Management of psychiatric disorders in Parkinson's disease : Neurotherapeutics - Movement Disorders Therapeutics.

Affective disorders (depression and anxiety), psychosis, impulse control disorders, and apathy are common and sometimes disabling psychiatric conditions in Parkinson disease (PD). Psychiatric aspects of PD are associated with numerous adverse outcomes, yet in spite of this and their high frequency, there remains incomplete understanding of epidemiology, presentation, risk factors, neural substrate, and management strategies. Psychiatric features are typically co- or multimorbid, and there is great intra- and interindividual variability in presentation [1]. The neuropathophysiological changes t

Hyperkinetic Movement Disorder Emergencies.

A movement disorder emergency has been defined by Fahn and Frucht as "any neurological disorder evolving acutely or subacutely, in which the clinical presentation is dominated by a primary movement disorder, and in which failure to accurately diagnose and manage the patient may result in significant morbidity or even mortality." In this review, we discuss the most common situations in which hyperkinetic movement disorders, including chorea, ballism, dystonia, myoclonus, tics, as well as psychogenic disorders, can present as emergencies. Some acute hyperkinetic issues that can complicate Parkin…

Eye movements in vestibular disorders.

The differential diagnosis of patients with vestibular symptoms usually begins with the question: is the lesion central or is it peripheral? The answer commonly emerges from a careful examination of eye movements, especially when the lesion is located in otherwise clinically silent areas of the brain such as the vestibular portions of the cerebellum (flocculus, paraflocculus which is called the tonsils in humans, nodulus, and uvula) and the vestibular nuclei as well as immediately adjacent areas (the perihypoglossal nuclei and the paramedian nuclei and tracts). The neural circuitry that contro

Treatment of movement disorder emergencies.

Movement disorder emergencies are defined as clinical scenarios where a movement disorder develops over hours to days, and in which morbidity and even mortality can result from failure to appropriately diagnose and manage the patient. The last decade has seen increasing recognition of various movement disorder emergencies, including acute parkinsonism, neuroleptic malignant syndrome, respiratory compromise in multiple system atrophy, dystonic storm, oculogyric crisis, and hemiballism, among others. This article will review the major movement disorder emergencies encountered in the hospital and

Amphetamine-induced movement disorder.

Recreational use of amphetamines is common in Australia and New Zealand when compared with other developed nations. The clinical effects are variable because of the potential of these drugs to increase the proportion of different biogenic amines in the central nervous system (CNS). The substances affected are adrenaline, noradrenaline, serotonin and dopamine. Movement disorders represent one of the less common presentations of amphetamine toxicity but one that health care workers should be aware of nonetheless.

Eye movements in neurodevelopmental disorders.

PURPOSE OF REVIEW: The aim of this paper is to review the literature on eye-movement abnormalities associated with neurodevelopmental disorders. Eye-movement testing is a non-invasive quantitative approach for evaluating brain systems across the age spectrum. It thus provides a promising methodology for characterizing and documenting maturational abnormalities in brain systems associated with neurodevelopmental disorders. RECENT FINDINGS: Recent oculomotor studies have made significant contributions to the understanding of neurodevelopmental disorders, most notably in autism, attention-defici

Supranuclear Disorders of Eye Movements.

OBJECTIVE: This article reviews the basic anatomy and pathophysiology of the supranuclear ocular motor pathways, emphasizing clinical correlations. LATEST DEVELOPMENTS: More advanced neuroimaging, particularly functional MRI, has yielded insight into previously unclear mechanisms of eye movement control with potential clinical implications. A better understanding of the mechanisms of eye movement control has implications for translational research, resulting in improved and early diagnosis and better treatment options. ESSENTIAL POINTS: Dysfunction of the supranuclear ocular motor pathways t

Disorders of Vergence Eye Movements.

PURPOSE OF REVIEW: The purpose of this review is to summarize current understanding regarding disorders of gaze with comitant ocular misalignment at distance or near and a full range of extraocular movement. Emphasis is placed on clinical features that may be used to differentiate underlying neurologic disease from the more common benign causes. The approach to the diagnostic evaluation and treatment is discussed. RECENT FINDINGS: Randomized controlled trials and Cochrane review suggest the superiority of formal office-based vision therapy in treating convergence insufficiency in children. Di

Movement disorder emergencies in childhood.

