Incidence of residual neuromuscular blockade and use of neuromuscular blocking agents with or without antagonists: A systematic review and meta-analysis of randomized controlled trials.

OBJECTIVES: Neuromuscular blocking agents (NMBAs) have revolutionized the field of anesthesiology as they facilitate airway management and ensure optimal surgical conditions. Despite their beneficial and ubiquitous use during surgery, delayed or partial recovery from NMBAs, referred to as residual neuromuscular block (rNMB), is a common clinical problem. While it is well accepted that the antagonist sugammadex, compared to neostigmine, can more rapidly reverse rocuronium-induced NMB regardless of depth of block, the occurrence of rNMB for routinely used combinations of NMBAs with sugammadex or…

Muscarinic receptor activation in the lumbosacral spinal cord ameliorates bladder irritation in rat cystitis models.

OBJECTIVE: To investigate whether activation of spinal cholinergic pathways affects bladder activity in rats with chemical cystitis induced by acetic acid (AA) and cyclophosphamide (CYP). MATERIALS AND METHODS: The effects of intrathecal (i.t.) application of neostigmine as an acetylcholine esterase (AChE) inhibitor, oxotremorine-M (OXO-M) as a muscarinic ACh receptor (mAChR) agonist or epibatidine as a nicotinic AChR (nACHR) agonist with or without atropine as a mAChR antagonist or mecamylamine (MEC) as a nAChR antagonist at the level of L6-S1 spinal cord on C-fibre mediated bladder irritati…

New-Onset Accelerated Idioventricular Rhythm During Dental Rehabilitation.

Accelerated idioventricular rhythm has been documented in several cases involving the induction of general anesthesia; however, it has not previously been known to occur during reversal of neuromuscular blockade with neostigmine and glycopyrrolate. The current understanding of the pathophysiology of accelerated idioventricular rhythm involves enhanced automaticity of ventricular myocardium in the setting of increased vagal tone suppressing sinoatrial node pace making. We present the case of an 8-year-old boy who developed accelerated idioventricular rhythm during dental rehabilitation. In this…

Comparison of the Effects of Glycopyrrolate and Atropine on Postoperative Delirium in Older Adult Patients Undergoing Laparoscopic Colorectal Surgery: A Randomized Controlled Trial.

BACKGROUND: Postoperative delirium (POD) is a common complication that hinders recovery in older patients. This study aimed to compare the effects of glycopyrrolate and atropine, used to counteract the peripheral effects of neostigmine, on POD incidence in older patients undergoing laparoscopic colorectal surgery. METHODS: This single-centre, double-blind, randomized controlled trial recruited patients (aged 65-80 years) undergoing laparoscopic colorectal surgery. Patients were randomized to receive either glycopyrrolate (0.04 mg kg-1 neostigmine and 0.008 mg kg-1glycopyrrolate) or atropine (…

Neuromuscular blockade: what was, is and will be.

Non-depolarizing neuromuscular blocking agents (NMBAs) produce neuromuscular blockade by competing with acetylcholine at the neuromuscular j … t (SRBA) in clinical use. At all levels of blockade, recovery after sugammadex is faster than after neostigmine. Sugammadex potentially also has some other advantages over neostigmine that are related to neostigmine's increase in the amount of acetylcholine and the necessity of co-administering anticholinergics. However, hypersensitivity reactions, including …

Clinically useful adjuvants in regional anaesthesia.

PURPOSE OF REVIEW: In this review, emphasis is placed on adjuvant drugs that are already in clinical use. The list of adjuvants studied during the review period includes adrenaline, clonidine, ketamine, neostigmine, nondepolarizing muscle relaxants, and nonsteroidal antiinflammatory drugs. Some future aspects are considered in a couple of experimental studies on slow-release local anaesthetic formulations. RECENT FINDINGS: Adrenaline not only acts as a vasoconstrictor, it may also produce analgesia through an alpha2-adrenergic mechanism. Adrenaline may facilitate the uptake of the local anaes…

Meningeal mast cell-mediated mechanisms of cholinergic system modulation in neurogenic inflammation underlying the pathophysiology of migraine.

Growing evidence indicates that the parasympathetic system is implicated in migraine headache. However, the cholinergic mechanisms in the pathophysiology of migraine remain unclear. We investigated the effects and mechanisms of cholinergic modulation and a mast cell stabilizer cromolyn in the nitroglycerin-induced in vivo migraine model and in vitro hemiskull preparations in rats. Effects of cholinergic agents (acetylcholinesterase inhibitor neostigmine, or acetylcholine, and muscarinic antagonist atropine) and mast cell stabilizer cromolyn or their combinations were tested in the in vivo and …

Implementation of a new strategy to improve the peri-operative management of neuromuscular blockade and its effects on postoperative pulmonary complications.

