More than a pain in the neck: vertebral artery dissection and subarachnoid hemorrhage.

Intracranial vertebral artery dissection is an uncommon but important cause of subarachnoid hemorrhage. These patients have a predilection for early rebleeding that makes early diagnosis and intervention imperative. The arterial dissection and associated dissecting aneurysm are often located close to the origin of the posterior-inferior cerebellar artery. Endovascular treatment techniques have been increasingly employed. An illustrative case that was treated successfully with coils but complicated by a posterior-inferior cerebellar artery territory infarction is presented. The objectives are t

Traumatic dissection of the common carotid artery after blunt injury to the neck.

BACKGROUND: Occlusive lesions of the common carotid artery (CCA) resulting from blunt injury are extremely rare, and their clinicopathologic and therapeutic features have not yet been clarified. OBJECTIVES AND RESULTS: Five patients with occlusive lesions of the CCA developed neurologic deficits at 1.5 hours to 10 years after blunt neck injury. Lesions included two complete occlusions, one severe stenosis, and two segmental intimal dissections of the CCA. In the two patients with CCA occlusion, bypass surgery was performed using a Dacron graft between the ipsilateral subclavian artery and the

Anatomical variants of the cervical sympathetic chain to be considered during neck dissection.

Horner's syndrome, as a complication of radical neck dissection, is given little attention in textbooks of head and neck surgery. To investigate which topographical anatomic factors, if any, might influence damage to the sympathetic chain during neck dissection, we undertook a series of 12 cadaveric neck dissections. The axial position of the cervical sympathetic chain varied. The chain could be clearly delineated from the carotid sheath except in two cadavers, in which it was found within the sheath. The presence of cervical ganglia also varied. We suggest that if the chain is within the shea

The syndrome of inappropriate secretion of antidiuretic hormone (SIADH) as a consequence of neck dissection.

The syndrome of inappropriate secretion of antidiuretic hormone (SIADH) can have multiple causes. Surgical neck dissections may have an association with this syndrome and represent the basis for this study. A retrospective review of 50 patients undergoing neck dissections was performed to evaluate for the development of hyponatraemia as a consequence of SIADH. Based on the results of this review, a prospective study of 20 consecutive patients undergoing 22 neck dissections was performed to determine the incidence of SIADH. A control group of 25 consecutive patients undergoing major non-neck di

Extent of neck dissection after transoral robotic surgical resection of oropharyngeal squamous cell carcinoma: Report of a case and potential indications for inclusion of level I in a selective neck dissection.

BACKGROUND: With new minimally invasive surgical techniques playing a role in the management of early oropharyngeal carcinomas, appropriate surgical management of the neck is an increasingly important consideration. METHODS: We present a case of a 47-year-old patient with a clinical T2N1M0 squamous cell carcinoma (SCC) of the right tonsil with anterior extension toward the floor of the mouth from the right glossotonsillar sulcus. Imaging revealed a solitary necrotic lymph node in level IIA. The decision was made to proceed with a transoral robotic pharyngectomy and selective neck dissection o

Does previous neck dissection or irradiation mitigate the response to immunotherapy in advanced/metastatic head and neck squamous cell carcinoma?

Immunotherapy has brought new hope to patients with locally advanced, recurrent or metastatic (R/M) head and neck squamous cell carcinoma (HNSCC). This study aimed to evaluate the outcomes at our institution, focusing on the response to immunotherapy in patients whose draining lymph nodes were previously dissected and/or irradiated. A retrospective study on patients treated with immunotherapy, indicated for unresectable or R/M HNSCC between 2014 and 2024, was conducted. Treatment response and survival outcomes were analysed. Eighty-four patients were identified, including 26 patients (31%) who

Planned neck dissection for advanced primary head and neck malignancy treated with organ preservation therapy: disease control and survival outcomes.

BACKGROUND: The role of planned neck dissection after organ preservation therapy with radiotherapy or chemotherapy/radiotherapy for advanced head and neck cancers presenting with clinically positive neck disease is still being elucidated. The aim of this study is to review the outcomes of such patients treated by organ preservation therapy at our institution. METHODS: A retrospective chart review of 33 patients who underwent planned neck dissections after organ preservation therapy for advanced primary head and neck malignancy. Endpoints measured were disease-free survival and local, regional

Surgical predictors of osteoradionecrosis in irradiated head and neck cancer patients: Jaw resection increases risk whereas neck dissection does not.

INTRODUCTION: This study investigated the impact of jaw resection (JR) and neck dissection (ND) on the incidence of jaw osteoradionecrosis (JORN) in patients with head and neck cancer (HNC) who underwent head and neck radiotherapy (HNR) involving the jawbone with/without ablative surgery. Evidence on this association remains scarce. MATERIALS AND METHODS: This retrospective study examined irradiated HNC patients treated at a German university hospital between 2007 and 2022, with follow-up until December 2024. The predictor variables were JR and ND, with JORN as the main outcome. Appropriate s

Elective neck dissection versus elective neck irradiation in cT3/4N0 maxillary sinus squamous cell carcinoma: a propensity score matching analysis.

