Naltrexone (Nalt) causes a rapid increase in luteinizing hormone (LH) level. This short term increase of LH concentration declines to baseline levels in less than 1 hour. Addition of pimozide (0.1 mg) caused a blunted response to Nalt challenge, with significantly reduced LH peak values compared with Nalt treatment alone. Pimozide alone caused a delayed decrease compared with baseline LH values. By following plasma prolactin (PRL) levels it was shown that pimozide administration increased PRL levels rapidly for more than 2 hours. Addition of Nalt to pimozide-treated rats significantly decrease…

In summary, evidence is presented associating typically asymptomatic and reversible elevations of serum transaminase values with high daily dosages of naltrexone. Statistical significance was found only between placebo and the 300 mg dosage. Subjects aged 40 years and over were significantly more likely to develop this finding than younger subjects. All subjects with significant elevations of transaminase values in these studies took daily naltrexone dosages higher than recommended for opioid addiction. The daily dosage of naltrexone recommended for opioid addiction did not cause abnormalities…

BACKGROUND: Compulsive sexual behaviour disorder (CSBD), characterised by preoccupation with and loss of control over sexual behaviours, is associated with negative consequences in several areas of life. Evidence for pharmacological treatment remains limited, with only two previous randomised controlled trials and no studies comparing fluoxetine and naltrexone.
OBJECTIVE: To investigate whether naltrexone was more efficient and tolerable than fluoxetine in the treatment of CSBD.
METHODS: In this open-label superiority trial, 80 participants diagnosed with CSBD according to International Clas…

A veterinary referral center specializing in veterinary oncology collaborated with a local compounding pharmacy to evaluate the effects of adding low-dose naltrexone (LDN) to the palliative care of dogs undergoing treatment for various cancers. Seven male neutered dogs were initially enrolled, each prescribed naltrexone 2 mg or 4.5 mg capsules (LoxOral), depending on body weight. Dosing began once daily for 30 days, then increased to twice daily for the remainder of the three-month study. Health-related quality of life (HRQoL) was assessed at baseline and every 9-10 days using a structured, ca…

Chronic alcohol exposure threatens central nervous system homeostasis, linking to interconnected dysregulated emotional behavior and robust neuroinflammation. Clinically, individuals with alcohol use disorder (AUD) frequently present comorbid anxiety and depression, yet whether naltrexone, an established therapeutic, can ameliorate AUD neuroinflammation remains unclear. This study investigated naltrexone's effects on anxiety- and depressive-like behaviors and neuroinflammatory responses in chronically alcohol-exposed mice. Modified 'Drinking-in-the-Dark' protocol for 4 weeks, which induced alc…

Chronic pain imposes a substantial burden on global health and remains challenging to manage, despite ongoing advances in pharmacologic and interventional therapies. Recognition of chronic pain as a condition driven by central sensitization and neuroimmune dysregulation has prompted interest in therapies that target these mechanisms rather than peripheral nociception alone. Low-dose naltrexone (LDN), administered at doses substantially lower than those used for opioid or alcohol use disorders, has emerged as a repurposed treatment with potential analgesic and anti-inflammatory properties. This…

Despite the contribution of the µ-agonist fentanyl to the United States's opiate overdose epidemic, no human studies specifically address the ability of extended-release preparations of the opiate antagonist naltrexone (NTX) to block fentanyl's life-threatening µ-agonist-mediated respiratory depression. This paper presents three case histories of clinically necessary opiate challenges in opiate-abusing patients implanted with extended-release NTX (ER-NTX). It also reviews the sparse literature and is the first evidence that antagonist blood levels from ER-NTX preparations can completely block …

Endogenous opioid release has been linked to exercise. We investigated if opioid blockade following forced swimming, a common model of rodent exercise, influenced cerebral glucose metabolism in mice. PET scan was used to assess the uptake of Fludeoxyglucose (FDG-18), a marker of cerebral glucose metabolism in 19 regions of the interest in the brain following: forced swimming, an acute dose of the opioid receptor blocker naltrexone or a combination of both. Forced swimming increased glucose uptake in the cerebellum, while naltrexone + forced swimming increased glucose uptake in the hypothalamus…

Background: Emerging evidence suggests that opioid receptor antagonists, such as naltrexone, are effective pharmacotherapies for alcohol, opioid, and possibly stimulant use disorders. It is posited that naltrexone exerts its effects, in part, by increasing functional connectivity between neural reward circuitry and frontal systems implicated in executive function. Yet no studies had examined whether executive function moderates these effects. Objectives: This study examined whether a composite measure of executive function (EF) moderates the effect of naltrexone on craving for methamphetamine …