Inappropriate dosing of neostigmine for antagonism of neuromuscular blockade has been associated with postoperative pulmonary complications. We evaluated the effects of a quality improvement initiative tailored to optimise the use of neostigmine in antagonising neuromuscular blockade on postoperative pulmonary complications, costs and duration of hospital stay. The quality improvement initiative consisted of: a reduction in available neostigmine aliquot sizes; a cognitive aid; an educational component; and a financial incentive for the intra-operative documentation of train-of-four measurement…

Recycling process of acetylcholine in the rat muscle controlled in a short-term memory manner.

The recycling process of acetylcholine (ACh) was studied in reference to muscle potentials evoked by sciatic stimuli in the presence of neostigmine. Wistar rats around 20 days old were anesthetized with urethane. Muscle potentials were recorded with concentric electrodes from their exposed medial gastrocnemius muscles before and after the injection of neostigmine. Attention was paid to changes in muscle potentials evoked by a set of 3 stimuli, and comparison was made mainly between successive two sets. Three potentials before neostigmine treatment were similar in amplitude. When a set of 3 sti…

Neuromuscular blocking properties of suxamethonium and decamethonium in normal and myasthenic rat muscle.

Patients with myasthenia gravis (MG) have increased tolerance to the neuromuscular blocking properties of suxamethonium (SCh) and decamethonium (C10) and exhibit a reversal of the C10-induced block by neostigmine. The effects of these drugs were compared in forelimb flexor digitorum longus muscle from normal rats and from rats with experimental autoimmune myasthenia gravis (EAMG) to investigate the similarity of EAMG to MG. The depolarization induced by 1, 5, 10 and 25 microM SCh or C10 at the motor end-plates was significantly higher in normal than in EAMG muscle. However, both normal and EAM…

Distribution of cholinesterase in canine venous system.

Cholinesterase (ChE) activity was examined in 15 veins from dogs by pharmacological and histochemical procedures. Potentiation of acetylcholine (ACh)-induced contractile response by neostigmine was observed in helical and longitudinal strips of portal, mesenteric veins and middle segment of the inferior vena cava, but not in the other 12 veins. Histochemical studies with an electron-microscope (the method of Karnovsky-Roots) revealed that ChE activity was identified as high density granules around the smooth muscle cells only in the former three veins. The visible ChE activity was abolished by…

Reversal by atropine of tetanic fade induced in cats by antinicotinic and anticholinesterase agents.

The tetanic fade induced by intraarterial administration of neostigmine or by subparalyzing doses of d-tubocurarine or hexamethonium was studied in cat anterior tibial muscle preparations. The prior administration of atropine significantly reduced the time for recovery from the tetanic fade induced by the above mentioned drugs. A similar response was seen when atropine was administered prior to neostigmine administration for reversal of paralyzing doses of d-tubocurarine. We interpret these results to show that presynaptic muscarinic receptors participate in a negative feedback affecting acety…

Nuclear medicine evaluation of motion sickness and medications on gastric emptying time.

Diminished gastric motility and lack of bowel sounds have been observed in astronauts aboard the Space Shuttle (4). In this study subjects were given scopolamine 0.6 mg with d-amphetamine 5 mg with and without neostigmine 15 mg. Neostigmine 15 mg alone was also compared with placebo for effect on gastric emptying time. In an additional test, subjects performed head movements in a rotating chair to an end-point of motion sickness short of vomiting. Ten ounces of isotonic saline containing 1 mCl of Tc 99mDPTA was ingested 2 h after the medications and immediately after rotation. The counts from …

Sugammadex and intraocular pressure: systematic review with meta-analysis and trial sequential analysis.

BACKGROUND: Elevated intraocular pressure (IOP) during emergence from general anesthesia may precipitate visual complications, particularly in patients with predisposing ocular conditions. Anticholinergics used with neostigmine for neuromuscular blockade reversal can transiently increase IOP, whereas sugammadex avoids anticholinergic use and may offer a safer ocular profile. This meta-analysis aimed to compare the effects of sugammadex versus neostigmine-anticholinergic combinations on IOP after extubation. METHODS: We systematically searched PubMed, Embase, Scopus, Web of Science, and Google…
