Interferon-gamma and colony-stimulating factors as adjuvant therapy for refractory fungal infections in children.

A human immunodeficiency virus-infected boy with Scedosporium apiospermum otomastoiditis and a girl with diabetes mellitus and Mucor sinusitis and orbital cellulitis had life-threatening disease progression despite antifungal treatment. Interferon-gamma and granulocyte-macrophage or granulocyte colony-stimulating factor were added, with good functional outcome in both children. Adjunctive therapy with interferon-gamma, granulocyte-macrophage colony-stimulating factor and granulocyte colony-stimulating factor can be considered for refractory invasive fungal infections.

Ocular sporotrichosis mimicking mucormycosis in a diabetic.

Primary sporotrichosis of the eye is very rare; most infections are limited to the conjunctiva or adnexa. We report a case of Sporothrix endophthalmitis associated with necrotizing ethmoid sinusitis developing in a young diabetic man with ketoacidosis. The infection clinically resembled rhino-ophthalmic mycormycosis. Cure followed evisceration and an abbreviated course (215 mg) of amphotericin B. Sporothrix must now be regarded as another fungal agent capable of causing primary rhino-ophthalmic infection similar to Mucor.

Beyond Candida: A clinical insight into rare fungal pyelonephritis in urological practice.

INTRODUCTION: Non-Candida fungal infections of the urinary tract, such as Aspergillus and Mucor, though uncommon, pose significant diagnostic and therapeutic challenges. Their variable presentation, antifungal resistance, and frequent association with immunocompromised states or prior instrumentation necessitate a high index of suspicion. This study aims to describe the clinical profile, diagnostic approach, and management outcomes of non-Candida fungal pyelonephritis in urological practice. MATERIAL AND METHODS: A retrospective observational analysis was conducted on four patients diagnosed …

Isolation and screening of antimicrobial biosurfactants obtained from mangrove plant root-associated bacteria.

The unique properties of biosurfactants obtained from microbes, including their activity at extreme temperatures, make them more attractive … purified biosurfactants were then screened for antimicrobial properties against pathogens including Mucor sp., Trichoderma sp. Morphological, biochemical, and 16S rRNA gene sequencing identified the two strains to be gram-positive, rod-shaped bacteria namely Virgibacillus halodentrificans CMST-ZI (GenBank Accession No.: OL336402.1) and Pseudomonas pseud …

Zygospores: the last word in identification of rare or atypical zygomycetes isolated from clinical specimens.

Three zygomycetes (order Mucorales), isolated from clinical specimens, whose generic or species identification were uncertain, were definitively identified on the basis of the production of zygospores resulting from mating studies. These three isolates were identified as Mucor circinelloides f. circinelloides, Rhizomucor pusillus, and Absidia corymbifera. The production of true zygospores, the "last word" in zygomycete taxonomy, should be considered as a diagnostic tool for the definitive identification of rare, unusual, or atypical heterothallic zygomycetes. Practical considerations, however,…

FUNGAL OSTEOMYELITIS IN TWO BUFFLEHEAD DUCKLINGS (BUCEPHALA ALBEOLA).

Over a 25-mo span from a single zoologic collection, two bufflehead ducklings (Bucephala albeola) presented with pelvic limb paresis and were euthanized. On postmortem examination, the first duckling had intralesional fungal hyphae consistent with Aspergillus sp. in the spinal vertebrae and within pulmonary granulomas. In the second duckling, evidence of a thoracic spinal lesion was detected antemortem by using thermographic imaging. At postmortem examination, fungal hyphae consistent with Mucor sp. were found within the vertebrae. Although fungal infections of the respiratory system are commo…

Kinetic resolution of 1-chloro-3-(1-naphthyloxy)-2-propanol, an intermediate in the synthesis of beta-adrenergic receptor blockers.

(R)- and (S)-1-chloro-3-(1-naphthyloxy)-2-propanol are intermediates in the synthesis of beta-adrenergic blocking agents and antihypertensive drugs such as propranolol and nadoxolol. Herein, improvement in the preparation of racemic 1-chloro-3-(1-naphthyloxy)-2-propanol generated from 1-naphthol and epichlorohydrin are reported. In addition, kinetic resolution studies have been conducted to obtain both (R) and (S)-1-chloro-3-(1-naphthyloxy)-2-propanol. These compounds were obtained in highly optically pure form by the stereoselective hydrolysis of its acyl derivatives using whole cell preparat

Adjunctive hyperbaric oxygen combined with surgery and antifungal therapy for the management of soft tissue mucormycosis: a case report and review of the literature.