The literature on paediatric acute-onset movement disorders is scattered. In a prospective cohort of 52 children (21 male; age range 2mo-15y), the commonest were chorea, dystonia, tremor, myoclonus, and Parkinsonism in descending order of frequency. In this series of mainly previously well children with cryptogenic acute movement disorders, three groups were recognised: (1) Psychogenic disorders (n = 12), typically >10 years of age, more likely to be female and to have tremor and myoclonus (2) Inflammatory or autoimmune disorders (n = 22), including N-methyl-d-aspartate receptor encephalitis, …

Disorders of vergence eye movements.

PURPOSE OF REVIEW: The aim is to re-interpret disorders of vergence in the light of recent studies that view disjunctive eye movements as but one component of three-dimensional gaze control. RECENT FINDINGS: Most natural eye movements combine vergence with saccades, pursuit and vestibular eye movements. Electrophysiological studies in epileptic patients, as well as evidence from monkeys, indicate that frontal and parietal cortex govern vergence as a component of three-dimensional gaze. Clinicians apply Hering's law of equal innervation to interpret disjunctive movements as the superposition o

Rhythmic movement disorder in children.

How should sleep-related rhythmic movements in children be assessed and treated? Rhythmic movement disorder (RMD) represents an unusual variety of childhood parasomnia characterized by repetitive motion of the head, trunk, or extremities, which usually occurs during the transition from wakefulness to sleep or arises during sustained sleep. Although the condition most often affects infants and toddlers in a transient and self-limited fashion, the condition occasionally persists in a problematic fashion, which may nevertheless be amenable to treatment. Since RMD may occasionally cause injury or

[Periodic limb movement disorder].

The periodic limb movements (PLM) are defined as stereotyped, periodic movements of the legs and/or upper limbs during sleep. The patient exhibits dorsifilexion of the ankle and extension of the big toe with occasional flexion of the knee and hip. PLM originally was described as "nocturnal myoclonus" by Symonds in 1953. Recently, the term "nocturnal myoclonus" has been replaced with PLM, because the movements are slower than true myoclonic movement. The appearance of PLM was reported in sleep apnea syndrome, delayed sleep phase syndrome, narcolepsy, spinal cord tumor, diabetes mellitus and ure

The movement disorder of adult opsoclonus.

We present three cases of the adult opsoclonus-myoclonus syndrome in patients with systemic carcinoma. In addition to opsoclonus and myoclonus, other clinical components of the syndrome can include ataxia, tremor, gait and stance dysfunction, altered mental status, and head and face dyskinesias. The most common etiologies are idiopathic, paraneoplastic, and infectious encephalitis. Radiographic and pathological studies suggest brain-stem dysfunction with associated cerebellar and/or cerebellar pathway dysfunction. In many cases, there is evidence for the involvement of immunologic and/or infla

Stereotyped movement disorder of rocking.

Despite the relatively common occurrence of rocking behavior in children, the literature contains little information on the diagnosis, management, and treatment of this condition. The authors report a case of body rocking in an 8-year-old girl, for which they discuss possible etiological factors and therapeutic strategy.

Extraction of sources of tremor in hand movements of patients with movement disorders.

This paper proposes an efficient method to acquire sources of tremor in patients with movement disorders based on blind source separation of convolutive mixtures. The extracted sources indicated neural activities that might be generated in the central nervous system. Four patients with essential tremor were tested in a set of movement tasks. Subjects wore a data glove that measured finger movements of the hand. The experimental data were then fed to a convolutive-mixture model, which revealed sources that imbibed in them the tremor frequency components of 2--8 Hz. Time--frequency analysis of t…

Immunotherapies in autoimmune movement disorders and cerebellar ataxia.

The spectrum of autoimmune movement disorders is very broad with regard to clinical presentations, associated antibodies, and underlying immunopathophysiology. Early and adequate immunotherapy is key in the management to achieve good outcomes, avoid deterioration, or even death. The rarity of these disorders, however, entails a lack of gold-standard treatment trials we could base our management decisions upon. Here, we summarize existing knowledge about the treatment of autoimmune movement disorders and cerebellar ataxias with neuronal antibodies. We start with a brief introduction of the basi…
