDs. However, the contribution of Nrf2 is likely to be dise

Positively charged, phenolic hydroxyl and anthraquinone structured polystyrene microspheres targeting dual elimination of bacterial pathogens and pathogen-associated molecular patterns for sepsis therapy.

During bacterial infection in sepsis, both the bacteria and pathogen-associated molecular patterns (PAMPs, including endotoxins and exotoxins) contribute to the progression of sepsis. Traditional single-target bactericidal therapies commonly prove ineffective against sepsis, particularly during the hyperinflammatory phase. This work develops a hemoperfusion adsorbent microsphere for the treatment of sepsis via simultaneously clearing bacterial pathogens and their subsequently released PAMPs. Styrene, 2-(dimethylamino)ethyl methacrylate (DMAEMA) and precisely modified aloe emodin (AED) are poly

Small Molecular Theranostic Assemblies Functionalized by Doxorubicin-Hyaluronic Acid-Methotrexate Prodrug for Multiple Tumor Targeting and Imaging-Guided Combined Chemo-Photothermal Therapy.

Because of high drug payload and minimized burden of foreign materials in the course of metabolism and excretion, carrier-free nanomedicine based on self-assembly of small-molecule therapeutic agents has attracted considerable attention in cancer therapy. However, obstacles still remained, such as lack of targeting efficiency, poor physiological stability, and serious drug burst release. Herein, we developed a self-dual-targeting prodrug conjugate by coupling methotrexate (MTX) and doxorubicin (DOX) to a hyaluronic acid (HA) backbone which enveloped the small molecular drug coassemblies of DOX

Uterine metastasis of lung adenocarcinoma under molecular target therapy with epidermal growth factor receptor tyrosine kinase inhibitors: A case report and review of the literature.

A 63-year-old woman presented with abnormal vaginal bleeding. Her disease history was significant, and included advanced lung adenocarcinoma with a deletion mutation in exon 19 of the epidermal growth factor receptor (EGFR) gene, which was managed by concurrent chemoradiotherapy, followed by molecular targeted therapy with tyrosine kinase inhibitors (TKIs) for a two-year period. Contrast-enhanced computed tomography showed the enlargement of a previously suspicious myoma node, with peripheral enhancement. Hemorrhagic necrosis was also observed on magnetic resonance imaging. Transabdominal hyst…

Targeted therapies in genetic dilated and hypertrophic cardiomyopathies: from molecular mechanisms to therapeutic targets. A position paper from the Heart Failure Association (HFA) and the Working Group on Myocardial Function of the European Society of Cardiology (ESC).

Genetic cardiomyopathies are disorders of the cardiac muscle, most often explained by pathogenic mutations in genes encoding sarcomere, cytoskeleton, or ion channel proteins. Clinical phenotypes such as heart failure and arrhythmia are classically treated with generic drugs, but aetiology-specific and targeted treatments are lacking. As a result, cardiomyopathies still present a major burden to society, and affect many young and older patients. The Translational Committee of the Heart Failure Association (HFA) and the Working Group of Myocardial Function of the European Society of Cardiology (

Identification of unique molecular heterogeneity of human CD79, the signaling component of the human B cell antigen receptor (BCR), and synergistic potentiation of the CD79-targeted therapy of B cell tumors by co-targeting of CD79a and CD79b.

We identified unique molecular heterogeneity of CD79 of human B cell antigen receptor (BCR) that may open a new approach to the ongoing CD79b-targeted therapy of B cell tumors. The primary purpose of the present study is to gain new information valuable for the enhanced CD79-targeted therapy. The molecular heterogeneity of CD79 was identified by sequential immunoprecipitation of BCR by use of anti-CD79b monoclonal antibody (mAb) SN8 and anti-CD79a mAb SN8b. SN8 is the antibody component of polatuzumab vedotin, an anti-CD79b antibody drug conjugate, that has been widely used for therapy of diff

Unraveling cellular and molecular mechanisms of relapse in CD19/CD22 dual-targeting chimeric antigen receptor T-cell therapy for B-cell acute lymphoblastic leukemia.

CD19/CD22 dual-target chimeric antigen receptor (CAR19/22) T cell therapies mark a key advance over single-target options for refractory B-cell malignancies. However, over 20% of patients relapse and underlying mechanisms remain less understood. This study evaluated relapse in 91 B-cell acute lymphoblastic leukemia (B-ALL) patients treated with CAR19/22 T cells a clinical trial (ChiCTR-OPN-16008526). The complete remission rate was 91.9%, and 32.9% (26/79) of responders relapsed at a median of approximately 7 months. Among late relapses (>7 months), 92.3% was CAR T functional insufficiency sho