Soft tissue mucormycosis is a rare fungal infection that can develop on contaminated wounds. We report a case of Mucor circinelloides ankle infection in an immunocompetent man following a high-energy motorcycle accident, which has successfully been treated by surgery (multiple debridements, arthrodesis, soft tissue defect reconstruction with flap and skin graft), systemic antifungal therapy (initial liposomal amphotericin B followed by isavuconazole) and hyperbaric oxygen. This case highlights the therapeutic challenges and need for a multidisciplinary approach of such infection.

Rhino-Orbital-Cerebral Mucormycosis: A Fatal Complication of Uncontrolled Diabetes Mellitus.

Mucormycosis is a progressively invasive disease, with a fatal outcome, on late presentation. A 38-year-old female presented with diabetic ketoacidosis with right eye ptosis and a frozen globe without any signs of inflammation, externally. She underwent transnasal endoscopic debridement of paranasal sinuses and exenteration of the right eye. The histopathology specimen revealed the growth of mucormycosis. She was treated with intravenous (IV) amphotericin B, IV insulin, and extensive debridement surgery, but had an unfavorable outcome due to rapid mucor invasion to the brain.

Craniofacial mucormycosis following assault: an unusual presentation of an unusual disease.

A case of craniofacial mucormycosis following assault is discussed. A female diabetic developed peri-orbital cellulitis adjacent to a scalp wound which progressed to a necrotizing fasciitis. This did not respond to treatment. Subsequently the patient developed a hemiparesis, with CT imaging showing peri-orbital and paranasal sinus inflammatory changes, evidence of cavernous sinus invasion and development of a middle cerebral artery territory infarction. The patient died shortly afterwards. The imaging findings and their relationship to the pathological spread of mucor infection are discussed.

Pathogenesis and epidemiology of opportunistic mycotic infections: a review.

The pathogenesis, epidemiology, and ecology of pathogenic and opportunistic systemic infecting fungi are reviewed. The pathogenic fungi including the etiologic agents of histoplasmosis, blastomycosis, coccidioidomycosis, and paracoccidioidomycosis are limited geographically, can establish an infection in a normal host, and exhibit thermal dimorphism. In contrast, the opportunist fungi (Asperigillus sp., Candida sp., Cryptococcus sp., Mucor sp., and Rhizopus sp.) are widely distributed in nature, require altered host defenses for infection, and do not exhibit thermal dimorphism. Mechanisms of p…

H3K4 methylation regulates development, DNA repair, and virulence in Mucorales.

Mucorales are basal fungi that opportunistically cause a potentially fatal infection known as mucormycosis (black fungus disease), which poses a significant threat to human health due to its high mortality rate and its recent association with SARS-CoV-2 infections. On the other hand, histone methylation is a regulatory mechanism with pleiotropic effects, including the virulence of several pathogenic fungi. However, the role of epigenetic changes at the histone level never has been studied in Mucorales. Here, we dissected the functional role of Set1, a histone methyltransferase that catalyzes t…

Features and evaluation of mucormycosis in COVID-19 patients from two referral hospitals in Iran.

OBJECTIVES: The present study aimed to assess the features, clinical characteristics, and species diversity among patients admitted to referral Hospitals for SARS-CoV-2 pneumonia and mucormycosis in Tehran, Iran, and the relationship between seasonal and species diversity was considered. METHODS: Confirmed COVID-19 patients with a positive reverse-transcriptase real-time (rRT-PCR) test for SARS-CoV2 were primarily included based on clinically suspected mucormycosis infection and confirmed by histopathology and mycology examination of biopsy specimens. The PCR technique was performed by the am…

Value of Imprint Cytology for the Rapid Diagnosis of Mucormycosis in the COVID-19 Pandemic Setting - A Pilot Study.

BACKGROUND: The second wave of the coronavirus disease 2019 (COVID-19) pandemic recorded a surge in rhino-orbital-cerebral mucormycosis (ROCM) infection in COVID-19-positive patients with diabetes and on concomitant steroid therapy. The rapidly progressive and devastating nature of the disease necessitated prompt diagnosis and early intervention to improve patient outcomes. Histopathology and fungal culture remain essential tools; however, these investigations have long and variable turn-around times (TATs) and may delay the initiation of treatment. Frozen section is not widely available and s…

To Cut (Minimally Invasively) Is to Cure: Robotic Lobectomy for Pulmonary Mucormycosis.

Pulmonary mucormycosis is a rapidly progressive and highly morbid disease commonly found in immunosuppressed patients. Rapid diagnosis and treatment are essential, with thoracotomy and video-assisted thoracoscopic surgery techniques previously described for surgical resection of infected lobes. Here we present the case of a patient with acute myeloid leukemia and nonresolving Mucor pneumonia treated with robotic left upper lobectomy. The patient had an uneventful postoperative course and significant clinical improvement, thereby demonstrating the feasibility of the robotic approach in treating…
