Subclinical Retinal versus Brain Findings in Infants with Hypoxic Ischemic Encephalopathy.

PURPOSE: To detect retinal features and abnormalities on optical coherence tomography (OCT) without pupil dilation and relate these to brain injury in infants with a clinical diagnosis of hypoxic ischemic encephalopathy (HIE). METHODS: Under an institutional review board-approved protocol, we imaged eight infants without pharmacologic mydriasis, using handheld, non-contact spectral-domain (Leica Microsystems, IL) or investigational swept-source OCT at the bedside in an intensive care nursery, after birth (depending on primary clinical care team permission based on health status) and weekly un…

Aggressive medical management of acute traumatic subdural hematomas before emergency craniotomy in patients presenting with bilateral unreactive pupils. A cohort study.

BACKGROUND: The outcome of patients with severe traumatic brain injury (TBI) and acute traumatic subdural hematoma (aSDH) admitted to the emergency room with bilaterally dilated, unreactive pupils (bilateral mydriasis) is notoriously poor. METHODS: Of 2074 TBI patients consecutively admitted to our facility between 1997 and 2012, 115 had a first CT scan with aSDH, unreactive bilateral mydriasis, and a Glasgow Coma Score of 3 or 4. Sixty-two patients were unoperated and died within hours or a few days. The remaining 53 patients (2.5% of the 2074 consecutive patients) were scheduled for emergen…

Neonatal diagnosis of ACTA2-related disease: A case report and review of literature.

Multisystemic smooth muscle dysfunction syndrome (MSMDS, OMIM # 613834) is a rare autosomal dominant condition caused by pathogenetic variants of ACTA2 gene that result in impaired muscle contraction. MSMDS is characterized by an increased susceptibility to aneurismal dilatations and dissections, patent ductus arteriosus, early onset coronary artery disease, congenital mydriasis, chronic interstitial lung disease, hypoperistalsis, hydrops of gall bladder, and hypotonic bladder. Here, we report an early diagnosis of a MSMDS related to ACTA2 p.Arg179His (R179H) mutation in a newborn and performe…

Urrets-Zavalia syndrome (fixed and dilated pupil following penetrating keratoplasty for keratoconus) and its variants.

Urrets-Zavalia Syndrome (UZS) was described 50 years ago as the appearance of a fixed and dilated pupil following penetrating keratoplasty for keratoconus in patients receiving atropine. The mechanism of UZS has still not been fully determined, but an acute increase in intraocular pressure and ischemia of the iris most probably play a major role. Fixed and dilated pupils also occur in association with other ophthalmic surgeries, such as lamellar keratoplasties, cataract surgery, and glaucoma procedures. Some have questioned the existence of this syndrome, as well as the linkage to keratoconus

Choroidal Rupture in a Collegiate American Football Player: A Case Report Highlighting the Importance of Protective Visors.

CASE: This case describes a 26-year-old male Division-1 college senior American football player who sustained a traumatic choroidal rupture from a direct blow to the eye during a regular season football game. Despite prompt ophthalmologic evaluation and treatment, the player developed permanent post-traumatic mydriasis (pupil dilation) and infratemporal quadrantanopia. CONCLUSION: Sports-related ocular injuries are common, and their impact may be devastating, potentially resulting in complete vision loss and permanent disability. The purpose of this case report was to highlight sports ocular …

[Logical Explanation of Medical Condition for Patients with Terminal Cancer at Home-Retrospective Analysis of 145 Cases].

Frequently, patients with advanced cancer are suffered by various symptoms at the end of life. Furthermore, sometime experience unexpected loss of consciousness(LOC)and/or respiratory arrest. I examined carefully following neck stiffness, LOC, masked face, urinary retention, tonic and/or clonic convulsions, apnea, dysphagia, head rotation, neurogenic pain, hyperthermia, mydriasis 145 patients with advanced cancer. By assuming meningeal carcinomatosis, multifocal neurological findings are logically expected. As a result, before the LOC or respiratory arrest, I was able to logically explain to a…

Autonomic manifestations in acute sensory ataxic neuropathy: a case report.

Acute sensory ataxic neuropathy (ASAN) is known to occur with acute and monophasic sensory ataxia. Although autonomic dysfunctions have been reported, no detailed descriptions are currently available. We describe a case of ASAN in which the autonomic manifestations were systematically investigated. Although the patient did not complain of any autonomic symptoms, except for photophobia due to mydriasis, autonomic testing revealed widespread autonomic dysfunctions. Norepinephrine and dobutamine infusion test indicated the presence of sympathetic dysfunction. Additionally, the pupillary response …

[Lidocaine intoxication in newborn following maternal pudendal anesthesia: report of three cases].

Pudendal nerve block may be indicated during instrumental delivery in situations where peridural anesthesia is unavailable. We report three cases of neonatal lidocaine intoxication following maternal pudendal block during delivery. Clinical features were hypotonia, pupillary mydriasis fixed to light, apnea, cyanosis and seizures. Two neonates required mechanical ventilation. Lidocaine was found in the serum of two babies. In all three cases, recovery was complete. The pharmacokinetics of lidocaine in a highly vascularized perineum during labor increase the risk of neonatal intoxication. A poss…

Tetanus-like syndrome secondary to metoclopramide administration.