BACKGROUND: Maxillary sinus squamous cell carcinoma (MS-SCC) is an infrequent malignancy, and determining the optimal neck management for patients with cT3/4N0 MS-SCC remains a topic of ongoing debate. The purpose of this study was to compare the prognoses and quality of life outcomes of patients who underwent either elective neck dissection (END) or elective neck irradiation (ENI) for cT3/4N0 MS-SCC. METHODS: In this retrospective study, we enrolled patients with surgically treated cT3/4N0 MS-SCC, and the impact of different neck management strategies on regional control and disease-specific

Initial experience with robot-assisted modified radical neck dissection for the management of thyroid carcinoma with lateral neck node metastasis.

BACKGROUND: Since the introduction of endoscopic techniques in thyroid surgery, several trials of endoscopic lateral neck dissection have been conducted with the aim of avoiding a long cervical scar, but these endoscopic procedures require more effort than open surgery, mainly because of the relatively nonsophisticated instruments used. However, the recent introduction of surgical robotic systems has simplified the operations and increased the precision of endoscopic techniques. We have described our initial experience with robot-assisted modified radical neck dissection (MRND) in thyroid canc

Neck level-specific nodal metastases in oropharyngeal cancer: is there a role for selective neck dissection after definitive radiation therapy?

PURPOSE: To determine the extent of the required neck dissection for patients with persistent lymphadenopathy after definitive radiation therapy for oropharyngeal cancer. If feasible, a conservative approach using selective neck dissection would likely minimize the extent of neck fibrosis and other adverse sequelae. METHODS: Analysis of pretreatment and posttreatment radiologic scans and pathology reports of 76 patients with oropharyngeal carcinoma (35 tonsil; 41 base of tongue), who had radiologic evidence of persistent nodal disease for level-specific involvement. Patients were treated with

The Prognostic Significance of Elective Level 4 Neck Dissection in Oral Tongue Cancer.

INTRODUCTION: Elective neck dissection (END) for oral tongue squamous cell carcinoma (OTSCC) typically involves level 4 due to potential metastases that may bypass levels 1-2. Our study challenges this notion and investigates the necessity of level 4 inclusion in END for OTSCC. METHODS: A retrospective cohort study performed in a tertiary-care university affiliated medical center and included all OTSCC patients treated with END from 2000 to 2020, with a minimum 2-year follow-up. The study compared patients with END levels 1-3 to those with levels 1-4 regarding regional recurrence, disease-spe

Outcomes of selective neck dissection in node positive oral cavity squamous cell carcinoma.

Management of the neck in oral cavity squamous carcinoma is debatable. The role of selective neck dissection (SND) in node positive neck in oral SCC is controversial. The aim of this study was to determine the efficacy of SND for the node positive neck in patients with oral cancer. A retrospective chart review was done of 424 oral cancer patients undergoing SND, who had pathologically node positive disease on histopathology treated from January 2017 to September 2024. A total of 424 patients were analysed with 82% male predominance. The median age was 49 years. The commonest primary site was t

Long-Term Outcomes After Elective Contralateral Neck Dissection for HPV-Related Oropharyngeal Cancer.

OBJECTIVE: Patients with human papillomavirus (HPV)-related oropharyngeal squamous cell carcinoma (OPSCC), particularly of the base of tongue, have a high rate of occult nodal disease in the contralateral neck. The risk of failure in the contralateral neck is reduced with elective treatment. The optimal treatment strategy to minimize treatment-related toxicity while preserving regional control in the contralateral neck has not been elucidated. STUDY DESIGN: Cross-sectional study of patients who underwent elective contralateral neck dissection as part of primary surgical treatment for HPV-rela

Study of the Anatomical Variations of Spinal Accessory Nerve Seen During Neck Dissection.

Neuroinflammation is known to be a contributing factor for several neurological disorders as well as cognitive dysfunction. Different signalling pathways, and a variety of supporting cells of CNS are suggested to be involved in the progression of neurodegeneration. Among the factors contributing to neuroinflammation, peripheral inflammation takes a lead role according to recent research, since persistent peripheral inflammation is believed to disrupt the blood-brain barrier (BBB). This, in turn, allows the peripheral immune cells to infiltrate the central nervous system (CNS), triggering a chr

Foregoing elective neck dissection for elderly patients with oral cavity squamous cell carcinoma.

OBJECTIVE: Elective neck dissection (END) improves outcomes among clinically node-negative patients with oral cavity squamous cell carcinoma (OCSCC). However, END is of questionable value, considering the potentially higher comorbidities and operative risks in elderly patients. METHODS: A retrospective review of all patients older than 65 years of age who were treated for OCSCC at a tertiary care centre between 2005 and 2020 was conducted. RESULTS: Fifty-three patients underwent primary tumour resection alone, and 71 had simultaneous END. Most primary tumours were located on the mobile tongu