BACKGROUND AND AIM: Extended-release naltrexone (XR-NTX) blocks the effects of opioids for 4weeks; however, starting treatment can be challenging because it requires 7 to 10days of abstinence from all opioids. In the present study we identified patient and treatment characteristics that were associated with successful induction onto XR-NTX.
METHODS: 144 unemployed heroin-dependent adults who had recently undergone opioid detoxification completed self-report measures and behavioral tasks before starting an outpatient XR-NTX induction procedure. Employment-based reinforcement was used to promot…

INTRODUCTION AND AIMS: To estimate the number of deaths that would have occurred among patients receiving oral naltrexone for opioid use under the Special Access Scheme if these patients had received methadone.
DESIGN AND METHODS: We analysed mortality in cohorts treated with oral naltrexone and methadone. Data were from 1097 patients of in WA providing oral naltrexone for opioid use under the SAS,1998-2000, and all participants in WA (n = 2520) and New South Wales (NSW) (n = 11,174) methadone programs over the same period. We calculated mortality rates among patients receiving naltrexone and…

Complex Regional Pain Syndrome (CRPS) is a neuropathic pain syndrome, which involves glial activation and central sensitization in the central nervous system. Here, we describe positive outcomes of two CRPS patients, after they were treated with low-dose naltrexone (a glial attenuator), in combination with other CRPS therapies. Prominent CRPS symptoms remitted in these two patients, including dystonic spasms and fixed dystonia (respectively), following treatment with low-dose naltrexone (LDN). LDN, which is known to antagonize the Toll-like Receptor 4 pathway and attenuate activated microglia,…

BACKGROUND: Naltrexone has considerable potential in helping to prevent relapse in heroin dependency. A longer-lasting formulation for naltrexone treatment is desirable to further reduce non-adherence and relapse during treatment of opiate dependence.
AIMS: To evaluate the safety and effectiveness of a 6-month naltrexone implant in reducing opioid use after in-patient treatment.
METHOD: A group of 56 abstinence-oriented patients who completed in-patient treatment for opioid dependence were randomly and openly assigned to receive either a 6-month naltrexone implant or their usual aftercare. D…

This clinical trial assessed the effects of monitored naltrexone treatment in 19 subjects with schizophrenia spectrum and alcohol use disorders in an eight-week prospective open pilot study. Naltrexone was directly administered to subjects in oral doses of 100 mg on Mondays and Wednesdays, and 150 mg on Fridays. Subjects received reimbursement for attending the three weekly study visits. Subjects continued to receive their usual psychiatric care with no added alcohol counseling provided. Alcohol use was assessed by self-report and biomarkers. Psychosis severity was measured by the Positive and…

The effects of naltrexone-precipitated withdrawal from buprenorphine on behavior and regional cerebral blood flow (rCBF) were studied in 11 opiate-dependent patients. Patients initially received buprenorphine, 2 mg sublingually, every day for 7 days. They were then challenged sequentially with placebo and naltrexone, 25 mg orally, before single photon emission computed tomography with technetium-99m-d,l-hexamethyl-propylene amine oxime as tracer. Behavioral ratings of withdrawal severity were made before and after naltrexone/placebo administration. Naltrexone produced significantly greater sig…

BACKGROUND: Drug-related mortality in the United States continues to transform, marked by a surge in overdose deaths involving both fentanyl and stimulants. This pattern presents specific risks for people at risk for and living with HIV, who have high rates of stimulant use and disproportionate risk of opioid overdose due to unintentional fentanyl use. STUDY OBJECTIVES: This pilot randomized controlled trial evaluates intramuscular naltrexone as a novel biomedical strategy for opioid overdose prevention among people who use stimulants living with or at risk for HIV. The primary outcome is acce…

People involved in the criminal justice system have higher rates of opioid use disorder (OUD) and are less likely to receive recovery services. Medications to treat OUD, such as extended-release naltrexone, have been found to reduce relapse and recidivism. We hope to provide practical lessons learned from the evaluation process of the Department of Corrections' Extended-Release Naltrexone Program and considerations for incorporating medications to treat OUD into a reentry program. The program evaluation followed participants for 1 year and conducted interviews and surveys on health, well-being…