A case of metoclopramide-induced pseudotetanus in a 24-year-old woman is described. Symptoms included flexor spasms of the neck muscles, neck pain, left deviation of the lower jaw and tongue protrusion. Tetanus was initially suspected. Mydriasis, hyperhydrosis and clinical observation indicated that this syndrome could be caused by metoclopramide taken for gastrointestinal symptoms. Orphenadrine hydrochloride, diazepam and ketoprofen completely resolved the symptoms. Pseudotetanus presents in many different ways, and it is sometimes very hard to distinguish it from infective tetanus, which is …

Total Spinal Anesthesia Following an Interscalene Block Treated with Intravenous Lipid Emulsion.

Total spinal anesthesia following interscalene block is a rare and life-threatening complication of regional anesthesia. A 56-year-old woman underwent an uncomplicated left shoulder bone spur removal under general anesthesia with an interscalene nerve block at an outpatient surgical center. Subsequently, she developed bilateral mydriasis, paralysis of all extremities, and respiratory arrest. She was intubated and transferred to the emergency department (ED) where she was given intravenous lipid emulsion (ILE) with complete recovery of neurological function. ILE therapy may be considered as a r…

Intraoperative dilated pupil during nasal polypectomy.

In endoscopic sinus surgery, it is often desirable to prepare the nasal mucosa with a nasal decongestant to minimise blood loss and enhance the operative field during surgery. A widely used commercially available preparation for such a purpose is a solution containing 5% Lidocaine Hydrochloride and 0.5% Phenylepherine Hydrochloride. We report a case of a unilateral dilated pupil occurring during nasal polypectomy. A dilated pupil during sinus surgery is associated with an intraorbital injury but many commonly used nasal decongestive agents can also cause mydriasis and this knowledge may preven…

[Management of cilio-choroidal detachment associated with rhegmatogenic retinal detachment].

51 cases (17%) of cilio-choroidal detachment were associated with rhegmatogenic retinal detachment in a series of 305 cases of the latter. The prominent feature in these cases was extraordinary hypotony, which was an important factor in pathogenesis of the transudative ciliochoroidal detachment. Among the 46 cases that underwent surgical operation, the rate of successful retinal reattachment was 70% (32/46) after a follow-up of 26 months. The good results were believed to have been due to early diagnosis, high dosage topical and systemic steroid therapy, intensive mydriasis, and performance of…

[Evaluation of comatose states].

The most informative neurological symptoms allowing the differentiation of 7 phases of consciousness disorders have been identified on the basis of clinico-physiological correlations and mathematical processing. In the first phase of coma the patient cannot be waken up. Addition of muscle atonia characterizes the transfer to the second phase of coma and of bilateral mydriasis indicates the transfer to the third phase. Eye opening in response to sound or pain points to awakening and transfer to the phase of apallitic syndrome. The performance of simple instructions is characteristic of the phas…

Is phenylephrine pivalate a prodrug?

Phenylephrine pivalate has been assumed to be a prodrug devoid of important intrinsic activity because of its structural similarity to dipivefrin (dipivaly epinephrine). However, unlike dipivefrin, the pharmacologic activity of phenylephrine pivalate was not prevented by prior administration of echothiophate iodide. Rabbits pretreated bilaterally with 0.25% echothiophate for two and seven days had similar mydriasis, both in quantity and duration, after receiving 10% phenylephrine hydrochloride to one eye and 1% phenylephrine pivalate to the other eye. This as consistent with the hypothesis tha…

The "facie sympathique" sign in hanging: historical background, forensic review, and perspectives.

The "facie sympathique" is a vital sign first described by Etienne Martin in 1899 referring to unilateral miosis, with or without ptosis, at the opposite side from the knot in hanging. This mark is scarcely reported in legal medicine textbooks and scientific papers. Moreover, when cited, it is referred to differently from its original meaning, both as unilateral contraction (miosis) and dilatation (mydriasis) of the pupil depending on the antemortem firmness of the ligature's neck pressure in hanging with little attention to ptosis. Due to the sympathetic nervous pathway supplying the eye, the…

[Pupil-sparing painful oculomotor neuropathy due to a carotico-cavernous aneurysm].

INTRODUCTION: In third cranial nerve palsy, the lack of mydriasis evokes a noncompressive mechanism. Case report. We report a case of a slightly painful, pure extrinsic third-nerve palsy, complete except for the partial ptosis secondary to the compression by an intracavernous carotid aneurysm. Percutaneous endovascular embolization was followed by complete regression of the palsy within 4 weeks. The pupil may have been spared by the mechanism of compression after the separation of the pupillomotor and extrinsic fibers or the ischemia of the third nerve by its arterial trunk lesion. CONCLUSION…
